The insidious presentation of intramesocolic perforation in diverticulitis has been reviewed, and two cases of intramesocolic perforation associated with pneumopylephlebitis have been reported. Review of the two previously reported cases of pneumopylephlebitis associated with diverticular perforation suggests that these may have been intramesocolic perforations as well. Exploratory laparotomy is clearly indicated in cases of pneumopylephlebitis. In patients with recurrent sepsis without a probable source, a water-soluble contrast enema is recommended. If conservative measures fail, an explorat

Pseudo-neoplastic sigmoid diverticulitis, a rare clinical form of pseudo-tumoural sigmoiditis, is characterised by the difficulty of its clinical, radiological, endoscopic and even per-operative diagnosis. None of these various phases in study may suffice to eliminate the possibility of an associated carcinoma. The diagnosis may be made by opening the operative specimen, but this is too late in time. The appearance of the bladder, rather than misleading the diagnosis, may provide arguments in favour of the benign nature of these pseudo-tumours.

PURPOSE: Recently, treatment of Hinchey III diverticulitis by laparoscopic peritoneal lavage has been questioned. Moreover, long-term outcomes have been scarcely reported. Primary outcome was to determine the recurrence rate of diverticulitis after a successful laparoscopic peritoneal lavage in Hinchey III diverticulitis. Secondary outcomes were identification of associated risk factors for recurrence and elective sigmoidectomy rate.
METHODS: A retrospective cohort study in a tertiary referral center was performed. Patients with Hinchey III diverticulitis who underwent a successful laparoscop

A healthy 65-year-old woman presents to the emergency department with a 12-hour history of sudden-onset severe lower abdominal pain. This is her first episode. She reports nausea, vomiting, and anorexia. Her last colonoscopy was at age 60, and was normal, except for diverticulosis of the sigmoid colon. Physical examination is significant for fever, tachycardia, and generalized abdominal pain with rebound tenderness. Pertinent laboratory findings include a leukocytosis and metabolic acidosis. A CT scan is obtained and is consistent with freely perforated diverticulitis, including a thickened si

Enterovesical fistular is an abnormal communication between the intestine and the bladder. It represents a rare complication of intestinal diverticulitis, colorectal malignancy, bladder cancer, inflammatory bowel disease, radiotherapy, and trauma. The most common etiology is diverticular disease. A 70-year-old man came to our hospital due to frequent urinary tract infection, dysuria, pneumaturia and fecaluria. Sigmoidoscopy revealed a large diverticulum with impacted stool at the sigmoid colon. When the scope was inserted into the site, the patient complained of severe urgency and pneumaturia.

OBJECTIVE: To evaluate the need for prophylactic antibiotics in automated peritoneal dialysis (APD) patients undergoing flexible colonoscopy.
PATIENTS AND METHODS: A total of 93 patients on automated peritoneal dialysis (APD) undergoing diagnostic colonoscopy were enrolled in a prospective, randomized study. Patients were randomized into 2 age- and sex-matched groups; group A (46 patients) with intraperitoneal (IP) ceftazidime prior to colonoscopy and group B (47 patients) without prophylactic antibiotics. The relations between peritonitis and different parameters were analyzed.
RESULTS: Of

INTRODUCTION: Colonic diverticulosis is the most common structural abnormality of the colon in developed countries, with an increasing global prevalence. Approximately 20-25% of affected individuals develop symptoms, collectively referred to as diverticular disease. Given its wide clinical spectrum, evolving pathophysiological insights and growing disease burden, updated guidance is essential.
METHODS: This International Consensus, developed by 32 experts from 14 countries through a structured Delphi process based on the PICO framework and GRADE methodology, provides evidence-based recommenda

INTRODUCTION: Colonic actinomycosis is an uncommon chronic infection associated with granulomatous inflammation resulting multiple abscesses and sinuses. Common modes of presentation include weight loss, malaise, abdominal pain, and abdominal mass, which might mimic neoplasia.
CASE PRESENTATION: A 60-year-old female presented with painful lump in right iliac fossa (RIF) for 3 weeks and acute complete bowel obstruction for 3 days. Examination revealed hard, irregular and fixed lump in the RIF. Ultrasonography of abdomen and pelvis showed ileocecal mass while contract enhanced computed tomograp

BACKGROUND AND AIM: Either clipping or band ligation will become the most common endoscopic treatment for colonic diverticular bleeding (CDB). Rebleeding is a significant clinical outcome of CDB, but there is no cumulative evidence comparing reduction of short-term and long-term rebleeding between them. Thus, we conducted a systematic review and meta-analysis to determine which endoscopic treatment is more effective to reduce recurrence of CDB.
METHODS: A comprehensive search of the databases PubMed/MEDLINE and Embase was performed through December 2019. Main outcomes were early and late rebl

PURPOSE: To evaluate the effectiveness of a therapeutic barium enema as a treatment for colonic diverticulum bleeding, by using a standard concentration as a diagnostic examination.
METHODS: We retrospectively analyzed 68 cases of the patients admitted to our hospital with colonic diverticular bleeding between October 2012 and September 2017. We evaluated the following items: (1) the presence/absence of a previous history of diverticular bleeding, (2) the use of medications (anticoagulants, nonsteroidal anti-inflammatory drugs, and antiplatelet drugs), (3) the location of bleeding, (4) the pr

BACKGROUND: Colonic perforation is associated with abysmal outcome. The aims of our study were to review the surgical outcome of patients with perforated colon and to identify factors predicting peri-operative complications.
METHODS: A retrospective review of all patients who underwent surgery for colonic perforation from January 2003 to August 2008 was performed. Patients with iatrogenic or traumatic perforation were excluded. The severity of abdominal sepsis was graded using the Mannheim peritonitis index (MPI).
RESULTS: A total of 129 patients, with median age of 65 years (22-97 years), f

BACKGROUND: Acute diverticulitis with perforation and peritonitis is a serious complication affecting up to 12% of patients. Peritonitis is classified into purulent (Hinchey III) or fecal (Hinchey IV) categories. The standard treatment has traditionally involved emergency surgery, such as bowel resection with or without anastomosis or Hartmann's procedure, both of which carry high morbidity and mortality risks.
METHODS: In 2008, laparoscopic peritoneal lavage (LPL) emerged as a less invasive alternative for treating purulent peritonitis. This article outlines the LPL technique, emphasizing pa

PURPOSE: The surgical interventions of diverticulitis vary according to its grade and severity. There is a controversy about the best of these different surgical procedures. We aimed to systematically review and meta-analyze randomized controlled trials (RCTs) comparing outcomes and complications between different surgical approaches for acute diverticulitis and its complications.
METHODS: Nine electronic databases including PubMed, Scopus, and Web of Science were searched for RCTs comparing different surgical procedures for different grades of diverticulitis. The risk of bias was assessed us

A retrospective study of 310 patients from 1978 to 87 showed emergency operations for peritonitis, acute obstruction (5%) and massive hemorrhage (3%) in 22% of the patients and various procedures for abscesses, such as delayed emergency in 14% and elective resections in 41%. Conservative treatment was given to 23% of patients with acute diverticulitis without further symptoms. During followup 68% had no symptoms, while 7% required resection for complications. The death rate for diffuse peritonitis has fallen: from 33 to 16% with Hartmann operation, and for perforations without and with abscess
