CASE: A 9-year-old girl presented with a painful stiff elbow consequent to an injury she had sustained 4 months ago. The radiographs were suggestive of a type 2 Monteggia equivalent injury. She underwent an open reduction of the radial neck fracture and proximal ulna extension osteotomy to restore radiocapitellar alignment. At one-year follow-up, the child had a painless elbow with a good functional outcome.
CONCLUSION: Monteggia lesions can be frequently missed and result in significant morbidity. Obtaining appropriate radiographs for a comprehensive understanding of the injury and proper su

CASE: A right hand dominant 10-year-old girl presenting with an open right distal humerus fracture dislocation involving complete medial column bone loss and a >50% trochlea defect was reconstructed with a size-matched, fresh distal humerus osteochondral allograft. Successful bony integration and functional elbow range of motion were achieved.
CONCLUSION: A paucity of literature exists to guide the treatment of pediatric distal humerus fractures with significant bone loss. Despite complications associated with osteochondral allografts, they provide a reasonable treatment option to preserve el

Coronal shear fractures of the nonossified humeral trochlea are extremely rare in skeletally immature children. This fracture is difficult to diagnose accurately on routine radiographs. In addition, the mechanism of injury and appropriate treatment methods are unknown. We present the case of a 7-year-old boy who sustained an isolated coronal shear fracture of the nonossified humeral trochlea. Internal fixation of the fracture was performed with 2 K-wires. Two and a half years after surgery, the patient had pain-free active range of motion of the elbow from 0° to 130° with full forearm rotation

Coronoid fractures are not easily accessed surgically. Despite the majority of cases of coronoid fracture responding to conservative treatment, limited range of motion or instability can develop after such treatment. Therefore, unstable coronoid fractures should be treated surgically to improve clinical outcome. Several approaches and treatment methods have been used for coronoid fractures. The authors report the clinical characteristics, results, and outcomes of 10 cases of Regan-Morrey Type I, Type II, and Type III coronoid fractures treated using a novel arthroscopic technique.

BACKGROUND: The most widely used surgical approach to treat radial head fractures is through the Kocher interval. However, the extensor digitorum communis (EDC) splitting approach is thought to allow easier access to the anterior half of the radial head, which is more commonly fractured. The aim of this cadaveric study was to compare the osseous and articular surface areas visible through the EDC split and the Kocher interval.
METHODS: Four approaches were used in fresh frozen cadaveric upper extremities: EDC splitting (n = 6), modified Kocher (n = 6), extended EDC splitting (n = 6), and exte

AIMS: The purpose of this study was to evaluate the relationships between the degree of injury to the medial and lateral collateral ligaments (MCL and LCL) and associated fractures in patients with a posterolateral dislocation of the elbow, using CT and MRI.
METHODS: We retrospectively reviewed 64 patients who presented between March 2009 and March 2018 with a posterolateral dislocation of the elbow and who underwent CT and MRI. CT revealed fractures of the radial head, coronoid process, and medial and lateral humeral epicondyles. MRI was used to identify contusion of the bone and collateral

BACKGROUND: The use of a metal radial head prosthesis to help stabilize an elbow with traumatic instability is appealing because internal fixation of multifragment, displaced fractures of the radial head is susceptible to either early or late failure. The newer modular prostheses are easier to size and implant, but their effectiveness has not been investigated, to our knowledge.
METHODS: Twenty-seven patients in whom a radial head replacement with a modular metal spacer prosthesis had been performed to treat traumatic elbow instability were evaluated with use of the Mayo Elbow Performance Ind

In caring for athletes, the physician must be able to accurately diagnose and appropriately treat all forms of elbow injuries. Traumatic injuries to the elbow are common in the athlete. The late cocking phase of throwing produces tremendous valgus stress on the elbow that can lead to medial epicondyle avulsion fractures in adolescents or rupture of the medial ulnar collateral ligament in skeletally mature overhead throwers, such as baseball pitchers and javelin throwers. Common traumatic elbow injuries suffered by athletes, surgical techniques for operative repair of these injuries, as well as

Closed reductions of displaced coronal fractures of the capitellum were successfully obtained for nine patients. The method involves spontaneous reduction by allowing the elbow to fully extend under anesthesia and then gradually flexing the elbow while distracting the elbow joint. Distraction allows the radial head to capture the capitellar fragment in the joint rather than push it proximally. The elbow is immobilized at 90 degrees for 3 weeks. Gentle active motion is then started. Closed reduction is often or usually obtained, but open reduction and internal fixation are performed in those ca

BACKGROUND: Heterotopic ossification around the elbow can result in pain, loss of motion, and impaired function. We hypothesized that a single dose of radiation therapy could be administered safely and acutely after elbow trauma, could decrease the number of elbows that would require surgical excision of heterotopic ossification, and might improve clinical results.
METHODS: A prospective randomized study was conducted at three medical centers. Patients with an intra-articular distal humeral fracture or a fracture-dislocation of the elbow with proximal radial and/or ulnar fractures were enroll

INTRODUCTION: The aim of this study was to determine whether an above-elbow cast (AEC) is better than a below-elbow cast (BEC) at maintaining the initial reduction in the orthopaedic management of a distal radius fracture (DRF).
METHODS: It is a prospective randomized study carried out in a single emergency trauma department. There were 72 patients older than 55 years of age (55-96) with a distal radius fracture treated orthopaedically. They were randomized into two groups: group B (AEC 32 patients) and group A (BEC 40 patients). Randomization was done by a computer program. Four subgroups we

BACKGROUND: It is controversial whether the collateral ligaments should be repaired primarily for capitellum fractures with ligamentous injury. This research was conducted to summarize the current evidence for this issue.
METHODS: Databases, including Medline, Cochrane library and EMBASE, were searched from their establishment to December 31, 2024 for clinical articles on capitellum fractures. The reference lists of the relevant studies were also checked successively. The general information including first author, publication time, location, the number of cases, treatment for the capitellum
