Transient cavity dilatation during supine exercise bicycle stress testing: mechanistic insights.

BACKGROUND: Cavity dilatation is occasionally observed during supine bicycle exercise stress echocardiography (SBSE). The underlying mechanisms are poorly understood. AIMS: This study aimed to characterise patients with left ventricle (LV) cavity dilatation and a decrease in the left ventricular ejection fraction (LVEF) during SBSE. METHODS: A total of 653 patients who underwent SBSE were evaluated. Those with exercise-induced cavity dilatation (defined as increased cavity size and a decrease in LVEF) were evaluated (n = 29). A control group (n = 37) of patients with a hypertensive response …

Facet joint dilatation as a radiological indicator of vertebral segmental microinstability.

BACKGROUND: Chronic low back pain (LBP) is a major healthcare burden that reduces quality of life and productivity. Vertebral segmental microinstability is considered a contributing factor in LBP, while radiological signs reflecting biomechanical dysfunction, including disc degeneration, Modic changes, muscle fatty infiltration, and facet joint (FJ) dilatation, remain insufficiently defined as clinical markers. METHODS: This was a retrospective cohort study involving 168 adults with chronic LBP. Patients underwent clinical examination including passive lumbar extension (PLE), supine-sit, ilio…

Antegrade transcystic balloon dilatation in emergency laparoscopic cholecystectomy with MRCP validation.

PURPOSE: Endoscopic retrograde cholangiopancreatography (ERCP) remains the predominant treatment of common bile duct stones but single-stage laparoscopic management via antegrade transcystic balloon dilatation (ATBD) offers a compelling alternative. All prior studies validate success solely by intraoperative fluoroscopic cholangiography, with no prospective trial employing postoperative magnetic resonance cholangiopancreatography (MRCP). METHODS: This single-centre prospective study enrolled 100 consecutive patients with confirmed CBD stones requiring emergency laparoscopic cholecystectomy (L…

Congenital Biliary Dilatation With Pre-Existing Diffuse Hepatolithiasis: A Challenging Case.

Although postoperative intrahepatic stones are known to occur after surgery for congenital biliary dilation, preoperative diffuse hepatolithiasis is uncommon. We present a challenging case of a 14-year-old female with a Todani type IVa choledochal cyst complicated by numerous stones. She presented with abdominal pain, obstructive jaundice, and pancreatitis. To stabilize her condition, she was initially treated with endoscopic biliary and pancreatic stenting. Subsequently, laparoscopic radical surgery was performed. Although many stones were removed intraoperatively, complete clearance remained

Neurovascular large artery dilatation increases the risk for Alzheimer's disease pathology.

Alzheimer's disease and related dementias are increasing globally, highlighting the urgent need to clarify disease mechanisms to improve diagnosis and treatment. Vascular alterations are a major pathological feature of Alzheimer's disease. Beyond the established contributions of small vessel disease and large artery atherosclerosis, our group has shown that dilatation of large cerebral arteries is associated with an increased risk of dementia and Alzheimer's pathology. The most severe manifestation of this non-atherosclerotic vascular phenotype is dolichoectasia, characterized by abnormal enla…

Mitral Regurgitation Associated with Mitral Annulus Remodeling and Left Atrial Dilatation.

BACKGROUND: Atrial functional mitral regurgitation (MR) involves functional MR with left atrial (LA) dilatation and mitral annulus (MA) remodeling. The relationship between LA dilatation and MA remodeling, and the mechanism of MR associated with MA remodeling, are unclear and were investigated in this study. METHODS: This single-center, cross-sectional retrospective study prospectively enrolled 97 consecutive patients with atrial fibrillation (AF) referred for three-dimensional transesophageal echocardiography. Mitral valve echocardiographic data of 18 AF patients with moderate or severe MR (…

Incidentally detected pancreatic duct dilatation on ultrasound and role of MRI.

OBJECTIVE: The clinical significance of incidentally detected pancreatic duct (PD) dilatation at ultrasound (US) without a visualized underlying cause is unclear. We aimed to assess the role of subsequent MRI (including MRCP) and to identify US imaging and laboratory findings predictive of underlying pancreaticobiliary malignancy at the time of initial US. MATERIALS AND METHODS: Patients with incidentally detected PD dilatation at ultrasound from 2011 to 2019 that had an ensuing MRI were included. Based on MRI results patients were divided into three groups: malignant pancreaticobiliary cause…

A new clinical classification of congenital biliary dilatation - HUAXI CBD classification.

