Necrotizing fasciitis affecting the chest wall is a rare condition and carries high mortality. It spreads rapidly, requiring early diagnosis and immediate extensive surgical debridement. The case of a 32 year old man afflicted with this uncommon condition following tube thoracostomy for empyema thoracis is described and literature reviewed.

Necrotizing fasciitis is a rare, subcutaneous infection. It can occur in patients after solid-organ transplantation. We herein report two patients who developed necrotizing fasciitis following combined liver and small intestine transplantation. The first patient experienced this infection 4 yr after transplantation and 1 yr after the closure of the ileostomy. The second patient suffered from necrotizing fasciitis 2 days after the transplant. Both cases were diagnosed on the physical findings, culture of subcutaneous lavage, and the computed tomography findings. The site of entrance of the orga…

Necrotizing fasciitis is a rapidly progressive infection of the fascia and subcutaneous tissues accompanied by a high mortality rate approaching 80% to 100%. Factors that predispose patients to this life-threatening complication include obesity, malnutrition, malignancy, chronic alcoholism, drug abuse, peripheral vascular disease, diabetes mellitus, and immunosuppressive therapy. The pathomechanisms for the development of this rare disease still remain unclear. We report a case of necrotizing fasciitis with Clostridium perfringens after laparoscopic cholecystectomy. The patient left the hospit…

A prospective single survey was conducted among Security Forces personnel at the Mobile Hospital, Ministry of Interior, Makkah Al-Mukarramah … enty-one patients had spur on the calcaneum. The prevalence of painful heel attributable to plantar fasciitis/calcaneum spur was 1.18%. This study suggests that obesity is a cause and initiator of heel pain and plantar fasciitis/calcaneal spur and that improper footwear aggravates the condition.

Diffuse fasciitis (DF) [diffuse fasciitis with eosinophilia-Shulman's syndrome] has occasionally been linked to a precedent infection with Borrelia burgdorferi. Here, we report on another case of DF in a 25 year old male, in whom Borrelia burgdorferi infection as possible inciting agent could be identified based on the patient's history and laboratory data. Efforts to microscopically demonstrate spirochetes or to amplify Borrelia-DNA by nested PCR in lesional tissue failed after antibiotic treatment had already been initiated. Although only a few cases of Borrelia associated diffuse fasciitis …

A case is presented of a 59-year-old male with rapidly progressive septic shock and necrotizing fasciitis. The patient was admitted in shock with an extensive skin lesion on the anterior chest wall. The history was relatively short and there was only a questionable history of preceding trauma. Necrotizing fasciitis was suspected from the appearance of the lesion. Antibiotics and anti-shock therapy were given but despite this, his condition deteriorated and he died from septic shock. At autopsy, the diagnosis of necrotizing fasciitis was confirmed. The importance of rapid diagnosis and primary …

BACKGROUND: Necrotizing fasciitis (NF) is an acute and life-threatening soft-tissue infection however rarely seen in oro-cervical region. Therefore, the details of oro-cervical NF (OCNF) are not well known. The purpose of this study was to investigate the characteristics of OCNF by comparing it with severe cellulitis of oro-cervical region (OCSC) or NF of other body regions (e.g., limb, perineum, and trunk) (BNF), respectively.
MATERIALS AND METHODS: At first, various risk factors for OCNF in oro-cervical severe infection (OCSI; composed of OCNF and OCSC), including neutrophil-to-lymphocyte r…

Background Necrotizing fasciitis (NF) is a rapidly progressing infection with a high mortality rate, requiring prompt diagnosis and treatment. The Laboratory Risk Indicator for NF (LRINEC) score has emerged as a useful tool for early risk stratification. Methods A cross-sectional study was conducted on 112 patients diagnosed with NF in a tertiary hospital in Chennai. Clinical data, including LRINEC scores and associated laboratory parameters, were collected and analyzed. The relationship between LRINEC scores and risk factors, such as gender, comorbidities (diabetes mellitus [DM] and periphera…

Necrotizing fasciitis is a rapidly advancing soft-tissue infection with high morbidity and mortality, and markedly worse prognosis when complicated by delayed diagnosis and intervention. Distinguishing necrotizing fasciitis from less severe soft-tissue infections remains a significant clinical challenge, prompting the development of the Laboratory Risk Indicator for Necrotizing Fasciitis (LRINEC) score to aid early recognition. Early enthusiasm for the score led to widespread clinical use; however, subsequent experience has raised concerns about its utility as a sensitive rule-out tool. We pre…

OBJECTIVE: To analyze own experience in treating children with necrotizing fasciitis (NF) and literature data regarding clinical features, course of disease, predictors of complications and likelihood of death.
MATERIAL AND METHODS: We analyzed treatment, follow-up and fatal outcomes in 10 patients aged 4-144 months with NF. Data analysis was performed using the StatTech program.
RESULTS: Mean duration of disease was 1.45±1.17 days. In 2 patients, manifestation of disease was in abdominal wall, in other cases - in limbs. We established the entry gates in 6 children. Most children had concomi…

Obesity complicates clinical evaluations of acute abdominal conditions like appendicitis, with excess fat limiting the accuracy of physical examinations and delaying diagnosis. This report presents a case of a 49-year-old morbidly obese female with uncontrolled diabetes who developed acute peritonitis secondary to retrocaecal appendicitis, which presented as retroperitoneal necrotising fasciitis (NF). Diagnostic delays were attributed to atypical clinical signs and limitations of ultrasound in obese patients. A computed tomography (CT) scan eventually revealed extensive retroperitoneal fluid c…

Eosinophilic fasciitis is a rare sclerodermiform disease characterized by upper and lower limb edema. We present the case of a 71-year-old woman currently hospitalized for painful lower limb edema. Laboratory tests reveal moderate eosinophilia (0.8 g/L) and an inflammatory syndrome. Abdominopelvic ultrasound reveals no abnormalities. In light of the unexplained inflammatory syndrome, a 18 F-FDG PET/CT scan currently shows intense hypermetabolism of the fasciae in the lower and upper limbs. Following an MRI, a biopsy is performed, and the anatomopathology currently confirms eosinophilic fasciit…
