BACKGROUND: Kidneys from deceased donors with acute renal failure (ARF) are generally not accepted for transplantation because of the expected poor outcome. This prospective study examined the utilization of kidneys from donors with ARF for transplantation and the outcomes.
METHODS: Fifty-five kidneys from donors with ARF were transplanted. The outcome was compared with concurrent and matched 55 recipients of standard criteria donor (SCD) kidneys and 55 expanded criteria donor (ECD) kidneys. ARF kidneys were accepted from donors aged <50 years, a negative history for kidney disease, and a neg

In the past decade liver transplantation has become the standard therapy for terminal liver failure. An increasing organ shortage, specific regional allocation systems within the Eurotransplant area and the lack of an efficient system to identify donors have resulted in decreasing numbers of liver transplants. Approximately 20% of patients are dying on the waiting list, a list that has exploded in numbers and waiting time in the past 3 years. In particular children and small adults are put at a disadvantage since standard donors have standard size livers. Two options to solve this dilemma are

With the growing number of patients in need of liver transplantation, there is a need for adopting new and modifying existing allocation policies that prioritize patients for liver transplantation. Policy should ensure fair allocation that is reproducible and strongly predictive of best pre and post transplant outcomes while taking into account the natural history of the potential recipients liver disease and its complications. There is wide acceptance for allocation policies based on urgency in which the sickest patients on the waiting list with the highest risk of mortality receive priority.

IMPORTANCE: Advanced age in unrelated donors (URDs) is a well-established risk factor in allogeneic hematopoietic cell transplant (HCT), leading registries to prioritize younger donors. However, this paradigm relevance is uncertain in the era of posttransplant cyclophosphamide (PTCy) for graft-vs-host disease (GVHD) prophylaxis, a strategy increasingly adopted for its effectiveness. Clarifying this association is crucial for optimizing donor selection and potentially expanding the donor pool.
OBJECTIVE: To determine whether the association of older URD age with overall survival differs betwee…

BACKGROUND: Skin grafts, frequently used for ear elevation in ear reconstructions, may suffer from color mismatches, scar hypertrophy, secondary contraction, and auricular sulcus depth differences. To determine the most suitable donor area, we compared the contralateral postauricular area, groin, and scalp as potential donor sites.
METHODS: Since 2008, we have used three donor sites for obtaining skin grafts used in ear reconstructions. Full-thickness groin skin, split-thickness grafts from the scalp, and contralateral postauricular and groin composite full-thickness skin grafts were used in

The problems encountered by the author in 70 cases of toe-to-hand transfers are considered under three main headings: (1) operative problems (2) post-operative problems and (3) functional problems. The operative problem of preserving the integrity of the donor vessels is related to the varying patterns of vasculature of the foot. Bony fixation of the donor toe to the recipient site and the selection of veins of the right size and length are the other main operative problems. The post-operative problems are concerned with the problem of survival of the transplanted toe and these include venous

Background: The deep inferior epigastric artery perforator (DIEP) flap is currently the most widely used method for autologous breast reconstruction. Its primary advantage over the transverse rectus abdominis muscle (TRAM) flap is the reduction in donor-site morbidity, as it preserves the integrity of the abdominal muscles and motor nerves. Importantly, each patient's unique vascular anatomy requires an individualized approach to perforator selection and the surgical technique. Objective: We aimed to minimize donor-site morbidity and refine the perforator selection strategy in delayed DIEP fla

BACKGROUND: The 2018 heart allocation policy led to increased use of venoarterial extracorporeal membrane oxygenation (VA-ECMO) as a bridge to heart transplantation (HT). Despite known poorer outcomes with ECMO bridging, no established risk stratification tool exists to guide candidate selection. This study analyzed risk factors for 1-year mortality after VA-ECMO bridged to HT to provide a risk stratification guide for decision-making.
METHODS: The United Network for Organ Sharing registry was used to identify all adults bridged to HT with VA-ECMO from January 2000 to June 2024. Recipient and

BACKGROUND: Blood donors are often regarded as healthier than the general population, a phenomenon described as the "healthy donor effect". This perception arises from strict donor selection criteria and routine pre-donation screening, yet these evaluations focus largely on hemoglobin, vital signs, and a short medical history, thereby providing only a partial assessment of donor health. As cardiovascular disease (CVD) remains the leading global cause of mortality, assessing cardiovascular risk factors in blood donors provides an opportunity to evaluate whether this group is genuinely healthier…

Intrahepatic cholangiocarcinoma (iCCA) is the second most common primary liver tumor and usually associated with a poor oncological prognosis. The current gold standard is the surgical resection of the tumor with subsequent adjuvant therapy. However, in case of irresectability e.g. in case of liver cirrhosis, a palliative treatment regime is conducted. This report demonstrates the case of an irresectable iCCA in liver cirrhosis due to primary sclerosing cholangitis (PSC) treated by living-donor liver transplantation (LDLT) facilitated by minimal invasive donor hepatectomy. No postoperative comp

