Use of flow-diverting stents in the treatment of ruptured intracranial artery dissections.

BACKGROUND: Flow-diverting stents have rapidly become widespread as the preferred treatment for intracranial aneurysms. They function by optimizing blood flow within the parent vessel to reduce shear stress in the aneurysm, facilitating thrombus formation within the aneurysm, and providing a scaffold for endothelialization and vascular remodeling. Early literature indicates this mechanism also applies to the treatment of ruptured intracranial aneurysms and unruptured intracranial artery dissections, demonstrating favorable occlusion rates. We highlight a novel application of flow-diverting ste

[A case of cholangiocarcinoma with hepatomesenteric trunk and intestinal malrotation treated with pancreaticoduodenectomy].

The patient was a 61-year-old man. He was underwent pancreaticoduodenectomy for common bile duct cancer. The preoperative dynamic CT revealed that hepatic artery was branched from the superior mesenteric artery, run through pancreatic surface caudal to cranial and flowed into liver. And this vessel had several branches into pancreas. In operation, to ensure blood flow to the liver, hepatic artery was detached from pancreas by cutting off branches into pancreas. In addition, intestinal malrotation was also confirmed (Treitz ligament was not formed and small intestine was present in the right si

[Discussion about cervical incision of goiter in mediastinum posterior and its indications].

OBJECTIVE: To explore the feasibility of cervical approach for goiter in posterior mediastinum. METHOD: According to the mechanism that goiter filed into posterior mediastinum and the dissection of thyroid gland and mediastinum, we designed the following surgery principles (1) From top to bottom. (2) Find out recurrent nerve at the place where it enters larynx, then dissect recurrent nerve as long as possible and protect it carefully, meanwhile, search thyroid vessels along recurrent nerve. (3) To avoid and uncontrollable serious hemorrhage in the operation, all normal and aberrant blood vess

Development and Validation of a Measure of Psychological Distress After Spontaneous Coronary Artery Dissection: The Spontaneous Coronary Artery Dissection Distress Inventory.

BACKGROUND: Spontaneous coronary artery dissection (SCAD) is a highly distressing cause of acute myocardial infarction. No SCAD-specific distress measure currently exists, with researchers relying on generic measures of anxiety, depression, and distress that fail to capture the psychological impacts unique to SCAD. We developed and validated the SCAD Distress Inventory (SDI) to provide a condition-specific distress measure. METHODS: The SDI was developed through a multistep process. First, 48 SCAD-related distress items were generated from focus groups with SCAD survivors (N=30) and relevant

Rare complex anatomical variation of right pulmonary vessels and bronchi: a case report.

BACKGROUND: The patterns of bronchopulmonary vascular bifurcation within the lung exhibit considerable diversity. To perform safe and accurate anatomical pulmonary resections, an understanding of the anatomy of the pulmonary vessels and bronchi, including variations, is of utmost importance to general thoracic surgeons. CASE DESCRIPTION: We performed a 3-dimensional (3D) computed tomography (CT) reconstruction of the pulmonary vessels and bronchi for a 66-year-old female patient. From the 3D reconstruction, we were able to observe clearly that this patient had complex variations in the right

History of occlusive cerebrovascular disease. II. After Moniz, with special reference to surgical treatment.

Angiography and related procedures are discussed. Ophthalmodynamometry, recording of bruits and temporal pulse, and thermography have been mentioned. Transient ischemic attacks (TIAs) and some causes are described: embolic obstruction, followed by absorption of the clot or its removal in the blood, with symtomatic recovery; hemodynamic crisis resulting from lowering of systemic blood pressure by hemorrhage due to trauma, gastrointestinal bleeding, and other causes with recovery following blood replacement. Special types of occlusive disease include fibromuscular hyperplasia of the carotid, dis

Intraoperative Evaluation of Bladder Perfusion Using Indocyanine Green Fluorescence Imaging During Total Pelvic Exenteration After Interruption of Blood Flow From the Internal Iliac Vessels.

BACKGROUND: In lateral lymph node dissection (LLND) for locally advanced or recurrent rectal cancer, concomitant resection of the internal iliac vessels is sometimes required. Because the bladder receives its primary blood supply from branches of the internal iliac artery, concerns arise regarding bladder perfusion when these vessels are resected. However, the extent to which bladder perfusion depends on the internal iliac system remains unclear. METHODS: Between 2020 and 2023, 25 patients with locally advanced or recurrent rectal cancer who underwent total pelvic exenteration (TPE) with bila

The blood supply of the inferior nasal concha (turbinate): a cadaveric anatomical study.

The blood supply of the lateral nasal wall acquires a great deal of interest for many reasons especially for the treatment of clinical conditions such as epistaxis that sometimes needs surgical interventions and endoscopic sinus and skull base surgeries that requires a good choice of endonasal flaps including inferior nasal concha (commonly named as inferior turbinate by clinicians) flap to close the dural defects. Successful treatment of epistaxis and closure of such defects depends on detailed anatomical knowledge of lateral nasal wall vasculature and a vital flap with good blood supply, res

Strategy of laparoscopic surgery for colon cancer of the splenic flexure: a novel approach.

