Island thoracodorsal artery perforator-based V-Y advancement flap after radical excision of axillary hidradenitis.

Chronic axillary hidradenitis invariably ultimately requires surgical intervention to ameliorate this often painful, unrelenting, recurrent disease that can be disabling. The treatment of choice requires radical excision of at least all hair-bearing skin and its associated apocrine sweat glands. The ensuing large defect often is amenable to closure using a local flap instead of a skin graft to better expedite healing, to allow more rapid rehabilitation, and to minimize the risk of later scar contracture. The island thoracodorsal artery perforator V-Y advancement flap is another alternative tha

Screening strategies to identify new chemical diversity for drug development to treat kinetoplastid infections.

The Drugs for Neglected Diseases initiative (DNDi) has defined and implemented an early discovery strategy over the last few years, in fitting with its virtual R&D business model. This strategy relies on a medium- to high-throughput phenotypic assay platform to expedite the screening of compound libraries accessed through its collaborations with partners from the pharmaceutical industry. We review the pragmatic approaches used to select compound libraries for screening against kinetoplastids, taking into account screening capacity. The advantages, limitations and current achievements in identi

Spontaneous rectus sheath hematoma diagnosed by point-of-care ultrasonography.

Spontaneous rectus sheath hematoma is an uncommon condition that can mimic other conditions associated with an acute abdomen. We report the case of a patient with a spontaneous rectus sheath hematoma due to a ruptured inferior epigastric artery pseudoaneurysm who presented with hypotension and severe abdominal pain and was diagnosed using emergency department point-of-care ultrasonography. Point-of-care ultrasonography has been increasingly used in the evaluation of emergency department patients with acute abdomen and hypotension to expedite the diagnosis and management of aortic aneurysm and

Enhancing nurse practitioner understanding of urachal anomalies.

PURPOSE: To provide nurse practitioners an understanding of urachal anomalies, their presentations, and treatment options. DATA SOURCES: Scientific literature, including articles that address the pathogenesis, presentation, complications, diagnosis, and treatment of acquired and congenital urachal anomalies. CONCLUSION: Urachal disorders occur in 1 of 5000 persons. Complications from these disorders are even rarer. Early recognition of these anomalies can reduce the incidence of more serious complications such as secondary infections, sepsis, abscess formation, recurrent cystitis, and neopla

The role of CNS glia in preserving axon function.

Axons are the physical conduits by which information is relayed within the nervous system and as such, are essential for normal neurological function. In the central nervous system (CNS), axons comprise the bulk of the white matter, where they are closely associated with glial cells. Primary alterations of glial cell functions can have detrimental secondary consequences for axons, demonstrating that white matter glia are important custodians of axonal integrity. For example, genetic ablation of key oligodendroglial molecules abrogates the oligodendrocytes' supportive function, while expression

Hypoplastic left heart syndrome with intact atrial septum: Utilization of a hybrid catheterization facility for cesarean section delivery and prompt neonatal intervention.

Hypoplastic left heart syndrome (HLHS) with intact atrial septum (IAS) is a severe congenital cardiac malformation with little possibility of postnatal survival, unless an intra-atrial connection is created promptly after delivery. The timing of that intervention is often dictated by logistic restrictions, such as the proximity of the delivery room to the cardiac catheterization laboratory. This report details the use of a hybrid cardiac catheterization suite to expedite the delivery, transfer, and initial treatment of a fetus with HLHS/IAS. The mother underwent cesarean delivery in one hybrid

Splenic artery embolization in the management of an acute immune thrombocytopenic purpura-related intracranial hemorrhage.

Although relatively rare, intracranial hemorrhage remains the most common cause of immune thrombocytopenic purpura-related mortality [Medeiros D. Current controversies in the management of idiopathic thrombocytopenic purpura during childhood. Pediatr Clin North Am . 1996;43:757-72]. The required decompressive treatment has the potential for substantial blood loss and must often be delayed because of resistant thrombocytopenia responsive only to splenectomy. Splenic embolization is a novel approach to this problem that can expedite definitive neurosurgical care and minimize permanent sequelae.

Blocking adhesion molecules as therapy for multiple sclerosis: natalizumab.

Immunologists have long hypothesized that particular 'molecular addresses' govern lymphocyte entry to a given organ. In 1992, alpha4beta1 integrin was identified as the key molecule involved in homing to inflamed regions of the brain. An antibody to alpha4beta1integrin blocked paralysis in an animal model of multiple sclerosis, and the humanized monoclonal antibody natalizumab, which binds alpha4beta1 integrin, reduced relapses 66% in clinical trials in multiple sclerosis. Three months after its expedited approval by the FDA, natalizumab was removed from the market after two cases of deadly pr

Vacuum-assisted closure in the treatment of complex gynecologic wound failures.

BACKGROUND: Complex wound failures are a source of significant morbidity and mortality. They are costly and time consuming to treat, and may evolve into chronic, debilitating conditions. Vacuum-assisted closure is a novel wound healing technique applying subatmospheric pressure to wounds to expedite healing. CASES: We report the successful use of vacuum-assisted closure therapy on three patients on a gynecologic oncology service with complex wound failures of various chronicity. In all cases, vacuum-assisted closure therapy was well tolerated and demonstrated efficacy within 48 hours of initi

Tube decompression of the dilated colon.

