Leprosy and lymphatic filariasis comorbidity: the case for an integrated functional limitation grading system.

Worldwide, both lymphatic filariasis (LF) and leprosy are major causes of morbidity and functional limitation. Despite an abundance of data on the impairment caused by these infections in isolation, there is little data on disability in patients unfortunate enough to be affected by both infections. We present two cases of patients with LF and leprosy comorbidity. Both cases suffer from Grade 2 disability and chronic lymphedema in the same limb (left and right leg, respectively). These morbidities cause significant functional limitation in the patients' activities of daily living. Both LF and l

Application of geographical information system for lymphatic filariasis and malaria control in Nigeria.

Geographical Information System (GIS) is defined as an information system used to capture, store, edit, retrieve, analyze and visualize geographically referenced data. The use of GIS is one technology that is very useful in the prevention and control of Vector Borne Diseases (VBDs) such as lymphatic filariasis (LF) and malaria which cause high morbidity and mortality in Nigeria. This paper focuses on how the use of Geographical Information System (GIS) can be harnessed for surveillance, prevention and control of LF and malaria in Nigeria. GIS can be used as an operational tool to assist with r

Population ecology of mosquitoes and the status of bancroftian filariasis in El Dakahlia Governorate, the Nile delta, Egypt.

Mosquitoes were surveyed (Oct. 2010 & Apr. - Oct. 2011) in some localities representing 13 centers of El-Dakahlia Governorate. Six mosquito species were collected: Culex pipiens, Cx. antennatus, Cx. perexiguus, Ochlerotatus detritus, Anopheles pharoensis and An. tenebrosus. Culex pipiens was predominating (ca 79% larvae, 51% adults). Culex antennatus and Cx. perexiguus were also common. Of the Four types of the breeding habitats, the drainage canals were the most productive (53.4% larvae). For the three common species, the compiled larval density increases as water temp. increased and decrease

Evaluation of antibacterial soap in the management of lymphoedema in Leogane, Haiti.

In areas endemic for lymphatic filariasis, progression of lymphoedema is associated with recurrent bacterial acute dermatolymphangioadenitis (ADLA). The role of antibacterial soap in preventing ADLA is unknown. In a randomized double-blinded clinical trial in Leogane, Haiti, lymphoedema patients washed affected legs with antibacterial (n = 97) or plain soap (n = 100). Reported ADLA incidence (by recall) before the study was 1.1 episodes per person-year, compared to 0.40 assessed during the 12-month study. ADLA incidence was significantly associated with age, illiteracy and lymphoedema stage, b

Successful control of lymphatic filariasis in the Republic of Korea.

A successful experience of lymphatic filariasis control in the Republic of Korea is briefly reviewed. Filariasis in the Republic of Korea was exclusively caused by infection with Brugia malayi. Over the past several decades from the 1950s to 2006, many investigators exerted their efforts to detection, treatment, and follow-up of filariasis patients in endemic areas, and to control filariasis. Mass, combined with selective, treatments with diethylcarbamazine to microfilaria positive persons had been made them free from microfilaremia and contributed to significant decrease of the microfilarial

Interspecific hybridization yields strategy for South Pacific filariasis vector elimination.

BACKGROUND: Lymphatic filariasis (LF) is a leading cause of disability in South Pacific regions, where >96% of the 1.7 million population are at risk of LF infection. As part of current global campaign, mass drug administration (MDA) has effectively reduced lymphatic filiariasis prevalence, but mosquito vector biology can complicate the MDA strategy. In some regions, there is evidence that the goal of LF elimination cannot be attained via MDA alone. Obligate vector mosquitoes provide additional targets for breaking the LF transmission cycle, but existing methods are ineffective for controlling

Transmission dynamics of diurnally subperiodic lymphatic filariasis transmitted by Ochlerotatus (Finlaya) niveus in the Andaman & Nicobar Islands.

BACKGROUND & OBJECTIVE: In India diurnally subperiodic filariasis is prevalent only in the Nicobar district of Andaman and Nicobar Islands, with significant health problem. Sample surveys indicated that this form of filariasis is restricted to a small region of Nancowry group of islands with Ochlerotatus niveus as the vector. We therefore carried out a comprehensive study to assess the transmission dynamics of LF in Teressa island in Nicobar district in view of its control and evaluation of interventions. METHODS: Entomological studies were carried out for a period of twelve months covering a

Large-scale preventive chemotherapy for the control of helminth infection in Western Pacific countries: six years later.