BACKGROUND: The splenic flexure (SF) anatomy is complex due to multiple vessels, surrounding organs, layers, and irregular adhesions [1-3]. METHODS: Our laparoscopic approach involves a lateral-to-medial approach to the left-sided transverse mesocolon (TM), a medial-to-lateral approach to the left mesocolon (LM), and take-down of the remnant SF. First, the omental bursa is opened and its posterior wall and the anterior layer of the TM are dissected along the pancreas, where a gauze is placed. The TM is spread cephalad. A window in the TM is opened in the gauze seen through the TM. If necessar

Endovascular repair of bilateral common iliac artery aneurysms following open abdominal aortic aneurysm repair with preservation of both hypogastric arteries using commercially available stent grafts.

Endovascular treatment of aneurysmal disease has become the predominant form of repair for all aneurysms. Some areas continue to pose specific challenges to stay within the general tenets of successful repair, mainly achieving adequate seal without sacrificing other arterial pathways. Following aortic aneurysm repair, the common iliac arteries can continue to have aneurysmal degeneration. We present a case of bilateral common iliac artery aneurysms that presented 9 years after open repair of an infrarenal abdominal aortic aneurysm in conjunction with an extensive aortic dissection. These were

Metabolomic Signatures of Spontaneous Isolated Superior Mesenteric Artery Dissection: Indole Derivatives and Lysophospholipids as Novel Biomarkers.

Spontaneous isolated superior mesenteric artery dissection (SISMAD) is a rare but increasingly recognized vascular disorder that can lead to intestinal dysfunction with severe complications. Currently, no plasma biomarkers, particularly metabolomics-based markers, have been identified for SISMAD. To identify novel plasma biomarkers and potential therapeutic targets for SISMAD and its associated intestinal dysfunction, we analyzed plasma samples from 30 patients with SISMAD and 30 healthy controls. Blood was collected within 72 h of diagnosis. Ultra-high-performance liquid chromatography couple

Recurrent Cerebral Embolism Caused by Sluggish Retrograde Flow from the Residual False Lumen of the Aortic Arch after Type A Aortic Dissection.

A residual false lumen after type A aortic dissection repair may rarely serve as an embolic route to the cerebral circulation. A woman in her 60s underwent ascending aortic replacement for Stanford type A aortic dissection, leaving a residual patent dissection involving the supra-aortic branches and the descending aorta. Despite receiving anticoagulant and antiplatelet therapies, the patient developed recurrent multifocal cerebral infarctions. Transesophageal echocardiography (TEE) demonstrated an entry tear in the descending aorta communicating from the true lumen to the false lumen. Flow bec

Thoracic Endovascular Repair for Retrograde Type A Aortic Dissection: True Lumen Device Sizing.

BACKGROUND: Optimal device sizing for retrograde type A aortic dissection during thoracic endovascular aortic repair (TEVAR) remains challenging because the true lumen at the proximal landing zone is frequently compressed and elliptical. This study evaluated the clinical outcomes and investigated how true lumen morphology influences device selection. METHODS: Between October 2012 and December 2022, 34 retrograde type A aortic dissection patients underwent TEVAR. Patients with major cardiac or neurological complications were excluded. We retrospectively analyzed the clinical and anatomical out

[Arterial hypertension--multicirculus vitiosus vasorum].

AIM: The aim of this review is to address documents and a number of studies on hypertension published in the last years in order to assess their contribution to our expanding knowledge of arterial hypertension. DISCUSSION: Arterial hypertension is not defined by symptoms and signs but by numbers of blood pressure values. Arterial hypertension is vascular disease (vascular risk factor) of many vascular diseases (atherosclerosis; arteriolosclerosis/arteriolonecrosis/arteriolocalcinosis; arterial thrombosis; arterial embolism; arterial thromboembolism; arterial dissection; complicated arterial a

Mid-term morphological changes in Frozenix.

OBJECTIVES: The frozen elephant trunk is widely used for aortic arch aneurysms and DeBakey type I and III aortic dissections. However, radial force and spring-back of the stent can cause morphological changes, contributing to stent-induced new entry. This study aimed to evaluate postoperative morphological changes of frozen elephant trunk and provide biomechanical insights. METHODS: This retrospective study included 107 patients who underwent Frozenix placement between April 2014 and December 2023. Clinical outcomes and morphological changes of frozen elephant trunk were assessed. Morphologic

Cerebral malperfusion resolution after repair of acute DeBakey type I dissection with a novel hybrid prosthesis: early results of the PERSEVERE Study†.

OBJECTIVES: Patients undergoing hemiarch repair for acute DeBakey type I dissection (ADTI) are high risk for postoperative stroke, especially if cerebral malperfusion is present preoperatively. We sought to evaluate whether the AMDS Hybrid Prosthesis (AMDS), a bare metal stent designed to promote positive aortic remodelling and prevent distal anastomotic new entry tears, may improve neurological outcomes of patients with ADTI presenting with cerebral malperfusion. METHODS: PERSEVERE enrolled patients presenting with ADTI and malperfusion at 26 sites in North America. Among 93 enrolled patient
