Surgical bypass to the left subclavian artery: an extra-anatomical transpleural approach.

A distal anastomosis in zone 3 is technically demanding during the frozen elephant trunk procedure. Proximalization of the distal anastomosis to zone 2 with subsequent revascularization of the left subclavian artery is an attractive alternative. This video tutorial describes the technique of an extra-anatomical bypass from the aortic prosthesis to the infraclavicular left subclavian (axillary) artery in arch replacement with the distal aortic graft anastomosis in zone 2.

Is open triple-branched stent graft the next stage?

Due to the pathology of acute type A aortic dissection (AAD), including fragile dissected aortic wall and visceral malperfusions, surgical procedures are still in development. This commentary is a review of a report by Bin and Yang et al. published in the Journal of Cardiac Surgery that reported similar effectiveness of the open triple-branched stent graft for acute type AAD and total arch replacement with the frozen elephant trunk technique.

Hosts, seasonal occurrence and life cycle of Rhipicentor nuttalli (Acari: Ixodidae).

There are only two species worldwide within the genus Rhipicentor, namely Rhipicentor bicornis and Rhipicentor nuttalli and both occur only in Africa. Rhipicentor nuttali has a widespread distribution in South Africa and the present investigation was initiated to elucidate its host preference, seasonality and life cycle. Rock elephant shrews, Elephantulus myurus were examined for ticks at four localities in the Free State Province, one in Gauteng Province and two in Limpopo Province, South Africa. Cape elephant shrews, Elephantulus edwardii were examined at two places in the Western Cape Provi

Evolutionary origins, diversification, and biogeography of liver flukes (Digenea, Fasciolidae).

Fasciolid flukes are among the largest and best known digenetic trematodes and have considerable historical and veterinary significance. Fas … asciolids examined comprise a monophyletic group with the most basal species recovered from African elephants. We hypothesize fasciolids migrated from Africa to Eurasia, with secondary colonization of Africa. Fasciolids have been conservative in maintaining relatively large adult body size, but anatomical features of their digestive and reproductive systems …

Prior reconstruction of an isolated left vertebral artery for Stanford type A aortic dissection.

Although isolated left vertebral artery is a supra-aortic trunk variant, it is not so rare. It may pose additional difficulties during total arch replacement surgeries. The aim of this study was to present our experience with prior reconstruction of isolated left vertebral artery by isolated left vertebral artery-left common carotid artery during total arch replacement combined with stented elephant trunk implantation.

Natural Frequencies in Sexual Pelvic Thrusting.

Seventy percent of mammals copulate using repeated pelvic thrusting, while the transfer of sperm requires just a single intromission. Why did thrusting evolve to be the dominant form of sexual intercourse? In this study, we investigate how the rate of sexual pelvic thrusting changes with body size. By analyzing films of copulating mammals, from mice Mus musculus to elephants Elephantidae, we find that bigger animals thrust slower. The rate of pelvic thrusting decreases from 6 Hz for the pocket mouse Pergonathus to 1.3-1.8 Hz for humans to an absence of thrusting for the rhino Rhinocerotidae an…

Case Report: Area of focus in a case of giant aortic arch pseudoaneurysm following fish bone penetration.

BACKGROUND: Esophageal foreign body impaction represents a common clinical emergency. In severe cases, it can lead to esophageal perforation and aortic injury, potentially causing life-threatening aorto-esophageal fistulas. While immediate intervention is critical, concurrent infection and retained foreign material substantially complicate management. CASE PRESENTATION: We report a rare case of a giant aortic arch pseudoaneurysm secondary to a migratory fish bone. Contrast-enhanced computed tomography (CT) confirmed the diagnosis, prompting urgent surgery. Median sternotomy under cardiopulmon

Soft Biological Actuators for Meter-Scale Homeostatic Biohybrid Robots.

