Brain stem auditory-evoked potentials in meningomyelocele. Natural history of Chiari II malformations.

Brain stem auditory-evoked potentials (BAEPs) were recorded in 30 patients with meningomyelocele and shunted hydrocephalus ranging in age from birth to 33 years. Twenty-two of them had Chiari II malformation, one of which was symptomatic. In 22 cases, including 15 with Chiari II malformation, BAEPs were recorded repeatedly after periods ranging from 18 months to 7 years. The results were as follows: (1) III-V interpeak latency (IPL) decreased progressively from prolonged to the normal range with growth until 25 years of age; (2) I-III IPL tended to prolong gradually from the normal range; (3) …

Effects of alcohol on myoclonus and somatosensory evoked potentials in dyssynergia cerebellaris myoclonica.

Three brothers with dyssynergia cerebellaris myoclonica received alcohol to study the correlation between improvement of myoclonus and alteration in somatosensory evoked potentials (SEPs). Alcohol considerably improved myoclonus for about six hours in two patients (cases 1 and 2) but had only a mild effect in one (case 3). All three patients had giant cortical SEPs. The amplitudes of median N20-P25 and P25-N35 components and tibial N30-P40 and P40-N50 components were considerably decreased after alcohol ingestion in two patients (cases 1 and 2) but unchanged or slightly decreased in one (case …

Sensory and motor trigeminal evoked potentials to localize the position of trigeminal electrodes.

Analysis of verbal responses to electrical stimulation of the trigeminal ganglion and rootlets has been the only method available so far to … we describe an electrophysiological method complementary to clinical responses. Sensory trigeminal evoked potentials (STEPs) induced by separate successive orthodromic cutaneous stimulation of the three trigeminal divisions and recorded by the isolated tip of a transoval thermocouple electrode, together with motor trigeminal evoked potentials (MTEPs) elicited by s …

[A comparative study of multimodality evoked potentials and computed tomography in head injuries].

Multimodality evoked potentials (EPs) or three types of EPs--auditory brainstem response (ABR), somatosensory evoked potential (SEP), and visual evoked potential (VEP)--were recorded in 51 cases of traumatic intracranial hemorrhage within 3 days after injury. In order to assess these EPs, five EP grades were constructed, from normal, Grade I, to highly abnormal, Grade V. Furthermore, an EP pattern classification was devised to integrate the respective EP grades. Namely, PA, consisting of all three EPs within Grades I-III; PB, composed of one type of EP or both ABR and VEP at Grades IV and V; P…

The value of brainstem auditory evoked potentials in early diagnosis of Wernicke's encephalopathy.

Statistical analysis of the interpeak latencies of brainstem auditory evoked potentials (BAEP) shows that a delayed IPL I-V is a very sensitive indicator for an early diagnosis of Wernicke's encephalopathy. In cases of uncomplicated delirium tremens no significant deviations of BAEP were found.

Visual evoked potentials in optic nerve injury--does it merit to be mentioned?

The value of Visual Evoked Potentials (VEP) in the management of indirect optic nerve injury was prospectively studied in 78 patients. The initial VEPs were normal in 10, abnormal in 29 and absent in 39 patients. All 10 patients with normal VEP showed visual recovery. Amongst 29 patients with abnormal VEP, 26 (86.6%) showed improvement. In 39 patients initial VEPs showed no wave, however, subsequent VEP recordings demonstrated wave formation. Thus in 31 patients repeated VEP recordings failed to demonstrate wave formation, and none of them improved. This study, thus brings out the high predict…

Somatosensory evoked potentials as indicators of altered cerebral excitability during psychotropic drug treatment.

We observed an increase in the amplitude of the early cortical somatosensory evoked potentials (SSEPs) in five patients who developed myoclonus and/or generalized seizures during treatment with antidepressants. The increases correlated closely with the course of the clinical disturbances. In every case the SSEPs returned to normal values after the discontinuation of the psychotropic drugs. We suggest that SSEPs might help to identify and monitor patients who are at an increased risk of potentially hazardous side effects during psychopharmacological treatment.

[Visually evoked cortical potentials for early detection of optic neuritis in ethambutol therapy].

Ethambutol leads to chronic, severe optic neuritis with sudden onset in approximately 5-14% of all patients. Therefore, the ability of visually (pattern) evoked potentials for the early diagnosis of ethambutol-induced optic neuritis was tested. During ethambutol therapy prolonged latency was documented in 5 of 15 cases (33%). One of these 5 cases showed a reversible decrease in visual acuity. Thus, during treatment with ethambutol visually (pattern) evoked potentials may reveal a surprisingly high percentage of subclinical optic neuritis. These patients need close supervision in order to facil…

[Evoked potentials in neurologically asymptomatic persons during the early stages of HIV infection].

There are few reports on the relevance of evoked potentials in neurologically asymptomatic HIV seropositives. We studied 31 HIV infected males without AIDS or associated clinical neurological abnormalities. Visual evoked potentials by foveal checkerboard stimulation revealed a prolonged VEP latency in 37% of them. HIV seropositives with a pathologic VEP latency showed a significant reduction of their absolute numbers of peripheral blood T-helper cells. This increase of the mean VEP-latency was already significant in HIV seropositives with a T-helper cell count greater than 400/microliters. In …

[Somatosensory evoked potentials over the unaffected hemisphere in patients with unilateral intracerebral hematoma].

