Theory versus data: how to calculate R0?

To predict the potential severity of outbreaks of infectious diseases such as SARS, HIV, TB and smallpox, a summary parameter, the basic reproduction number R(0), is generally calculated from a population-level model. R(0) specifies the average number of secondary infections caused by one infected individual during his/her entire infectious period at the start of an outbreak. R(0) is used to assess the severity of the outbreak, as well as the strength of the medical and/or behavioral interventions necessary for control. Conventionally, it is assumed that if R(0)>1 the outbreak generates an epi

The frailty hypothesis revisited: mainly weak children die of measles.

It has been suggested that measles infection mainly kills frail children who are likely to die anyhow of other infections. If that were true, the proportion of frail children should increase after the introduction of measles vaccination and post-measles mortality compared with mortality in uninfected children should increase when the case fatality declines and frail children are no longer dying of measles. The latter deduction was investigated in Niakhar, Senegal, where the measles case fatality has declined markedly. Measles has been studied in Niakhar during 12 years from 1983 to 1994. We co

Daily deaths are associated with combustion particles rather than SO(2) in Philadelphia.

OBJECTIVES: To assess whether the association between SO(2) and daily deaths in Philadelphia during the years 1974-88 is due to its correlation with airborne particles, and vice versa. METHODS: There is a significant variation in the relation between total suspended particulate (TSP) and SO(2) in Philadelphia by year and season. Firstly, 30 separate regressions were fitted for each pollutant in the warm and cold season of each year. These regressions controlled for weather, long term temporal patterns, and day of the week. Then a meta-regression was performed to find whether the effect of SO(

Behavior of the exposure odds ratio in a case-control study when the hazard function is not constant over time.

In case-control studies of adverse drug effects there is rarely much evidence to support the assumption that the hazard function among users is constant during therapy. Nonetheless, this assumption is often implicitly made. We will use both clinical reasoning and a simple model to show how non-constancy of the hazard function affects odds ratio interpretation. When the hazard function is non-constant and there is more than one temporal pattern of drug usage, the odds ratio will estimate a weighted mean of incidence ratios with weights dependent on the corresponding fractions of person-time. If

Techniques for detecting and determining risks from low-level radiation.

Epidemiology is the study of disease in man. In evaluating radiation hazards, analytic studies have utilized the cohort type of investigation (where persons exposed and not exposed to radiation are followed forward in time for determination of disease experience) or case-control approaches (where persons with and without a specific disease are evaluated for previous exposure to radiation). Most radiation studies have evaluated cohorts (e.g., radiologists), although important case-control studies have been conducted (e.g., childhood leukemia as related to prenatal x ray). At its best, epidemiol

Forecasting regional COVID-19 hospitalisation in England using ordinal machine learning method.

BACKGROUND: The COVID-19 pandemic caused substantial pressure on healthcare, with many systems needing to prepare for and mitigate the consequences of surges in demand caused by multiple overlapping waves of infections. Therefore, public health agencies and health system managers also benefitted from short-term forecasts for respiratory infections that allowed them to manage services. While quantitative forecasts treating hospital admissions as continuous variables existed, many health managers prefer discrete levels of demand, similar to approaches used in weather and flooding. However, effec

Estimating the impact of influenza on the epidemiological dynamics of SARS-CoV-2.

As in past pandemics, co-circulating pathogens may play a role in the epidemiology of coronavirus disease 2019 (COVID-19), caused by the novel severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). In particular, experimental evidence indicates that influenza infection can up-regulate the expression of ACE2-the receptor of SARS-CoV-2 in human cells-and facilitate SARS-CoV-2 infection. Here we hypothesized that influenza impacted the epidemiology of SARS-CoV-2 during the early 2020 epidemic of COVID-19 in Europe. To test this hypothesis, we developed a population-based model of SARS-CoV-

Plant-based food groups and cardiovascular disease: inverse associations with variable mechanistic support.