Skeletal muscle's elegant protein-based architecture powers motion throughout the animal kingdom, with its constituent actomyosin complexes driving intra- and extra-cellular motion. Classical motors and recently developed soft actuators cannot match the packing density and contractility of individual muscle fibers that scale to power the motion of ants and elephants alike. Accordingly, the interdisciplinary fields of robotics and tissue engineering have combined efforts to build living muscle actuators that can power a new class of robots to be more energy-efficient, dexterous, and safe than e…

Rapid Dwarfing of an Insular Mammal - The Feral Cattle of Amsterdam Island.

The island rule describes a graded trend in insular populations of vertebrates from gigantism in small species to dwarfism in large species. The dwarfing of large mammals on islands has been observed both in the present fauna and in the fossil record. Elephants, hippopotami, deer, and other species became dwarfed on islands scattered all over the world, from the Mediterranean Sea to Indonesia, from the Eastern to Western Pacific Ocean, from the Caribbean to Canary Islands. The most rapid and well documented cases of island dwarfing known thus far took place over thousands of years. Here, we de…

Emergency one-stage hybrid surgery for ruptured aneurysm of the distal aortic arch.

Rupture of an aortic arch aneurysm is a life-threatening condition that requires emergency operation. For rupture of a distal arch aneurysm, we performed--with the patient under total circulatory arrest--an emergency total arch replacement with an elephant-trunk procedure, followed by one-stage antegrade stent-grafting. Prompt institution of cardiopulmonary bypass and total circulatory arrest has the advantage of lessening the risk of overt aortic rupture in this emergency situation.

Anal skin tags in inflammatory bowel disease: new observations and a clinical review.

BACKGROUND: The association between intestinal Crohn's disease (CD) and specific perianal abnormalities called anal skin tags (AST) has been recognized but not well defined. Skin tags have been classified into 2 types: 1) raised, broad, or narrow, single or multiple, soft or firm, and painless, often referred to as "elephant ears"; or 2) edematous, hard, often cyanotic, tender or not, and typically arising from a healed anal fissure, ulcer, or hemorrhoid. The aims of this study were to i) better characterize those skin tags identified by the term "elephant ears" and differentiate them from oth

Proteus syndrome: a rare cause of hemihypertrophy and macrodactyly on bone scanning.

Proteus syndrome is a rare, sporadic genetic disorder characterized by overgrowth of multiple different tissues in a mosaic pattern. It is associated with connective tissue nevi, epidermal nevi, disproportionate overgrowth of multiple tissues, vascular malformations, characteristic tumors, and specific facial anomalies. Joseph Merrick, popularly known as the Elephant Man, is now believed to have suffered from Proteus syndrome. A case of Proteus syndrome and associated findings on bone scintigraphy are presented.

Extended Cervical Access for Cerebral Malperfusion in Acute Aortic Dissection in a Circulatory Unstable Patient.

A 63-year-old man who had acute aortic dissection type A with signs of cerebral malperfusion and hemodynamic instability underwent immediate carotid artery cannulation. A Bentall procedure and total arch repair with frozen elephant trunk implantation were performed with extended cervical access to address supraaortic vessel involvement. We demonstrate that prudent cannulation and repair strategies can be successful even in the most complicated cases of aortic dissection.

STABILISE Technique via a Transapical Approach to Repair Residual Type A Aortic Dissection.

The stent-assisted balloon-induced intimal disruption and relamination in aortic dissection or STABILISE concept is a novel endovascular strategy in Type A and Type B dissections. We report a case of Type A aortic dissection repair combining, first, an open thoracic aortic surgery with an elephant trunk procedure and, second, an endovascular treatment using the STABILISE technique via a combined transapical approach commonly used for transcatheter aortic valve implantation and a femoral pathway.

Can the left subclavian artery revascularization during aortic arch surgery be managed using the in situ left internal mammary artery?

The management of the left subclavian artery (LSA) revascularization during aortic arch surgery is controversial and often challenging, especially during an emergency life-saving procedure. We report on a case of a 64-year old man, admitted to our institution with a Type A acute aortic dissection who underwent a frozen elephant trunk procedure with debranching of the supra-aortic vessels completed with an LSA revascularization using the in situ left internal mammary artery.
