A retrospective study of 26 cases of supramalleolar fracture is presented. Types II and III were most common. All but the one had accompanying fibular fracture. The incidence of open fracture was 41.7% (10/24). The most common mechanism of injury was a high energy impacting force in the direction of axial compression. The supramalleolar fracture was subgrouped into four types. Twenty patients were treated by closed reduction and cast immobilization for an average of five months, six by open reduction and internal fixation, and two by skeletal traction in the early course of treatment. Of the 2

Bilateral Posterior Fracture Dislocation (BPFD) of the shoulder is an uncommon but not rare presentation. We describe etiology, diagnostics, treatments and outcome and give a historic review and with a current approach of this pathology. We reviewed 55 cases (110 shoulders), mostly men (49/55), with a mean age of 49.2 years, mean follow up 21.9 months, mean delay until diagnose of 12.7 days (0-112 days), with a seizure as the cause in 80.0% . Other causes are electrocution, trauma or other. If the mechanism is not clear an epileptic insult should be considered the cause until proven otherwise.

BACKGROUND: Zygomatic arch is a bony arch constituting the lateral midface, which consists of 25% of all midface fractures. There are a number of ways to evaluate the extent of zygomatic arch fracture. Some authors have reported successful treatment outcomes using ultrasound (U/S). To add to the previous methods, we have considered ways to accurately display the location of the fracture line while using U/S with 23 gauge needle marking. We introduce our method, which provided satisfactory results for reduction using a portable U/S, and it can evaluate the fracture line simultaneously when redu

BACKGROUND: The American Academy of Orthopaedic Surgeons (AAOS) recently developed an Appropriate Use Criteria (AUC) for pediatric supracondylar humerus fractures (PSHF). The AUC is intended to improve quality of care by informing surgeon decision making. The aim of our study was to cross-reference the management of operatively treated PSHF with the AAOS-published AUC.
METHODS: The AUC for PSHF include 220 patient scenarios, based on different combinations of 6 factors. For each patient scenario, 8 treatment options are evaluated as "appropriate," "maybe appropriate," and "rarely appropriate.

Atlantoaxial rotatory fixation (AARF) is a rare pathological condition predominantly seen in pediatric populations. In adults, particularly when accompanied by C2 facet fractures, it poses unique diagnostic and therapeutic challenges. While conservative treatment may be effective in select cases, the presence of mechanical and/or ligamentous instability often necessitates surgical intervention. We report the case of a 73-year-old male who sustained traumatic AARF with an associated fracture of the right C2 superior articular facet following a bicycle collision. Initial conservative management,

INTRODUCTION: Bosworth fracture dislocations of the ankle are rare injuries of the ankle caused by extreme external rotation of the supinated foot where the proximal fibula fracture fragment is posteriorly dislocated and entrapped behind the posterior-lateral ridge of the tibia. This case series aims to document three such cases treated in our institution over a nine year period. We also provide a review of 129 cases in the existing literature.
MATERIALS AND METHODS: Medical records and relevant radiographs for each patient were analysed and collected from the time of presentation till the po

The aim of this study is to compare splint versus cast immobilization for maintaining alignment following closed reduction of distal 1/3 radius and both-bone forearm fractures. We performed a retrospective review of patient records between 5 and 14 years old with a distal 1/3 radius or radius and ulna fracture requiring reduction. A cost comparison was also performed using facility costs for materials. Reduction was maintained with acceptable alignment in most cases (94%). Although a sugar-tong splint slightly maintained fracture alignment better, this was not significant. Cost analysis favore

BACKGROUND: Radial neck fractures account for 1% of all pediatric fractures and 5% to 10% of pediatric elbow fractures. The mechanism of injury is typically a fall with the elbow in hyperextension and the forearm in supination. A valgus force compresses the radial head against the capitellum, causing a radial neck fracture. Displaced radial neck fractures are difficult to treat and account for a disproportionate number of bad outcomes, including malunion, nonunion, and osteonecrosis. The preferred treatment is closed reduction and fixation, as open reduction is associated with an inordinately

