Effects of local application of methylprednisolone delivered by the C/GP-hydrogel on the recovery of facial nerves.

CONCLUSIONS: Local administration of MP delivered by the C/GP-MP-hydrogel can improve the recovery of facial nerve following crush injury. The findings suggested that locally injected MP delivered by C/GP-hydrogel might be a promising treatment for facial nerve damage. OBJECTIVES: In this study, the aim is to assess the effectiveness of locally administrating methylprednisolone(MP) loaded by chitosan-β-glycerophosphate hydrogel (C/GP-hydrogel) on the regeneration of facial nerve crush injury. METHODS: After the crush of left facial nerves, Wistar rats were randomly divided into four differen

New methodology for facial nerve monitoring in extracranial surgeries of vascular malformations.

OBJECTIVE: To develop a more reliable methodology for monitoring the facial nerve in surgeries of vascular malformations where the extracranial segment of the nerve is at risk. METHODS: Our methodology comprises: (1) preoperative mapping to identify the anatomical location of the nerve branches, (2) continuous intraoperative monitoring of the compound muscle action potential (CMAP) by stimulating the facial nerve extracranially, in close proximity to where the trunk of the facial nerve exits the skull at the stylomastoid foramen, (3) intraoperative mapping to identify the nerve branches durin

Surgical Treatment of Complex Craniofacial Fractures.

OBJECTIVE: Severe craniofacial fractures may present different needs in treating intracranial lesions over facial injuries. This paper examines the results of our strategy, consisting of a single-stage combined neurosurgical-maxillofacial treatment. METHODS: A retrospective review was conducted of 33 consecutive patients with complex fractures of the anterior cranial fossa and facial skeleton, who required elective surgery for craniofacial reconstruction. Patients who required emergency surgery for intracranial clots or penetrating wounds were excluded. In all cases, all or almost all the ant…

Preventing Facial Pressure Ulcers in Acute Respiratory Distress Syndrome (ARDS).

BACKGROUND: In patients with acute lung injury and/or severe acute respiratory distress syndrome (ARDS), prone positioning is a therapeutic intervention to improve oxygenation. Positioning a patient in a prone position increases the risk of medical device-related pressure ulcers in the facial area. CASE STUDIES: This article summarizes experience with 4 patients with ARDS. Two did not receive pressure ulcer preventive measures and subsequently developed multiple necrotic facial pressure ulcers related to prone positioning for treatment of ARDS. The other 2 patients were managed on a thin sili

Blindness and severe visual impairment in facial fractures: an 11 year review.

We present an 11 year review of facial fractures treated in the Australian Craniofacial Unit and the Department of Plastic and Reconstructive Surgery at the Royal Adelaide Hospital, specifically looking at those fractures in adults that resulted in blindness or severe visual impairment. During the period 1989-2000, a total of 2516 patients with facial fractures were treated operatively. From our facial-fracture database, 317 patients were identified as having an ophthalmological complication on presentation. Of these, 19 had severe visual impairment or blindness. The exact fracture pattern and

[Spiral CT in maxillo-facial trauma].

Fractures of facial skeleton are common in a busy emergency department in often requires careful radiological study before surgical treatment. Radiographic evaluation of facial injuries is especially challenging because the complex facial structure results in a confusing overlapping of densities on the radiograph. Instead Spiral-CT enables obtaining images on different planes so that even subtle or complex fractures may be evaluated in the best way. Moreover, associated lesions of important organs can be diagnosed. Our purpose is to verify the value of Spiral-CT in traumatology of facial skele…

Outcome of Facial Nerve Integrity After Parotid Gland Surgery With and Without Intraoperative Monitoring: A Ten-Year Retrospective Study.

Background: Facial nerve injury is one of the most concerning complications of parotid gland surgery, with temporary and permanent dysfunction rates varying widely in the literature. This study aimed to identify factors associated with facial nerve injury during surgery for benign parotid tumors and assess the protective efficacy of intraoperative nerve monitoring (NIM) in preventing nerve injury. Methods: This retrospective study analyzed 329 patients who underwent parotid gland surgery between 2010 and 2023. Data collected included patient demographics, tumor characteristics (size, location,

The Epidemiology and Mechanisms of Pediatric Facial Fractures.