BACKGROUND: Current biliary dilatation (BD) classifications are complex and based on cases including secondary BD, leading to unclear distinctions. Notably, congenital and secondary BD differ in etiology, symptoms, and prognosis. OBJECTIVE: To propose a more concise and more suitable classification of congenital biliary dilatation (CBD), and exploring the feasibility and effectiveness of this classification in diagnosis and treatment. METHODS: Based on the preoperative imaging data of patients with CBD admitted to the Department of Pediatric Surgery of West China Hospital of Sichuan Universi…

Radiographic findings in dogs with 360 degrees gastric dilatation and volvulus.

Gastric dilatation and volvulus (GDV) is a life-threatening emergency that requires urgent intervention. Radiographic features associated with 360-GDV in dogs have not been investigated. The aim of this retrospective observational study is to describe radiographic features and clinical variables in dogs affected with 360-GDV and to report agreement rates between different radiologists. We also report the sensitivity and specificity of radiographs to diagnose 360-GDV in dogs. Confirmed 360-GDV cases were retrieved, and the radiographic findings were compared with dogs presenting with gastric di…

Acute Gastric Dilatation: A Retrospective Case Series from a Single Institution.

INTRODUCTION: Acute gastric dilatation (AGD) is a massive distension of the stomach caused by the accumulation of gas, gastric secretions, or food material. AGD is a radiological diagnosis with no clear etiopathogenesis and is often misdiagnosed owing to a lack of clear diagnostic criteria and physician awareness. CASE PRESENTATION: In this case series, we describe the clinical presentations and outcomes of 4 patients with AGD. Three (75%) of the patients were female, and one (25%) was male. The patients' ages ranged from 53 to 84 years, with an average age of 73.5 years. Abdominal pain, naus…

Early and late outcomes of congenital biliary dilatation in pediatric patients.

BACKGROUND: This study aimed to reveal the early and late postoperative complications and outcomes after surgery for congenital biliary dilatation (CBD) by reviewing cases over the past 40 years. METHODS: We retrospectively evaluated 59 patients with CBD who underwent radical surgery for complications and outcomes, based on medical records. Early complications were defined as those requiring treatment within 5 years of the initial operation. Late complications were defined as those treated more than 5 years later. RESULTS: The median age at the first surgery was 37 months. Regarding biliary …

Factors predicting surgical difficulties in congenital biliary dilatation in pediatric patients.

BACKGROUND: The effects of disease classification and the patient's preoperative condition on the difficulty of performing a laparotomy for pediatric congenital biliary dilatation (CBD) have not been fully elucidated. METHODS: The present study retrospectively analyzed 46 pediatric CBD laparotomies performed at the study center between March 2010 and December 2021 and predictors of operative time. The patients were separated into a short operative time group (SOT) (≤ 360 min, n = 27) and a long operative time group (LOT) (> 360 min, n = 19). RESULTS: The preoperative AST and ALT values were …

Updated Information on Gastric Dilatation and Volvulus and Gastropexy in Dogs.

Gastric dilatation and volvulus (GDV) is an acute, life-threatening syndrome of dogs, particularly large and giant breeds with a deep chest conformation. Rapid diagnosis is important for prompt initiation of stabilization therapy and surgical correction. Negative prognostic factors include hyperlactatemia not responding to fluid therapy, gastric perforation, or need for splenectomy or gastric resection. Gastropexy is essential for all dogs affected by GDV, following correct gastric repositioning. Prophylactic gastropexy for at-risk breeds can be performed via minimally invasive laparoscopic su…

Patent Foramen Ovale and Ascending Aortic Dilatation Causing Platypnea-Orthodeoxia Syndrome.

Platypnea-orthodeoxia syndrome (POS) is an underdiagnosed clinical syndrome characterized by dyspnea (platypnea) and hypoxemia (orthodeoxia) in the upright position that resolves when recumbent. POS is often due to an underlying right-to-left shunt. Four broad mechanisms for the shunt have been described: intracardiac shunts, intrapulmonary shunts, hepatopulmonary syndrome, and pulmonary ventilation-perfusion mismatch. A 68-year-old male with a past medical history of chronic obstructive pulmonary disease (COPD), obstructive sleep apnea, ascending aortic dilation (3.9 cm), myelofibrosis, and s