BACKGROUND AND AIMS: Liver transplantation has evolved from a treatment restricted to patients with end-stage liver disease to a therapeutic option for selected patients with primary and metastatic liver malignancies. This review explores the rapidly expanding field of transplant oncology, highlighting its role in hepatocellular carcinoma, colorectal liver metastasis, neuroendocrine liver metastasis, and intrahepatic and perihilar cholangiocarcinoma. Emphasis is placed on strategies to broaden eligibility, optimize donor organ use, and improve oncologic outcomes.
APPROACH AND RESULTS: We synt

BACKGROUND: Evolution in breast oncology has increased the use of breast-conserving therapy and expanded the role of locoregional autologous reconstruction. While bilateral reduction is common for patients with large, ptotic breasts, those with small to moderate symmetric breasts often prefer unilateral volume restoration. This study demonstrates a free-style approach to flap selection and design for partial and total breast reconstruction, emphasizing intraoperative decision-making and preservation of muscle and nerve function.
METHODS: A retrospective review was performed on patients underg

Bilateral metacarpal hands, if not treated properly, leave a patient without prehensile ability in both hands. Since 1990, six patients with bilateral metacarpal hands caused by accidents have undergone reconstruction with multiple-toe transplantations. Four or five toes were used for each patient, with a total of 27 toes transplanted to the hands. There was no toe loss. One nonunion in a middle-finger reconstruction was treated successfully with bone grafting. Secondary operations for functional improvement included one joint fusion and one flexor tendon tenolysis. Only one patient required e

We investigated factors associated with pregnancy approximately 60 days after transfer of Japanese Black (JB) embryos into JB, Holstein (HO), and their crossbred (JB×HO) heifers. A total of 26,196 embryo transfer (ET) records collected between 2008 and 2023 were analyzed. Recipient-, embryo-, and donor-related factors were initially evaluated using Akaike information criterion (AIC)-based variable selection, and the retained fixed-effect structure was subsequently evaluated in a mixed-effects logistic regression model with donor as a random intercept. The overall pregnancy rate was 70.7%. Preg

Post-reperfusion syndrome (PRS) remains a critical complication in living donor liver transplantation (LDLT). While sevoflurane is recognized for its organ-protective properties, the impact of the donor's anesthetic choice on recipient outcomes has not been clearly established. This study evaluated whether donor sevoflurane anesthesia reduces the incidence of PRS in recipients compared to desflurane. We retrospectively analyzed 5006 adult LDLT recipients whose donors received either sevoflurane or desflurane. Propensity score matching was employed to minimize selection bias, resulting in 941 m

PURPOSE: Donor age is a critical determinant of severe primary graft dysfunction (PGD) after heart transplant (HT), yet the safety and efficacy of using younger donor hearts for adult recipients remains incompletely characterized. Given ongoing organ shortages, an understanding of the relationship between pediatric donor age and post-transplant outcomes is essential to inform donor selection and optimize utilization.
METHODS: The International Consortium on PGD database retrospectively enrolled adult HT recipients and their donors from 14 centers across the United States, Canada, and Europe. …

BACKGROUND: Using older living donors is a strategy to expand the kidney donor pool, but concerns remain regarding mid-term graft performance. We evaluated 5-year patient and graft outcomes after living donor kidney transplantation from donors aged 60 years and older versus younger donors.
METHODS: We conducted a single-center retrospective cohort study of 279 living donor kidney transplants. Recipients were grouped by donor age (<60 years, n = 237; ≥60 years, n = 42). To reduce selection bias, inverse probability of treatment weighting was applied. Outcomes included longitudinal recipient es

BACKGROUND: Alloimmunization against red blood cell (RBC) antigens is an uncommon but clinically significant complication following allogeneic hematopoietic stem cell transplantation (HSCT). While ABO and Rh incompatibilities are often considered in donor selection, other RBC antigens are rarely assessed.
CASE PRESENTATION: We report the case of a 50-year-old male with high-risk acute myeloid leukemia who underwent unrelated HSCT. Over 2 years posttransplant, the patient developed an anti-U alloantibody, despite a previously U-positive phenotype. Immunohematological investigations revealed a …

Due to shortage of donor, kidney transplants (KTs) from donors with acute kidney injury (AKI) are expanding. Although previous studies comparing clinical outcomes between AKI and non-AKI donors in KTs have shown comparable results, data on high-volume analysis of KTs outcomes with AKI donors are limited. This study aimed to analyze the selection trends of AKI donors and investigate the impact of AKI on graft failure using the United states cohort data. We analyzed a total 52,757 KTs collected in the Scientific Registry of Transplant Recipient (SRTR) from 2010 to 2015. The sample included 4,962