Somatosensory evoked potentials (SEPs) recorded from the unaffected hemispheres were studied in patients of less than one month after the onset of unilateral intracerebral hematoma. We examined 66 SEPs obtained from 49 patients with putaminal hemorrhage (midline shift 0-15 mm), 38 SEPs from 25 patients with thalamic hemorrhage (midline shift 0-8 mm), and 14 SEPs from 10 patients with subcortical hemorrhage (midline shift 0-12 mm). These examinations were made after electrical stimulation of the median nerve at the wrist by using square waves of 0.2 msec in duration delivered at a rate of 3 Hz.…

[Visual evoked potentials in patients with a history of non-traumatic intracranial hemorrhage].

The authors present the results of preliminary investigations of visual evoked potentials in patients long after intracerebral haemorrhage. In 32 studied patients prolongation was observed of the latency of waves N1 and N2 as well as P1, and this prolongation was more pronounced in the case of stimulation of the eye contralateral to the hemisphere with the lesion. This asymmetry of latency occurred with similar irregularity in case with all locations of the lacuna, with the exception of the occipital lobe. In the latter case a much greater prolongation of Ni and Pi latency on the affected side…

Brain-stem auditory evoked potentials in the alligator. Effects of temperature and hypoxia.

Brain-stem auditory evoked potentials (BAEPs) were recorded from young alligators (Alligator mississippiensis), and the effects of hypothermia, hyperthermia and hypoxia on the wave forms were determined. The wave form shape was similar to the human BAEP, although extra waves were routinely seen. The responses were highly repeatable and varied in a predictable manner as a function of stimulus frequency, polarity, intensity, and body temperature. Rarefaction clicks produced longer wave form latencies than condensation clicks. BAEPs were present over the entire temperature range studied (0-36 deg…

Brainstem auditory evoked potentials: intraoperative monitoring technique in surgery of posterior fossa tumors.

Brainstem auditory evoked potentials (BAEPs) were used to monitor eight neurosurgical procedures involving posterior fossa tumors to assist the neurosurgeon in preservation of hearing postoperatively. The technique included placement of recording electrodes over the Cz (vertex) and both earlobes. Stimulation was accomplished intraoperatively with a specifically designed intraauricular click stimulator that did not interfere with surgical access to the suboccipital region. Continuous BAEP recording was performed with particular attention to the sequence of preincision, opening of the dura, tumo…

Auditory brainstem evoked potentials in evaluating the central effects of middle ear effusion.

Auditory brainstem evoked potentials (ABEP) were recorded to determine whether children who suffered in the past, or suffered at time of examination from Secretory otitis media (S.O.M.), exhibited slowed conduction along their auditory brainstem. Five groups of subjects were examined: children with active S.O.M. during the examination, children with S.O.M. in the past who were treated non-surgically and recovered, children with S.O.M. in the past who were treated surgically and recovered, children who were treated by insertion of ventilating tubes (V.T.), and had S.O.M. during the examination …

Brainstem auditory evoked potentials in fullterm and preterm newborns with hyperbilirubinemia and hypoxemia.

Brainstem auditory evoked potentials (BAEP) were studied in 93 newborns (49 preterm and 44 fullterm) admitted to the neonatal intensive care unit. One group of 42 infants (28-42 wk CA) were considered at low risk for perinatal complications and served to establish the normal ranges of the BAEP parameters (wave I peak latency and wave I-V interwave interval). Two groups of high risk infants were studied during or shortly following clinically significant hyperbilirubinemia (31 infants) or hypoxemia (20 infants). Though statistically significant BAEP abnormalities were found in the high risk infa…

Effects of intraperitoneal carbon disulfide on sensory-evoked potentials of Fischer-344 rats.

Sensory-evoked potentials (EPs) were studied in male Fischer-344 rats to characterize the electrophysiologic concomitants of carbon disulfide (CS2) neurotoxicity. Behavioral measures also studied were forelimb and hindlimb grip strengths and performance of a conditioned avoidance response task. CS2 (172, 286, and 400 mg/kg) administered intraperitoneally five days per week for eleven weeks caused a decrease in grip strength, interfered with escape from shock, and disturbed visual and auditory EPs. Latency of the fifth but not the first component of the brainstem auditory-evoked response was pr…

The value of somatosensory-evoked potentials and bulbocavernosus reflex in patients with impotence.

In 14 normal male adults and 97 male patients having impotence alone or together with systematic and/or neuro-psychiatric symptoms and signs, the somatosensory cerebral-evoked potentials were obtained by glans penis stimulation (penile SEP); by peroneal nerve stimulation (peroneal SEP) and by electrically-induced bulbocavernous (BC) reflex. In normal subjects, the configurations of both SEPs were basically similar, except that the onset of latency was 10-15 msec longer and the amplitude in the P1-N1 component was higher in penile SEP. BC-reflex latency was abnormally prolonged, especially in d…
