Gastric Peristalsis Control by Mono Situ Electrical Stimulation: a Preliminary Study.

BACKGROUND: This study was initiated 3 years ago when antral gastric stimulation was first used successfully to reduce free feeding in swine. METHODS: Three swine weighing 45 kg each were implanted with one subserosal bipolar electrode, positioned in the antrum, close to the pylorus, at the anterior side of the lesser gastric curvature. RESULTS: During 4 hours of kethamine anesthesia we paced the stomach by various patterns of electrical stimulation and obtained both forward and backward peristalsis, as well as gastric peresis. CONCLUSION: Variations in antral electrical stimulation produce ch

Effects of electrical stimulation to the soft palate on snoring and obstructive sleep apnea.

Obstructive sleep apnea (OSA) is a disorder and has been associated with social and physical problems. Many treatments of OSA have been proposed with various levels of success. Seven patients with histories of snoring and OSA participated in this pilot study to evaluate the effects of electrical stimulation to the soft palate. Each patient slept with a palatal appliance that delivered a weak electrical stimulus to the soft palate on activation. A 3 milliampere stimulus in the range of 9 to 10 volts was found to be effective in terminating snoring without causing patient arousal. The effects of

Electrical stimulation of the trigeminal nerve root for the treatment of chronic facial pain.

Between March 1990 and December 1992, 23 patients with chronic intractable facial pain due to various forms of injury to the trigeminal nerve or nerve root underwent implantation of an electrical stimulating system to treat their pain. All patients had failed previous extensive pain treatment efforts. A monopolar platinum-iridium electrode was implanted on the trigeminal nerve root via percutaneous puncture of the foramen ovale. All patients experienced at least 50% reduction in pain intensity during a period of trial stimulation and underwent internalization of the electrode and connection to

Suppression of motor cortical excitability by electrical stimulation over the cerebellum in ataxia.

We studied the effect of electrical stimulation over the cerebellum on electromyographic responses evoked by magnetic stimulation over the cerebral motor cortex in 41 normal volunteers and 32 patients with ataxia due to various disorders. In all the normal subjects, stimulation over the cerebellum significantly reduced the size of electromyographic response in the first dorsal interosseous muscle evoked by magnetic cortical stimulation, when the cerebellar stimulus preceded the cortical stimulus by 5, 6, and 7 msec. This suppression was absent or reduced in ataxic patients who had atrophy of t

Rat model for the study of penile erection: pharmacologic and electrical-stimulation parameters.

We report the use of a modified rat model for the study of the mechanisms of penile erection. In 92 Sprague-Dawley rats, the cavernous nerve was stimulated with different pulse intensities and frequencies, and the intracavernous pressure, time to maximal pressure and total duration of tumescence were measured. A maximal response was elicited at 20 pulses per second (pps) and 1.5 mA. Using this as 100%, we determined the relative pressure responses obtained with other frequencies: 5 pps, 57.3% (p = 0.007), 10 pps, 84.9% (p = 0.043); 30 pps, 99.5% (p = 0.832); 40 pps, 97.8% (p = 0.168); 50 pps,

Effects of electrical and electromagnetic stimulation after anterior cruciate ligament reconstruction.

A need exists to develop new methods of neuromuscular electrical stimulation (NMES) that are both effective and relatively pain-free. The purpose of this pilot study was to determine the effects of both NMES and a new method of electromagnetic (NMES/PEMF) stimulation for reducing girth loss and for reducing pain and muscle weakness of the knee extensor muscles in patients during the first 6 weeks after reconstructive surgery of the anterior cruciate ligament (ACL). Seventeen patients receiving ACL reconstructive surgery participated as a control group (N = 3), as an NMES group (N = 7), and wit

The use of transcutaneous electrical nerve stimulation during the biliary lithotripsy procedure.

Transcutaneous electrical nerve stimulation (TENS) has been suggested as a means to reduce the amount of intravenous analgesia/sedation in patients undergoing extracorporeal shock wave lithotripsy (ESWL). A retrospective analysis of 79 ESWL procedures on 73 consecutive unselected patients was done in an attempt to determine the effectiveness of TENS in reducing the amount of intravenous fentanyl citrate and/or midazolam HCl needed to control pain and anxiety during the ESWL procedure. The study was divided into two parts: (1) all patients receiving TENS (n = 44) versus a non-TENS group (n = 35

Advances in electrical nerve stimulation techniques to manage chronic pain: an overview.

Pharmacologic treatments to manage chronic intractable pain have long been sought. Neuropathic pain is usually resistant to analgesics. At present, no analgesic drug totally relieves pain without producing significant unwanted side effects. Electrical stimulation can offer a degree of relief to some sufferers without risk. Transcutaneous electrical nerve stimulation (TENS) has a long clinical history. Stimulation parameters have been studied to improve the efficacy of this method. TENS is inadequate for extensive and bilateral pain, and epidural spinal cord stimulation is indicated. Dorsal col

Cardiac electrical responses to vagal stimulation of fibers to discrete cardiac regions.

