OBJECTIVE: Erythema multiforme (EM) is an immune-mediated condition characterized by the appearance of target-like lesions on the skin and often accompanied by erosions or bullae involving the oral, genital, and/or ocular mucosae. 70% of recurrent EM cases are associated with HSV reactivation and it is labelled as herpes-associated erythema multiforme (HAE M). Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) may be fatal. Although classified by body surface area skin detachment, initial stages of both may present with erythema multiforme (EM)-like lesions. BACKGROUND: Dysmenorrhea, or painful menstrual periods, is one of the most common gynecological complaints. Most reports of uterine contractions range between moderate to severe pain, and patients often elect to cope with their discomfort without assistance from a physician. In the process, women experiencing dysmenorrhea are more likely to report absenteeism from work and school.

We treated two children with a total of five recurrences of erythema multiforme major with pulsed dose, systemic corticosteroid therapy on an open basis, and report herein the beneficial effects of this regime. We mean to further reevaluate this therapy on a controlled study basis.

Three young and middle-aged patients developed severe bullous eruption after receiving various penicillins. The clinical findings included high fever, prostration, widespread bullous eruption mainly on the head, face, and extremities, targetlike lesions on the palms and soles, and severe erosions in almost all the mucous membranes. Direct and indirect immunofluorescence studies were those of bullous pemphigoid. Complete clearing of the eruption occurred with prednisone therapy. We conclude that drug-induced bullous pemphigoid is a different entity from the classic bullous pemphigoid.

As reliable data on prevalence rates of drug-induced oral mucosa side effects are scarce in the medical literature, a retrospective analysis of drugs as possible inducers in 173 in-patients with either minor or major type of erythema multiforme (EM) admitted to the Erlangen University Department of Dermatology from 1970 through 1981 was carried out. In 62 of these cases precipitation by drugs was suspected by history, in 31 of them with oral mucosa involvement. In 11 out of 19 patients with mucocutaneous EM drug testing either in vivo or in vitro provided evidence for drug induction of EM.

Erythema multiforme developed in an 80-year-old man following the use of scopolamine hydrobromide ophthalmic drops. The erythema multiforme cleared when the medication was discontinued and recurred on challenge. Later, he was given tropicamide, an anticholinergic ophthalmic preparation that, like scopolamine, has a tropic acid residue. Within 15 minutes an immediate hypersensitivity reaction with generalized urticaria developed in the patient.