Pediatric facial fractures, while less common due to the unique skeletal characteristics of children, represent a significant source of morbidity with potential long-term developmental consequences. This comprehensive national study utilizes data from the Trauma Quality Improvement Project (TQIP) to analyze trends in pediatric facial fractures across the United States between 2017 and 2022, noting trends observed before and after the COVID-19 pandemic. The authors examined a robust cohort of 121,407 pediatric patients aged 0 to 18 who sustained facial fractures. The most prevalent fracture typ

Facial Paresis After Mandibular Reconstruction.

Condylar neck fractures are common injuries that occur in the facial and mandibular regions. The proximity of this area to the vital neurovasculature creates the increased importance of proper surgical intervention to limit damage to the underlying structures. Here, we report a case of a condylar neck fracture that resulted in temporary paresis of the facial nerve. In addition, a review of the literature regarding condylar fracture treatment and its complications was conducted.

Facial Nerve Palsy Associated With Orthognathic Surgery.

The authors report a 25-year-old female who presented facial palsy after undergoing bimaxillary orthognathic surgery for retrognathism correction. Orthognathic surgery is a procedure used to treat dentofacial deformities which aims to achieve an adequate relationship between dental archs, improving function (such as chewing, breathing, and speaking) and facial aesthetics. Even though there are some complications that can occur during the intraoperative and postoperative periods like bleeding, tooth, soft-tissue damage, nerve damage, bad split, infection, and nonunion, facial nerve injuries are

Hair restoration.

The introduction of micrografts (1-2 hair follicular unit grafts) and minigrafts (3-4 hair follicular unit grafts) has made a most significant advancement in the care of male pattern baldness and female androgenic alopecia. Finally, natural and aesthetically pleasing results are possible. Additionally, many other applications in the reconstruction of facial hair and scalp have been found, some of these include: restoration of hair loss due to (iatrogenic) post-surgical causes, ie, after facial rejuvenation procedures or procedures involving incisions on hair bearing facial skin or scalp; scalp

[Emergency surgical treatment of injuries to the lateral region of the face].

If not treated correctly, injuries to the lateral part of the face can result in permanent mimetic disturbances. Since the final results of such treatment are often determined by the initial measures taken, close adherence to several basic principles for emergency surgical treatment of injuries to the parotid gland and facial nerve is imperative. To ensure optimal conditions for a successful follow-up nerve plasty, a special method of marking the nerve ends with silver clips and silicone tubes is recommended.

Facial Nerve Danger Zones.

Facial nerve injury is one of the most feared complications in surgical rejuvenation of the aging face. Understanding the three-dimensional architecture of the facial soft tissue, the fascial planes that exist within this architectural arrangement, and the danger zones where the facial nerve is situated superficial and adjacent to the planes of dissection commonly used in face-lift techniques, are the keys to safety in preventing motor branch injury. The two-dimensional branching patterns of the marginal and cervical branches of the facial nerve are variable, making it difficult to ascertain e

Use of virtual 3-dimensional surgery in post-traumatic craniomaxillofacial reconstruction.

Traumatic craniofacial injuries often present as difficult reconstructive challenges for maxillofacial surgeons. Reconstruction is often complicated by significant soft tissue loss, comminuted bony fragments, a tenuous blood supply, and wound contamination. For panfacial injuries, restoration of normal facial width, facial height, and sagittal projection may be difficult to achieve. Marked swelling may limit the surgeons' ability to palpate and recognize subtle bony defects and malunion. Furthermore, a true 3-dimensional assessment of bony alignment may not be possible with traditional surgica…

Facial form and function: films versus physical examination.

Criteria for the use of radiography for facial trauma in the emergency department have not been the subject of any critical reviews. Many recent reviews have been done on the advantages of various radiologic techniques to delineate specific facial injuries, both bony and soft tissue. No prospective study has addressed the criteria for ordering radiographs in the ED. Ideally the criteria would need only to consider the utility of studies in making therapeutic decisions. Unfortunately other considerations are involved, such as cost, medicolegal liability, and the real and imagined risks of radia…
