BACKGROUND: Early initiation and exclusive breastfeeding for 6 months reduces infant morbidity and mortality and can positively impact on cognitive function. In Uganda, exclusive breastfeeding for 6 months is recommended but many women introduce alternative feeds early. Interventions to scale-up peer support provision for exclusive breastfeeding are limited. We explored the barriers, facilitators and solutions to scaling-up of peer counselling support for exclusive breastfeeding in Uganda.
METHODS: A qualitative study was conducted in Mbale District and Kampala City between April and July 201

BACKGROUND: Most low-income countries have prioritised implementing national health insurance schemes (NHIs) as a solution to reducing the high out-of-pocket expenditures on health and enhancing access to healthcare, especially among informal sector workers. However, their perceptions remain unexplored in Uganda. This study aimed to assess the awareness and perceptions of the informal sector workers towards the proposed NHIs in Iganga and Mayuge districts, Uganda.
METHODOLOGY: A cross-sectional mixed-methods study was conducted in the Iganga and Mayuge districts of eastern Uganda between Apri

The 2014-2016 West African Ebola epidemic was devastating in many respects, not least of which was the impact on healthcare systems and their health workforce. Healthcare workers-including physicians, clinical officers, nurses, midwives, and community health workers-serve on the front lines of efforts to detect, control, and stop the spread of disease. Risk mitigation strategies, including infection prevention and control (IPC) practices, are meant to keep healthcare workers safe from occupational exposure to disease and to protect patients from healthcare-associated infections. Despite ongoin…

In Nigeria, there is a persistent reluctance to provide adolescents with contraceptives and this is based largely on the premise that the culture does not support pre-marital sexual activity. However, several studies have revealed that a large number of adolescents are experiencing early sexual debut, some as early as 13 years. Furthermore, mortality resulting from termination of unwanted pregnancies is on the increase among Nigerian adolescents. The aim of this study is to assess the attitudes and practice of health workers in Ibadan regarding adolescent contraception. A structured questionna

BACKGROUND: Co-infection with tuberculosis adversely affects the quality of life of HIV infected individuals and additionally, HIV testing among TB patients gives an opportunity for prevention and treatment of HIV infection. TB-HIV coordination activities are therefore a good public health intervention.
METHODS: It was a three-year Public Private Partnership Project, implemented in four districts of Maharashtra, to improve access to public health facilities through community awareness and motivating referrals. Outreach workers were engaged to motivate patients attending Integrated Counselling

The aim was to investigate and analyze the social, health, and economic impact of the Covid-19 pandemic on Indigenous communities in the province of Catamarca (Argentina), while also identifying the various strategies developed by these communities to address this phenomenon. The research was conducted between December 2021 and December 2022, with an exploratory nature and a predominantly qualitative methodological design. During the fieldwork, 15 in-depth interviews were conducted with community authorities, community members, territorial technicians, and health workers, complemented by 30 st

BACKGROUND: Across much of the developed world, the number of older people in custody has been increasing, which presents challenges for correctional systems due to the complex social, medical and mental health needs of this subgroup, especially those living with dementia. The present study therefore aimed to increase insight into the extent to which older people in custody are (a) potentially living with dementia and (b) receiving appropriate supports/services (particularly, with respect to community reintegration).
RESULTS: Cross-sectional data were drawn from a sample of 29 older people in

BACKGROUND: Evidence from literature has established that tracing lost to follow-up clients is an effective strategy for complementing other mechanisms for infectious disease control like human immunodeficiency virus (HIV), tuberculosis, and other diseases such as Ebola. As a long-standing successful public health method of optimizing acceptance and/or adherence to infectious disease treatment tracing lost to follow-up clients is usually carried out by manually investigating individuals who absconded or are absent from treatments designed to manage and/or promote their health status. BACKGROUND: World Health Organization (WHO) and Thailand's national policy both advocate for recovery-oriented, community-based mental healthcare. Village Health Volunteers (VHVs) in Thailand have limited involvement in mental health services despite their pivotal role in Thai primary healthcare, especially in rural settings. This study aims to engage stakeholders to identify and prioritise key areas for VHVs' role expansion, stigma reduction, training needs, and common mental health conditions, thereby enhancing VHVs' contributions to recovery-oriented mental healthcare in rural Thai communit

Stigma against people living with HIV in China is common, not only among the general public but also among Chinese health-care providers, impacting their ability to provide quality HIV treatment and care. HIV stigma may be particularly common in rural areas of China, where village doctors (i.e., "barefoot doctors") have limited medical training yet are charged with providing frontline care for much of China's rural population. Therefore, the present study aims to use communication and behavioral theory to better understand Chinese village doctors' beliefs about HIV, including their willingness

BACKGROUND: In Guatemala, childhood vaccination rates, particularly in Indigenous communities, are low due to inaccessible information and mistrust. Using a human-centered design (HCD) process with community partners, we developed CLAVE (Culturally and Linguistically Appropriate Vaccine Education), a video series on routine and human papillomavirus (HPV) immunization in Kaqchikel, K’iche’, and Spanish. Content was distributed via social media and through community health workers (CHWs). This study describes the instructional design process and assesses implementation feasibility through commun…

Homelessness among older individuals is increasing and women experiencing homelessness have been previously shown to have poorer health outcomes than their male counterparts. To address these concerns, the Bridges to Elders (BTE) program was developed to improve health and social outcomes for older women experiencing or at risk for homelessness. BTE consisted of a nurse practitioner (NP) and community health worker (CHW) dyad who provided intensive case management services for women 55+ with housing instability. This evaluation used a pretest/posttest design to examine three main outcomes from

The utilization of Village Health Workers (VHWs) was studied in a rural area of The Gambia 3 years after the introduction of a village-based Primary Health Care (PHC) programme. Of 23 children who died from conditions treatable at village level, only five were first seen by the VHW. Fourteen were seen elsewhere in the region by staff more qualified than the first tier workers. The implications of this pattern of utilization on the lack of impact of VHWs on mortality are discussed. Only half of the non-fatal illnesses were attended to by VHWs. Reasons for this included such factors as lack of k

Industrial hog operation (IHO) workers face a range of occupational hazards, including exposure to zoonotic pathogens such as livestock-associated antimicrobial-resistant Staphylococcus aureus and swine-origin influenza viruses with epidemic or pandemic potential. To better understand this population's occupational exposure to zoonotic pathogens, we conducted a community-driven qualitative research study in eastern North Carolina. We completed in-depth interviews with ten IHO workers and used thematic analysis to identify and analyze patterns of responses. Workers described direct and indirect