With the technique described herein, decompression of a massively dilated colon is effectively accomplished, allowing resection to proceed safely. This method of decompression has allowed us to perform colonic resection in all patients with toxic megacolon seen in recent years. It is desirable to remove the infected and inflamed colon in such a circumstance. We have not resorted to blowhole colostomies in cases of toxic megacolon, as this leaves the colon, which is the septic source, within the abdominal cavity. This technique has also been used successfully to expedite subtotal colectomy and

Arthroscopic surgery for the treatment of osteochondrosis in the equine shoulder joint.

Osteochondritis dissecans (OCD) and subchondral cyst-like lesions in 13 shoulders of 11 horses were treated arthroscopically by curettage and lavage. Lameness decreased in all 11 horses. Nine horses were sound, five of them athletically sound, after 5 to 20 months. Complications included the development of subchondral cyst-like lesions and signs of degenerative joint disease. Arthroscopic surgery of the equine shoulder can be done through two portals, one for the arthroscope and one for an instrument. A few hand instruments such as a probe, Ferris-Smith rongeurs, and small, large, and right-an

Lymphoma of the head and neck. A diagnostic dilemma.

The surgeon should remember that lymphoma may involve any tissue in the head and neck region. By maintaining a high level of suspicion when evaluating a tumor that appears to be more aggressive than expected (that is, multiple primary sites), the head and neck surgeon will expedite treatment of the patient with lymphoma. Aids in early diagnoses center around providing sufficient tissue to the pathologist by avoiding needle biopsy and piecemeal removal of the regional lymph nodes or obtaining undistorted representative tissue from extranodal sites. We stress the need for a continuing dialogue b

Primitive defenses: cognitive aspects and therapeutic handling.

In this paper the primitive defenses first described by Melanie Klein under the label of "schizoid mechanisms" are examined. The defenses considered are splitting the pathological uses of identification and projective identification, and the psychotic forms of denial. This examination is twofold: (1) the cognitive aspects of these defenses as described in terms of concepts developed by Jean Piaget; (2) concrete examples of the operation of these defenses during the treatment of schizophrenic patients are given and the effects of interventions based on the cognitive analysis are described. It i

Beyond the myth: the mermaid syndrome from Homerus to Andersen. A tribute to Hans Christian Andersen's bicentennial of birth.

Mermaid or sirens have been part of the cultural tradition of the sailors during the first expeditions in the western world. The Siren's Myth appeared for a first time with Homer, who described in the Odyssey some singing creatures that lured the enchanted sailors to death. More frequently described with a bird body and a female head, sometimes the female part was extended to torso, with arms prolonged in sturdy claws. In the Latin literature Publius Ovidius Naso presented in the Métamorphoses these creatures. Proposed ethymology for the word "siren" seems to confirm the prerogatives of these …

Combining Artificial Intelligence and Human Expertise in Pursuit of Novel PolQ Inhibitors.

Application of generative AI, synergistically with traditional knowledge-based approaches, has expedited the discovery of small-molecule PolQ polymerase domain inhibitors. Leveraging AI-generated ligand designs, a structurally diverse set of compounds were synthesized and evaluated, ultimately identifying a novel oxime-based scaffold as a promising starting point. Although the initial hit demonstrated only modest biochemical potency, the oxime moietyan unexplored vector in this chemical spaceoffered an opportunity for strategic substitution. Gratifyingly, using this strategy, a highly potent

Preoperative Diagnosis of Amyand Hernia in the Emergency Department with Point-of-care Ultrasound: A Case Report.

INTRODUCTION: Amyand hernia is a rare condition in which the appendix is found within an inguinal hernia sac, often mimicking incarcerated or strangulated hernias. Diagnosis is typically made intraoperatively, but increasing use of point-of-care ultrasound (POCUS) is enhancing preoperative recognition. CASE REPORT: A 46-year-old male presented with a chronic, partially reducible inguinal hernia. Point-of-care ultrasound revealed a bowel-containing hernia with fluid. Computed tomography confirmed an inflamed appendix within the sac. Laparoscopic appendectomy and open hernia repair were perform

Revolutionizing viral disease treatment: Phase separation and lysosome/exosome targeting as new areas and new paradigms for antiviral drug research.

With the advancement of globalization, our world is becoming increasingly interconnected. However, this interconnection means that once an infectious disease emerges, it can rapidly spread worldwide. Specifically, viral diseases pose a growing threat to human health. The COVID-19 pandemic has underscored the pressing need for expedited drug development to combat emerging viral diseases. Traditional drug discovery methods primarily rely on random screening and structure-based optimization, and new approaches are required to address more complex scenarios in drug discovery. Emerging antiviral st

PSTPIP1-associated myeloid-related proteinemia inflammatory (PAMI) syndrome: A case report and review of the literature.

PSTPIP1-associated myeloid-related proteinemia inflammatory (PAMI) syndrome is a rare autoinflammatory disorder often arising in pediatric patients. We present a case of an 18-year-old female with a past medical history of growth failure, immunoglobulin A nephropathy, and inflammatory arthritis who presented to a pediatric dermatology clinic with findings of acne, psoriasiform dermatitis, and hidradenitis suppurativa, whose clinical, genetic, and laboratory findings were most consistent with PAMI syndrome. We conducted a literature review to better characterize this rare condition in the conte
