Transmissibility of the 1918 pandemic influenza in Montreal and Winnipeg of Canada.

BACKGROUND: The threat of 2009 pandemic influenza A (H1N1) is still causing widespread public concern. A comprehensive understanding of the epidemiology of 1918 pandemic influenza commonly referred to as the Spanish flu may be helpful in offering insight into control strategies for the new pandemic. OBJECTIVE: We explore how the preparedness for a pandemic at the community and individual level impacts the spread of the virus by comparing the transmissibility of the 1918 Spanish flu in two Canadian cities: Montreal and Winnipeg, bearing in mind that each pandemic is unique and the current one

Exposure to lead and male fertility.

Lead is a reproductive toxicant. Exposure to inorganic lead is detrimental to human semen quality. The studies of the risk of spontaneous abortion and congenital malformation have shown contradictory findings. The aim of the following review is to summarise the epidemiological evidence of the effects of inorganic lead on male fertility. The focus is on epidemiological studies of time-to-pregnancy and related fertility measures. Blood lead measurements were applied to exposure assessment in all the studies. The results of the studies on fertility rates are consistent in showing an association b

Evaluation of the Remler M2000 blood pressure recorder. Comparison with intraarterial blood pressure recordings both at hospital and at home.

The Remler M2000 is a semiautomated device that has been used to collect epidemiological data and assess blood pressure variability. It has been subjected to limited evaluation in operation, however, and no studies of its accuracy away from the hospital or office environment have been undertaken. We recruited a group of 28 patients with essential hypertension who were undergoing intraarterial ambulatory blood pressure monitoring and compared the intraarterial recordings with those made with the Remler instrument both at home and in the hospital. The Remler recordings were also compared with si

Lyme disease in a warming world: Rising cases, shifting patterns, and clinical implications.

Lyme disease, the most common vector-borne illness in the United States, is increasingly affecting regions beyond historically endemic areas. Warming temperatures and evolving ecosystems have contributed to the expansion of tick populations and altered seasonal activity, raising the likelihood of human exposure in areas previously considered low-risk. Clinicians must maintain a high index of suspicion for Lyme disease, even in patients without traditional geographic exposure. This article reviews the impact of environmental changes on the distribution and epidemiology of Lyme disease. It also

Research Progress of Camptocormia in Parkinson Disease.

Camptocormia, also known as bent spine syndrome, primarily affects individuals with Parkinson disease (PD). This review provides an overview of camptocormia in PD, covering its definition, epidemiology, causes, diagnosis, and treatment. In the epidemiology section, we delve into its prevalence, gender disparities, and ongoing genetic research. Regarding diagnosis and assessment, we discuss evolving diagnostic criteria and measurement techniques, as well as new diagnostic tools. For management and treatment, a wide array of options is available, from conservative methods such as physical therap

Theoretical-methodological essay on gender identity information validity in epidemiological surveillance of violence.

The inclusion of the "gender identity" field in the Brazilian violence surveillance system, although representing a step forward, still has limitations that may compromise epidemiological data validity. Existing response options for victims' identities do not adequately cover the diversity of this analysis category, resulting in classification biases. Additionally, the absence of options for cisgender identities reflects an approach that naturalizes these identities, while trans identities are considered deviant and subject to surveillance. To overcome these limitations, it is imperative to ad

Superspreading in the emergence of COVID-19 variants.

Superspreading and variants of concern (VOC) of the human pathogen severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) are the main catalyzers of the coronavirus disease 2019 (COVID-19) pandemic. However, measuring their individual impact is challenging. By examining the largest database of SARS-CoV-2 genomes The Global Initiative on Sharing Avian Influenza Data [GISAID; n >1.2 million high-quality (HQ) sequences], we present evidence suggesting that superspreading has had a key role in the epidemiological predominance of VOC. There are clear signatures in the database compatible with

Systemic Effects of Segmental Vibration in an Animal Model of Hand-Arm Vibration Syndrome.

OBJECTIVE: Epidemiology suggests that occupational exposure to hand-transmitted (segmental) vibration has local and systemic effects. This study used an animal model of segmental vibration to characterize the systemic effects of vibration. METHODS: Male Sprague Dawley rats were exposed to tail vibration for 10 days. Genes indicative of inflammation, oxidative stress, and cell cycle, along were measured in the heart, kidney, prostate, and liver. RESULTS: Vibration increased oxidative stress and pro-inflammatory gene expression, and decreased anti-oxidant enzymes in heart tissue. In the prosta

Clinical and epidemiological differences in functional dyspepsia between the East and the West.

BACKGROUND: Functional dyspepsia (FD) is a common condition, affecting adults in both Western (North America and Europe) and Eastern (Asian) parts of the globe. The prevalence has been reported to range from 5% to 40%, largely due to variation from definition criteria and geographical location. Recent published reports in Western and Eastern populations separately indicate that differences in the epidemiology and clinical patterns of FD may exist. Such differences will have implications for the clinical management of and healthcare strategizing for FD at the local level. PURPOSE: This review

Predictive modeling of West Nile virus transmission risk in the Mediterranean Basin: how far from landing?

