[Neuromuscular effects of vecuronium d-tubocurarine and succinylcholine in malnourished rat's diaphragm].

The pharmacodynamics of neuromuscular blocking agents in malnutrition have not been well delineated. In the present study, the neuromuscular effects of vecuronium (Vb), d-tubocurarine (d-Tc) and succinylcholine (SCh) in malnutrition were investigated. Male Sprague-Dawley rats aged 7 weeks were divided into malnourished and control groups. One fifth of their usual daily consumption was allowed in the malnourished group for 10 days. The animals were anesthetized and phrenic nerve-hemidiaphragm preparations were dissected. The preparations were placed in an organ bath containing oxygenated Krebs'…

Diaphragm function in patients with cervical cord injury or prior poliomyelitis infection.

Diaphragm function was evaluated in cervical cord injury patients (CCI), 1-3 years after injury (CCI(1-3)) and 10 years or more after injury (CCI(> or = 10)), as well as in a group of prior poliomyelitis infection patients (PPI), requiring intermittent positive pressure ventilation (IPPV) for about 6 h per night. Measurements included transdiaphragmatic pressure swings (delta Pdi) at rest, during maximal inspiratory efforts against closed airways (delta Pdi(max)) and during sniff manoeuvres (ie maximal inhalation through the nose, delta Pdi(sniff)), vital capacity normalized to age and height …

Histochemical and mechanical properties of diaphragm muscle in morbidly obese Zucker rats.

The purpose of the present study was to evaluate the effects of chronic mass loading produced by obesity on the structural and functional characteristics of the diaphragm in lean and obese Zucker rats. The trapezius muscle served as an internal control. The studies were carried out on 17 lean (303 +/- 24 g) and 16 obese (698 +/- 79 g) Zucker rats. We observed that the diaphragms from obese animals were restructured such that the overall contribution of type I and IIa fibers was significantly increased. As a consequence of this remodeling, overall diaphragm thickness was selectively greater in …

Differential activation within costal diaphragm during rapid-eye-movement sleep in cats.

Simultaneous recordings of the diaphragmatic electromyogram (EMG) were made from two separate regions of the costal diaphragm in six normal cats. The diaphragmatic activities were always synchronous and the amplitudes and rates of rise were similar during slow-wave sleep. In contrast, during natural rapid-eye-movement (REM) sleep, different activity was often present in the two leads. These differences were in the time of onset and offset, as well as in the amplitude and spike patterns, and occurred in approximately 5-20% of the diaphragmatic bursts averaged over the entire REM sleep period. W…

Lipidic mitochondrial inclusions in experimentally-induced in vitro myopathy of mouse diaphragm.

1. Treatment of murine skeletal muscle with the Ca2(+)-channel agonist, Bay K 8644, induces myopathic changes including configurational changes of the mitochondria. 2. During the progression of the myopathy the mitochondria apparently engulf neighboring lipid droplets. 3. Mitochondrial changes are most marked when damage to the muscle is most severe, are prevented under conditions that should reduce Bay K 8644-induced Ca2(+)-influx, and are not modified by various maneuvers designed to prevent the generation of free radical species.

Occult pleural involvement in stage III ovarian carcinoma: role of diaphragm resection.

In a 2-year period 12 diaphragmatic resections were performed on 11 patients with invasive metastases from ovarian carcinoma without significant complications, via transabdominal approach in 10 instances and thoracoabdominal incision in 1. Full-thickness penetration of the diaphragm by tumor occurred in all instances without prior evidence of pleural involvement (occult Stage IV). Six patients had primary disease (Group A) and 5 had recurrent disease (Group B). Ten patients who underwent excision of nonpenetrating diaphragmatic metastases were analyzed for comparison (Group C). Mean diameters …

Effects of undernutrition on diaphragm fiber size, SDH activity, and fatigue resistance.

The influence of prolonged nutritional deprivation on the succinate dehydrogenase (SDH) activity and cross-sectional areas of individual fibers in the rat diaphragm and deep portion of the medial gastrocnemius (MGr) muscles was determined. Fatigue resistance of the diaphragm was measured by means of an in vitro nerve-muscle strip preparation. Fiber SDH activity and cross-sectional area were quantified by means of an image processing system. Diaphragm fatigue resistance was significantly improved in the nutritionally deprived (ND) group. In both muscles, nutritional deprivation resulted in a si…

Arm-diaphragm synkinesis: electrodiagnostic studies of aberrant regeneration of phrenic motor neurons.