We stimulated regional intracardiac parasympathetic nerve fibers to the atrioventricular (AV) nodal area (AVP stimuli) or to the sinoatrial (SA) nodal area (SAP stimuli) in autonomically decentralized, anesthetized dogs. AVP stimuli increased the AV interval (AV conduction time); the magnitude of the response depended directly on the stimulation frequency. AVP stimuli did not alter the atrial interval (heart period) in spontaneously beating hearts. The magnitude of the negative dromotropic response increased when the atrial interval was shortened. SAP stimuli increased the atrial interval, and

[Examination of the rectosphincteric reflex by means of endoscopic electrical stimulation. A new device].

An endoscopic electrical stimulation method (the new method) for the examination of the rectosphincteric reflex was devised to overcome difficulties attributable to the manometric method using balloon stimulation. Safety of the electrical stimulation under the conditions used routinely, was confirmed symptomatically and histologically. It was found that the optimal parameters of electrical stimulation to obtain the recto-sphincteric reflex were: 1 ms, 20 Hz, 5 to 7mA, and for 3 sec. The degree of the reflex relaxation became less and the time lag of the response to the stimulation became longe

Transcutaneous electrical nerve stimulation at acupuncture points in the induction of uterine contractions.

The effectiveness of transcutaneous electrical stimulation at acupuncture points for increasing uterine contractions in 20 post-dates pregnant women was assessed in a controlled study. Subjects were randomly assigned to either a treatment condition, consisting of the application via surface electrodes of a 30-Hz current to the points "spleen 6" (lower leg) and "liver 3" (foot), or a placebo condition, in which the equipment was attached but not activated. The frequency and strength of uterine contractions were monitored for 1 hour prior to stimulation and then for the final 2 hours of a 4-hour

Gap detection with sinusoids and noise in normal, impaired, and electrically stimulated ears.

Thresholds for the detection of temporal gaps were measured using two types of signals to mark the gaps: bandpass-filtered noises and sinusoids. The first experiment used seven subjects with relatively flat unilateral moderate cochlear hearing loss. The normal ear of each subject was tested both at the same sound-pressure level (SPL) as the impaired ear, and at the same sensation level (SL). Background noise was used to mask spectral "splatter" associated with the gap. For the noise markers, gap thresholds tended to be larger for the impaired ears than for the normal ears when the comparison w

Inhibition of antral and pyloric electrical activity by vagal afferent stimulation in the rabbit.

The effects of stimulation of cervical vagal afferents on the electromyographic activity (e.m.g.) of the gastric antrum and pylorus have been studied both in conscious and decorticate, curarized rabbits. In the latter group, stimulation at 10-15 Hz (0.3-0.4 mA; 5-7 ms duration) inhibited both the antral and pyloric e.m.g. in 21 rabbits, elicited an excitatory response in two more but produced no change in the remaining 13 animals. The inhibitory response was not affected by guanethidine (1 mg/kg, i.v.), phentolamine (1 mg/kg, i.v.) or propranolol (0.1 mg/kg, i.v.) and persisted in rabbits in w

Functional electrical stimulation for the treatment of bilateral recurrent laryngeal nerve paralysis.

We have previously presented the concept of electrophysiologic pacing of bilaterally paralyzed vocal cord abductors as a solution to the difficult problem incurred in this clinical situation. Initially, we demonstrated that it was indeed feasible to electrophysiologically pace abduction of the vocal cords synchronously with respiration, employing the EMG activity of the diaphragm as a trigger stimulus. Further research has led us to evaluate other possible physiologic trigger stimuli to ascertain which of these will prove most suitable in long-term pacing studies. In this article, we will repo

Electrical stimulation to reduce chronic toe-flexor hypertonicity. A case report.

Electrical stimulation reduces hypertonicity, but the mechanism for the effectiveness is not well understood. In this particular case, electrical stimulation to the toe extensors resulted in inhibition of the toe-flexor hypertonicity. This phenomenon may be explained by Sherrington's theory of reciprocal inhibition. Liberson found that electrical stimulation of an agonist is associated with a concomitant inhibition of the antagonist in the healthy individual. I applied Liberson's findings in my treatment of the patient. By facilitating the toe extensors, and thus inhibiting the toe flexors, to

Transcutaneous electrical nerve stimulation (TENS) after open heart surgery.

Open heart surgery is associated with postoperative sternal pain, which is exacerbated by cough, deep breathing and movement, thus limiting the physical activity of the patient. Transcutaneous electrical nerve stimulation (TENS) was administered to 40 patients suffering from persistent chest pain immediately following open heart surgery, and to ten other patients complaining of pain between three and eight weeks after operation. The efficacy of TENS was assessed by the subjective recordings, analgesic drug requirement, capability to carry out deep inspirations with an "Incentive Deep Breathing