The impact on human and horse health of West Nile fever (WNF) recently and dramatically increased in Europe and neighboring countries. Involving several mosquito and wild bird species, WNF epidemiology is complex. Despite the implementation of surveillance systems in several countries of concern, and due to a lack of knowledge, outbreak occurrence remains unpredictable. Statistical models may help identifying transmission risk factors. When spatialized, they provide tools to identify areas that are suitable for West Nile virus transmission. Mathematical models may be used to improve our unders

The clinical and epidemiological features of childhood malaria in a moderately endemic area of Sri Lanka.

This study describes some clinical and epidemiological features of childhood malaria in a moderately endemic area of southern Sri Lanka. Six hundred and sixty-two children, who experienced 1,138 attacks of malaria, and 172 children, who experienced 202 attacks of acute non-malarial fever, were followed over a period of two years. Of the 1,138 malaria infections followed, 776 were due to P. vivax, 359 were due to P. falciparum, and 3 were mixed infections. The majority of children presented within the first three days of the onset of symptoms. Headache (96%), feeling cold (81%) and arthralgia (

The next stage: molecular epidemiology.

The traditional approach in epidemiology of relating exposure to an environmental agent such as a drug or infective agent has been to measure an overall risk (i.e., average and then "adjust risk for demographic variables and other confounders"). An attempt is sometimes made to define a "susceptible" subgroup. The analyses are usually based on good statistical methodology rather than an understanding of the interaction of body of host and agent. A twofold risk for 1000 exposed versus nonexposed people could be an average twofold risk for all 1000 exposed or a 20-fold risk for 100 exposed indivi

Prevalence estimates of pathological gambling in Seville (Spain).

Pathological gambling is becoming an increasing concern for the Spanish population. Governmental figures from 1989 show that in the Autonomic Region of Andalucia, with a population of 7 million inhabitants, more than three hundred billion pesetas (approximately UK pounds 1,500 million) were spent during 1988 gambling in casinos, slot machines and at bingo. These figures represent an increase of 15% over the figures of the previous year and suggest that gambling may have come a very important problem in Spain. This paper presents the first epidemiological study carried out in Spain on gambling

Population dynamics in echinococcosis and cysticercosis: evaluation of the biological parameters of Taenia hydatigena and T. ovis and comparison with those of Echinococcus granulosus.

An evaluation has been made of the biological and epidemiological parameters that determine the basic reproductive rates of Taenia hydatigena and T. ovis. These host-parasite systems are characterized by (i) no overcrowding in either host; (ii) no parasite-induced mortality of either host; (iii) no density-dependent constraint in the definitive host, but a strong, rapidly mobilized, short-acting immunity in the intermediate host and (iv) egg production which, in the natural environment, is high enough to prevent superinfection. It is considered that tapeworms with these characteristics are mor

The comparative effectiveness, under epidemic conditions, of the intradermal versus subcutaneous antityphoid vaccination estimated by epidemiological surveillance and laboratory tests.

The inhabitants of a territory in which a severe epidemic of typhoid fever appeared, were immunized intradermally or subcutaneously with heat-inactivated phenol preserved typhoid vaccine. The great majority of individuals received booster doses during the following three years. The specific immunization together with other epidemiologic and hygienic measures resulted in a quick extinction of the epidemic and the reduction in time of the number of typhoid carriers; no case of typhoid fever occurred in the following ten years among the vaccinated subjects. These satisfactory epidemiological resu

The current status of human monkeypox: memorandum from a WHO meeting.

In spite of a recent increase in the number of reported cases, human monkeypox remains a rare sporadic zoonotic disease with limited capacity to spread between humans. As such, the disease does not at present require specific public health measures. However, much of the population in the enzootic region, especially in the 5-14-year age group, still retains some immunity as a result of vaccination against smallpox. Continuation of surveillance activities on the same scale as at present until 1989 should provide a clear indication of the extent to which human monkeypox may be considered a public

Shigellosis in day-care centres.

Increasing numbers of outbreaks of shigellosis in day-care centres have been reported to the Center for Disease Control since 1972. Investigations reveal certain unique epidemiological features of shigellosis in this setting. Attack-rates tend to be higher than in outbreaks in primary schools, and epidemiologically these outbreaks resemble those in custodial institutions. Person-to-person transmission is the usual mode of spread; secondary spread within households is common, and there may also be significant spread to the community at large. Preventive measures should be directed at children,

What Factors Are Associated With Outcomes Scores After Surgical Treatment Of Ankle Fractures With a Posterior Malleolar Fragment?

BACKGROUND: Psychosocial factors, such as depression and catastrophic thinking, might account for more disability after various orthopaedic trauma pathologies than range of motion and other impairments. However, little is known about the influence of psychosocial aspects of illness on long-term symptoms and limitations of patients with rotational-type ankle fractures, including a posterior malleolar fragment. Knowledge of the psychosocial factors associated with long-term outcome after operative treatment of trimalleolar ankle fractures might improve recovery. QUESTIONS/PURPOSES: (1) Which fa