In 2001, Urbani and Palmer published a review of the epidemiological situation of helminthiases in the countries of the Western Pacific Region of the World Health Organization indicating the control needs in the region. Six years after this inspiring article, large-scale preventive chemotherapy for the control of helminthiasis has scaled up dramatically in the region. This paper analyzes the most recent published and unpublished country information on large-scale preventive chemotherapy and summarizes the progress made since 2000. Almost 39 million treatments were provided in 2006 in the regio

[Discovery of a residual focus of bancroftian filariasis after declaration of its transmission interruption in Guangxi].

OBJECTIVE: To describe the discovery of a residual foci of bancroftian filariasis in Fuchuan County where the disease was announced to have been eliminated, and reveal its epidemiologic feature. METHODS: The investigation was carried out from August 2007 to March 2008 among residents in Changtang village where the first case of filariasis was found and the neighboring villages. They were screened with two thick blood smears. Immunochromatographic technology (ICT) was conducted for those going out but returned and those in surrounding areas. Vector mosquitoes were collected and dissected to fi

Integration of mass drug administration programmes in Nigeria: The challenge of schistosomiasis.

PROBLEM: Annual mass drug administration (MDA) with safe oral anthelminthic drugs (praziquantel, ivermectin and albendazole) is the strategy for control of onchocerciasis, lymphatic filariasis (LF) and schistosomiasis. District health officers seek to integrate treatment activities in areas of overlapping disease endemicity, but they are faced with having to merge different programmatic guidelines. APPROACH: We proceeded through the three stages of integrated MDA implementation: mapping the distribution of the three diseases at district level; tailoring district training and logistics based o

Entry lesions in bancroftian filarial lymphoedema patients--a clinical observation.

The prevalence of entry lesions in limbs was significantly higher in limbs with filarial lymphoedema (80.88%) than in normal limbs (42.86%, P = 0.000012). Among the various entry lesions in the lymphoedematous limbs, the prevalence of web space intertrigo was significantly higher in those who had acute dermatolymphangioadenitis (ADLA) than those who did not have ADLA (P = 0.04). Entry lesions were present only in 25% of those not using footwear, while 84.3% of those using footwear regularly or irregularly had these lesions (P = 0.01). None of the patients with good limb hygiene had ADLA, while

Chemotherapy for lymphatic filariasis: progress but not perfection.

Currently, only three drugs are used to control and treat the mosquito-borne parasitic disease lymphatic filariasis: diethylcarbamazine, ivermectin (Mectizan) and albendazole (Zentel). All interrupt transmission by eliminating microfilaria, the parasite stage that is responsible for transmission between hosts, but do not reliably kill the adult worms that are responsible for much of the pathology seen in the disease. There is an urgent need to develop drugs that will reliably kill adult worms and several compounds are under-going in vitro and animal testing. An alternative strategy - that of t

Susceptibility of mansonia indiana (Diptera: Culicidae) to nocturnally subperiodic Brugia malayi (Spirurida: Filariodea).

Mosquitoes, Mansonia indiana Edwards, 1930, were collected from non-endemic area of human lymphatic filariasis and tested for their susceptibility of infection using nocturnally subperiodic Brugia malayai Buckley & Edeson, 1956. Three cats naturally infected with B. malayi were used in the experiment for mosquitoes feeding. The data revealed that the susceptibility of mosquito infection ranged from 30 to 70%. The results also revealed that the susceptibility rates were not linearly correlated to the microfilarial densities in the cat at the time of feeding. The microfilarial density in cats ra

Evaluation of sequential intermittent pneumatic compression for filarial lymphoedema.

BACKGROUND: Lymphoedema is a major cause of morbidity in patients with lymphatic filariasis. There is no effective medical treatment and the results of surgery are uncertain. There are very few published studies assessing the volumetric response to the use of sequential intermittent pneumatic compression (SIPC) in patients. METHODS: A 12-celled instrument capable of providing sequential compression from the distal to proximal direction was used in 28 patients with unilateral grades II (n=17) and III (n=11) filarial lymphoedema in a planned 4-week session. The patients were followed up for 6 m

Entomological monitoring of annual mass drug administrations for the control or elimination of lymphatic filariasis.

Together, the interruption of transmission, through annual mass treatments with antifilarial drugs, and the alleviation of suffering in chronic cases form the principal strategy of the Global Programme to Eliminate Lymphatic Filariasis (GPELF). Entomological monitoring of the annual mass drug administrations is necessary to assess transmission levels and progress towards control or elimination of lymphatic filariasis. However, the traditional procedures for entomological monitoring, such as the dissection of mosquitoes caught manually, either as they come to bite at night or as they rest by da
