We present the unusual case of a 72-year-old woman whose chest X-ray showed an abnormal left hilar shadow. A pulmonary angiogram revealed an aneurysm in the pulmonary artery with a diameter of 55 mm that extended from the main pulmonary trunk to its bifurcation. Mild pulmonic stenosis with a systolic pressure gradient of 18 mmHg across the pulmonic valve was recognized. Mild dilatation of the ascending aorta was also present. A comparative study of congenital biliary dilatation with pancreatico-biliary maljunction in 15 children and 16 adults was undertaken. Several mechanisms have been suggested for the development of hypoxemia in cirrhosis. A few patients of portal hypertension due to non cirrhotic liver disease with cyanosis have also been reported earlier. We report probably the first documented case of portal hypertension with portal cavernoma and a normal liver, who had intrapulmonary vascular dilatations leading to hypoxemia and cyanosis. Our case suggests that changes leading to hypoxemia can occur due to portal hypertension alone, in the presence of a normal liver.

A congenital choledochal cyst, type Ia according to Todani's classification, was discovered fortuitously in a 54-year old man by abdominal sonography. The patient had no biliary symptoms, and liver tests were normal. Excision of the cyst was performed because of the theoretical risk of cholangiocarcinoma. Recovery was uneventful.

Cecal dilatation and volvulus are common entities in dairy cattle, whereas cecal torsion is not often encountered. These diseases have been related to excessive volatile fatty acid production. Obstruction of the remainder of the large colon may be simple or strangulating in nature. These obstructions are relatively rare. On the basis of a recent case of fundic gastric heterotopia in the gallbladder, the principal, possible embryogenetic hypotheses of the malformation are described and discussed. We present two patients with intractable constipation, in which investigation revealed a huge rectal ectasia without apparent other anomaly. Findings at operation are similar to those observed in the focal ectasia of the terminal bowel that we have already described in some cases of low anal deformities. As long as the physiopathology of this anomaly is not elucidated, complete amputation of the rectum seems to be the treatment of choice.

Two hundred sixty patients with severe chronic constipation were studied by rectal biopsy, barium enema, and anorectal manometry. None had any medical, neurologic, or anatomic cause for their complaint. All biopsies showed presence of ganglion cells and absence of hypertrophied nerves. Roentgenography without contrast media and sialography, as well as the basic facts concerning the radiodiagnostic of the salivary gland, are all discussed with regard to the most current equipment, examination techniques, the positioning of patients, and the problems involved in contrast media. The technique used in more than 4,000 sialograms is recommended. The diagnostic value of this method is illustrated by means of representative patient series: sialolithiasis, inflammatory disorders, and tumors. The limitations of the method are demonstrated and complementary examinations shown.

We encountered two children with chronic idiopathic myocarditis accompanied by marked right ventricular dilatation, who died of progressive right heart failure. A definitive diagnosis was made by histological examination of the myocardium at autopsy. The autogenous saphenous vein is the preferred conduit for repair of occluded arteries in the limb and coronary arteries. The aneurysmal dilatation of the vein graft is a unusual complication in the long-term period. A case of aneurysmal formation of the vein graft for the subclavian aneurysm 5 years after grafting with classic Behçet disease was reported. The biliary ultrasound scans of 132 patients having a laparotomy for jaundice have been reviewed to assess the role of ultrasound in the selection of patients for surgery.