CASE: We report an unusual case of a closed humeral shaft fracture, with no vascular compromise, resulting in brachial compartment syndrome. Our patient was successfully treated with fasciotomy and external fixation, followed by staged open reduction and internal fixation and skin grafting.
CONCLUSION: Although uncommon in the upper arm, suspicion for compartment syndrome should remain high for patients with unrelieved pain and swelling after humeral shaft fracture. Serial physical examination and invasive monitoring can assist in the diagnosis. Fasciotomy and staged fracture repair can yield

The term naso-orbital fracture refers to the backward displacement of the nasal bones into the interorbital space. The fracture is most commonly the result of traumatic contact with the dashboard in automobile accidents. The trauma results in soft tissue eyelid deformities, which may also involve nasolacrimal disruption and life-threatening intracranial injury. Early management consists of closed reduction of the nasal fracture when feasible. Late management, which is more common, consists of transnasal wiring, restoration of the nasolacrimal apparatus, and contouring of the nasal bones.

Extension-type supracondylar humerus (SCH) fractures are among the most common pediatric elbow fractures. Treatment algorithms for Gartland type I and type III fractures have been well established, yet managing type II SCH fractures has remained a topic of debate in the pediatric orthopaedic community. It is important to explore the controversies surrounding both the classification and treatment of type II SCH fractures. Radiographically, these fractures are difficult to reliably classify to guide decision making. In addition, the decision to pursue closed reduction and casting versus closed (

Closed reduction with cast immobilization is the preferred method of treatment for most fractures of the forearm and distal radius in children and adolescents. Nonunion of these fractures is exceptionally rare, and remodeling restores alignment for minor incomplete reductions. Most closed reductions can be performed in the emergency department with the patient sedated or with regional anesthesia. However, reduction in an operating room with the patient under general anesthesia lowers the threshold for surgical stabilization to avoid the need for repeat reductions under general anesthesia. PURPOSE OF THE STUDY: The most frequent type of elbow fracture in children is by far the supracondylar fracture. This type of fracture also raises the greatest risk of nerve injury. We wanted to study the clinical and prognostic features of nerve injuries in children presen-ting supracondylar fractures of the humerus.
MATERIAL AND METHODS: We analyzed the files of 55 children with nerve lesions identified among 1 180 files on supracondylar fractures in children. Most presented Lagrange and Ribault stage IV fractures. Nerve injuries involved the radial nerve (n=28), medial nerve (n=20), and ul

BACKGROUND: Nasal bone fractures are frequently encountered in clinical practice. Although fracture reduction is simple and correction requires a short operative time, low patient satisfaction and relatively high complication rates remain issues for many surgeons. These challenges may result from inaccuracies in fracture recognition and assessment or inappropriate surgical planning. Findings from immediate postoperative computed tomography (CT) scans and those performed at 4 to 6 weeks postoperatively were compared to evaluate the accuracy and outcomes of nasal fracture reduction.

BACKGROUND: The aim of this study was to assess the predictive factors of delayed union in pediatric forearm fractures. Few previous reports have approached this complication, but contrary to these studies we excluded cases with known pathogenic factors and included in the sample cases treated with conservative methods.
METHODS: This is a case-control study of all pediatric shaft forearm fractures presenting delayed union in a minimum follow-up period of 12 months, from 2003 through 2009, in a hospital covering a health area. Exclusion criteria were fracture dislocations, infection, suboptima

A 27 year-old man was involved in a high-energy-impact motor-vehicle accident and sustained multiple injuries including a medial subtalar fracture dislocation. The dislocation of the subtalar joint was reduced by closed means, but a large, comminuted, displaced posterior process fracture remained. Open reduction and internal fixation through a posteromedial approach with mobilization of the neurovascular bundle was used to restore congruity to the subtalar joint.

PURPOSE: A traction table is recommended for lower limb fractures, while it is unavailable for amputees to immobilize the ipsilateral foot to remain stationary and reduce the fracture. For these patients, our rapid reductor can be applied to guarantee stable fixation and optimal reduction, allowing satisfactory implant positioning. This study aims to evaluate the prognosis of amputee patients with lower limb fractures treated by minimally invasive techniques that employ a rapid reductor to reduce the fracture.
METHODS: Between 2013 and 2014, 11 cases of amputees suffering from a lower limb fr
