This study confirmed post-mortem ethanol concentrations in pericardial fluid and bone marrow aspirate in comparison with those in the blood in medicolegal autopsy cases (n = 140, within 48 h post-mortem). The specimens were examined by head-space gas chromatography/mass spectrometry. Epinephrine plays a controversial role in accidental hypothermia (<30°C). We report its use in the advanced life support of a 13-month-old white girl with pulseless electrical activity and 25°C core body temperature after 32 minutes of submersion in a fast-running Swiss mountain stream at 8°C. Two doses of epinephrine (10 μg/kg) were given in the field, followed by 12 doses (10 μg/kg) and an infusion of 0.1 μg/kg per minute during rewarming. Spontaneous circulation returned at 29.5°C after 2.5 hours of cardiopulmonary resuscitation. Neurologic long-term outcome was excellent. We conclude that

Spontaneous deliveries into toilet bowls always carry a special forensic significance. Forensic pathologist has to differentiate between a genuine precipitate labour and concealed birth or a neonaticide. Circumstances are challenging when a nullipara claims misapprehended precipitate labour. We report a similar case where a primigravidous unmarried girl delivered in a lavatory pan misjudging labour pains as that of bowel evacuation. Detailed obstetric history, postnatal maternal behaviour and visit to scene of incidence resolved the unnecessary charges of neonaticide on the embarrassed mother.

The December 2002 COM. A 19-year-old healthy male fell into stagnant water of the intercostal waterway (salt water of South Florida), following a jet ski accident. He sustained minor superficial injuries but engulfed significant quantities of water and sediment. A few days later he developed bifrontal headaches, vomiting, a stiff neck and a temperature of 102 degrees F. A CT scan on admission without contrast was negative. The CSF had markedly elevated white count but bacterial and fungal cultures were negative. He became progressively lethargic. On the fifth day he developed seizure activity.

The case reported here demonstrates the need to consider myositis ossificans after acute injury to the CNS, irrespective of the nature of the trauma. This latter by no means needs to be direct trauma--as the present case shows. When the condition does occur, the time needed for rehabilitation may be significantly lengthened.

This review of the literature on the association between accidents and alcohol abuse presents data from a variety of studies performed in many of the industrial nations of the world. These studies demonstrate a consistent relationship between alcohol abuse and accidents. The family physician should be aware of this association and utilize it in the identification and treatment of his alcoholic patients.