This review integrates recent evidence on the associations between major plant-based food groups and cardiovascular disease (CVD), the biological plausibility of the proposed mechanisms, certainty-of-evidence assessments, and recurrent methodological limitations. PubMed and Web of Science were searched for English-language publications from 2020 to July 2026, with earlier studies included when evidence was sparse. Association evidence was derived primarily from umbrella reviews and systematic reviews of observational studies, supplemented by prospective cohort studies where review-level eviden

Natural bioactive compounds as therapeutic agents against phthalate neurotoxicity: Cellular and molecular signaling pathways.

Phthalates are ubiquitous environmental contaminants and endocrine-disrupting chemicals used as plasticizers in consumer products, medical devices, and industrial materials. Evidence from in vitro experiments, animal models, and epidemiological studies suggests that phthalate exposure, particularly to di(2-ethylhexyl) phthalate (DEHP), dibutyl phthalate (DBP), and benzyl butyl phthalate (BBP), may induce neurotoxicity through multiple interconnected mechanisms. The developing brain is especially vulnerable, with prenatal and early-life exposures linked to cognitive deficits, behavioral abnorma

Impact of policy shifts and festival holidays on tuberculosis surveillance: a time-series analysis in Southwest China (2014-2024).

BACKGROUND: Accurate forecasting of pulmonary tuberculosis (PTB) is frequently impeded by surveillance artifacts, most notably health policy shifts and festival holidays. This study proposes a practical epidemiological forecasting framework to address these real-world disruptions within China's monitoring data. METHODS: We utilized an 11-year historical cohort (2014-2024) from Jiulongpo District, Chongqing. To ensure longitudinal comparability, we first performed an epidemiological standardization to mitigate the artificial structural break caused by the 2018 diagnostic reclassification. To e

Neurotoxicity of antimony: A review of epidemiological evidence and the underlying molecular mechanisms.

Antimony (Sb) is a toxic metalloid and a global pollutant. Sb exposure is known to cause pulmonary, cardiovascular, liver and kidney damage, as well as cancer. In addition, data showing neurotoxic effects of Sb have been also obtained recently. Therefore, the objective of the present review was to discuss existing epidemiological findings linking Sb exposure to brain diseases and the underlying molecular mechanisms of Sb neurotoxicity. Laboratory findings revealed neurotoxic effects of high-dose Sb exposure. Specifically, in vitro and in vivo studies show that Sb induces neuronal apoptosis thr

Transmission potential of human schistosomes can be driven by resource competition among snail intermediate hosts.

Predicting and disrupting transmission of human parasites from wildlife hosts or vectors remains challenging because ecological interactions can influence their epidemiological traits. Human schistosomes, parasitic flatworms that cycle between freshwater snails and humans, typify this challenge. Human exposure risk, given water contact, is driven by the production of free-living cercariae by snail populations. Conventional epidemiological models and management focus on the density of infected snails under the assumption that all snails are equally infectious. However, individual-level experime…

Optima: A Model for HIV Epidemic Analysis, Program Prioritization, and Resource Optimization.

Optima is a software package for modeling HIV epidemics and interventions that we developed to address practical policy and program problems encountered by funders, governments, health planners, and program implementers. Optima's key feature is its ability to perform resource optimization to meet strategic HIV objectives, including HIV-related financial commitment projections and health economic assessments. Specifically, Optima allows users to choose a set of objectives (such as minimizing new infections, minimizing HIV-related deaths, and/or minimizing long-term financial commitments) and th

Prenatal exposure to environmental toxicants and their association with alterations in kidney development.

Chronic kidney disease (CKD) is a global public health problem whose susceptibility may originate during prenatal development. According to the developmental origins theory of health and disease, alterations during gestation can permanently affect kidney structure and function. The kidney is especially vulnerable during this period, as nephrogenesis occurs -exclusively before birth and determines the total nephron endowment. This review summarizes the epidemiological and experimental evidence linking prenatal exposure to environmental pollutants with alterations in kidney development. Heavy me
