Cryptococcus neoformans is the causative agent of cryptococcosis and cryptococcal meningitis, which are serious pathological conditions affecting up to 10% of patients with AIDS. In this paper we present results magnetic resonance imaging and spectroscopic metabolites (1H MR) in brain cryptococcosis. In 1 HMR spectroscopy we find decreased metabolic ratios to nonsaturated water (H2O) signal N-acetylaspartate (NA/H2O, creatine (Cr/H20), choline (Cho/H2O). We show increased mioinositol to H2O ratio. Spectroscopy results suggest about massive neuronal injury and accompanying gliosis in brain cryp…

In the present study we investigated the role of nitric oxide (NO) in the effector mechanisms of host defense against Cryptococcus neoforman … fter the inhibition of NO production because all the animals infected and treated with AG died from cryptococcosis before 20 days postinfection (p.i.). These results indicate that NO is a crucial molecule in the effector mechanisms in this infection model.

We report two cases of cryptococcosis in patients with Waldenstrom's macroglobulinaemia and chronic lymphocytic leukaemia that responded to prolonged therapy with systemic amphotericin and flucytosine. Cryptococcosis, although more common in those with impaired cell mediated immunity, should also be considered as a complication in patients with impaired antibody responses.

Concomitant with the decline in CD4+ T-cells seen as human immunodeficiency virus (HIV) infection progresses, the prevalence of opportunistic mycoses increases dramatically. This article reviews selected recent advances in our understanding of the immunology, molecular epidemiology and treatment of fungal infections in patients infected with HIV. For cryptococcosis, studies are reported on how HIV infection affects the immune response to Cryptococcus neoformans and, conversely, how stimulation with C. neoformans induces HIV production from latently HIV-infected cells. In addition, studies are …

A 28-year-old patient with AIDS was found at autopsy to have disseminated cryptococcal infection involving the lungs, spleen, lymph nodes, kidneys, gastrointestinal tract, thyroid, bone marrow, and liver. Despite widespread organ dissemination the patient did not have clinical or pathological evidence of meningitis. Microscopic examination of the brain showed cryptococci limited to the brainstem and basal ganglia; microglial nodules with multinucleate giant cells, a histological hallmark of HIV-1 encephalitis, were shown to contain Cryptococcus neoformans. This feature may suggest a form of sy

BACKGROUND: Published opinion has generally favored amphotericin B over fluconazole as initial therapy for acquired immunodeficiency syndrome-associated cryptococcosis, although data that support this recommendation are limited.
METHOD: Retrospective review of 30 consecutive patients with acquired immunodeficiency syndrome-associated cryptococcosis seen at a single institution over a 1-year period and given fluconazole, 400 mg/d, as initial therapy.
RESULTS: No patient died within the first 30 days of therapy, and none of the 14 patients who died within 1 year had clinically detectable infec…

As experience with the acquired immunodeficiency syndrome has grown during the last decade, it has become important to recognize interactions between the opportunistic infections and malignancies that complicate the course of patients with acquired immunodeficiency syndrome. Our recent experience as described in this paper reveals that Kaposi's sarcoma and cryptococcal infection can associate in such a manner. The first reported case of oral Cryptococcus neoformans infection within a Kaposi's sarcoma lesion in a patient with acquired immunodeficiency syndrome is described. Based on the clinica

Initially the lesions were on his … with skin lesions resembling molluscum contagiosum, to diagnose mycotic infections, and especially cryptococcosis. Cutaneous cryptococcosis was, in this case, the first symptom of AIDS.

A 51-year-old immunosuppressed man presented with ulcerations of the abdominal wall. Biopsy of the ulcer margin was necessary for the diagnosis of cutaneous cryptococcosis. Since a variety of opportunistic organisms can present with non-specific cutaneous lesions, these infections must be ruled out by tissue diagnosis of all immunosuppressed patients. A review of 22 additional cases is included together with electron microscopic findings.

Cryptococcosis predominantly affects immunocompromised individuals, but cases have been reported in immunocompetent hosts. Pulmonary involvement often presents nonspecific features that could resemble lung malignancy. We present a case of a 49-year-old immunocompetent, HIV-negative, non-smoker man with progressive respiratory symptoms and significant weight loss. Imaging revealed multiple pulmonary nodules with mild metabolic activity. Non-invasive diagnostic studies were inconclusive. Histopathological examination of lung tissue demonstrated encapsulated yeasts consistent with Cryptococcus sp…

INTRODUCTION: Cryptococcosis is a severe invasive fungal infection with limited therapeutic options beyond fluconazole-based regimens. Isavuconazole, a broad-spectrum triazole antifungal, has emerged as a potential alternative, although clinical data supporting its use remain scarce. We aimed to evaluate the real-world effectiveness and safety of isavuconazole in patients with different forms of cryptococcosis.
METHODS: A retrospective observational study was conducted at a tertiary-care hospital, including patients with cryptococcosis who received isavuconazole at any treatment phase. Standa…

Pulmonary cryptococcosis is one of the pulmonary fungal infections caused by the inhalation of Cryptococcus neoformans. Pulmonary cryptococcosis shows various radiological findings and sometimes a pulmonary nodule on CT. We experienced three cases of pulmonary cryptococcosis showing a solitary nodule in patients with previous malignancy. All three cases show a newly emerging or growing nodule. Two cases whom FDG-PET was tested showed accumulation of FDG. All three cases underwent pulmonary resection and were pathologically diagnosed with pulmonary cryptococcosis. It is difficult to distinguish…

BACKGROUND: Cryptococcosis, while well-documented in immunocompromised hosts, remains a rare complication in myasthenia gravis (MG) patients undergoing immunosuppressive therapy.
METHODS: We reported three cases of cryptococcal infection in MG patients diagnosed via cryptococcal antigen (CrAg) testing and/or histopathology, coupled with a comprehensive literature review of 14 additional cases that highlights the diagnostic and therapeutic challenges in this population. We also explored potential immunodeficiency by whole-exome sequencing (WES).
RESULTS: The comibined cohort (median age 57.1 …

Cryptococcal meningitis remains a significant opportunistic infection in people living with human immunodeficiency virus (HIV), occurring with advanced immunosuppression. We report the case of a 59-year-old HIV-type-1-positive man, previously with an undetectable viral load and a CD4+ count of 504 cells/μL, who presented with altered mental status and right-sided ptosis. He had missed follow-up for one year, and repeat testing showed a CD4+ count of 218 cells/μL and a viral load of 258,000 copies/mL. Brain MRI revealed leptomeningeal enhancement of the basal cisterns. Cerebrospinal fluid analy