Arm-diaphragm synkinesis may occur after injury to the proximal portion of the brachial plexus or cervical nerve roots. Regenerating axons of phrenic motor neurons are misdirected to supply limb muscles. Electrodiagnostic investigations of three patients with symptoms referable to upper brachial plexus or cervical roots revealed motor units in either biceps or triceps muscles that discharged during inspiration. These units did not fire during forced or passive expiration or Valsalva maneuver. Activation of these units could not be achieved by volitional contraction of the appropriate arm muscl…

Traumatic rupture of diaphragm: surgical reconstruction with special reference to delayed closure.

Thirteen cases of diaphragmatic rupture following blunt trauma or gunshot wounds are presented. In 10 cases the diagnosis of diaphragmatic rupture was made immediately following the injury, and the defect was closed by primary diaphragmatic suture. In three cases, the diagnosis was delayed for 3 to 16 years after the initial trauma. In all of them, abdominal organs such as the colon or liver had migrated into the thoracic cavity. One of them had acute intestinal obstruction and died following several unsuccessful operations. The remaining two patients required plastic repair of the diaphragmat…

A Rare Case of Spindle Cell Rhabdomyosarcoma of the Diaphragm in Adult with Thoracoabdominal Extension.

Rhabdomyosarcoma (RMS) is an uncommon malignant soft-tissue tumor, with diaphragmatic spindle cell RMS being extremely rare in adults. A 41-year-old male patient had progressive breathlessness and right-sided pleuritic chest pain. Imaging and biopsy that includes X-ray, computed tomography, magnetic resonance imaging, histopathology, and immunohistochemistry established a large thoracoabdominal spindle cell RMS. Early diagnosis, careful imaging, and combined treatment were important, and they helped increase knowledge of this unusual entity.

Diaphragm function in patients with asthma and healthy controls: A cross-sectional study.

BACKGROUND: Asthma is a chronic respiratory disease characterized by airway inflammation and variable respiratory symptoms. While peripheral muscle deconditioning is known to affect lung function and exercise tolerance, the role of respiratory muscle dysfunction, particularly the diaphragm, remains underexplored. OBJECTIVE: We aim to evaluate potential differences in diaphragm function in patients with asthma. METHODS: We conducted a prospective, observational study comparing diaphragmatic function of 50 patients with asthma and 50 healthy controls. Two independent operators evaluated diaphr…

The hyperaemic and metaboreflex response of the diaphragm during fatiguing diaphragmatic work in humans.

During inspiratory pressure-threshold loading (ITL), the inspiratory muscle metaboreflex (IMM) increases respiratory muscle blood flow, but its effect on diaphragm blood flow (Q̇di) and fatigue in humans, is unclear. Females exhibit an attenuated IMM compared to males during diaphragmatic loading, suggesting less diaphragm blood flow impedance and metaboreflex activation. We hypothesized that diaphragm hyperaemia would relate positively to metaboreflex sensitivity (i.e. mean arterial pressure change; ΔMAP) and fatigue development (i.e. transdiaphragmatic twitch pressure change; ΔPdi,tw), and b…

A Case of Persistent Hiccups Induced by an Enlarging Hepatic Cyst Underneath the Diaphragm.

Although hiccups are regularly self-limited and rarely life-threatening, prolonged hiccups are bothersome, which can significantly decrease the quality of life. Here, we report a case of persistent hiccups coexisting with an enlarging liver cyst situated just below the diaphragm in a patient with autosomal dominant polycystic kidney disease (ADPKD). No other underlying etiologies related to the symptoms were suspected. The cyst was difficult to drain surgically and the patient continued with symptomatic treatment. Although rare, hepatic cysts should be considered a potential cause of prolonged…

Diaphragm Ultrasound in Different Clinical Scenarios: A Review with a Focus on Older Patients.

Diaphragm muscle dysfunction is increasingly recognized as a fundamental marker of several age-related diseases and conditions including chronic obstructive pulmonary disease, heart failure and critical illness with respiratory failure. In older individuals with physical frailty and sarcopenia, the loss of muscle mass and function may also involve the diaphragm, contributing to respiratory dysfunction. Ultrasound has recently emerged as a feasible and reliable strategy to visualize diaphragm structure and function. In particular, it can help to predict the timing of extubation in patients unde…
