Necrotizing fasciitis is a rare but potentially fatal disease which is often confused with cellulitis. By examining the pathogenesis, clinical presentation, treatment, and a representative case study, this article proposes to lend a broader perspective to this infection. Emphasis is placed on the necessity of surgical debridement in combination with antibiotic therapy to minimize the possible morbidity associated with this condition.

BACKGROUND: Necrotizing fasciitis of the head and neck is a rare occurrence. It spreads rapidly along fascial planes causing extensive necrosis; it often results in gangrene of the overlying skin. Mostly involved are immunodeficient patients with banal infections of the upper aerodigestive tract, small traumas, but also after surgical procedures. The "Federal Health Agency" of the Federal Republic of Germany estimates a number of 40 cases a year in Germany for all regions of the body. The mortality is about 20%-50% dependent on the localisation of this soft tissue infection.
PATIENTS: Four ca

This article is based on the retrospective study of 4 cases of necrotic fasciitis of the upper extremity, in adult patients with a mean age of 57 years (range: 36 to 78 years) and with a male predominance (3 M/1 F). Presenting signs were variable: pain, febrile and inflammatory oedema, ecchymoses with inflammatory masses containing clear or haemorrhagic fluid. Treatment with antibiotics and anti-inflammatory drugs did not prevent progression to painless, necrotic ulcers. Rapid medical and surgical treatment constitutes an element essential of the prognosis and must include wide large debrideme

OBJECTIVE: The purpose of this study was to report the aerobic and anaerobic microbiology of periumbilical necrotizing fasciitis in newborn infants.
STUDY DESIGN: Retrospective review was done of the author's 20-year experience.
RESULTS: Specimens obtained from 11 newborn infants with periumbilical necrotizing fasciitis were cultured for aerobic and anaerobic bacteria. A total of 38 bacterial isolates was recovered: 21 aerobic and facultative and 17 anaerobic. Aerobic or facultative bacteria only were present in 1 specimen (9%), anaerobes only in 2 (18%), and mixed aerobic and anaerobic flor

Group A beta-hemolytic Streptococcus pyogenes (GAS) causes a spectrum of highly aggressive, invasive infections. We report two cases of necrotizing fasciitis in which GAS was identified as the presumptive causative organism with the use of the standard rapid streptococcal diagnostic kit. We believe the rapid test kits may be a useful adjunct in the diagnosis and treatment of this catastrophic illness and may play a role in limiting the spread of infection.

Necrotizing fasciitis is a severe and sometimes life-threatening soft tissue infection that is characterized by rapidly widespreading necrosis of fascia and subcutaneous tissue. Fifty-four cases of necrotizing fasciitis were reviewed over a 5-year period. Among them, 25 patients (46%) had diabetes mellitus. The majority of the wound bacterial cultures (71%) yielded a mixed growing. There were negative cultures in six patients, and single organism growth in 14 patients (29%). The predominant organisms growth were Streptococci, Staphylococci, and Escherichia coli. The overall mortality rate was

Two neonates who presented with necrotizing fasciitis (NF) secondary to omphalitis were treated by radical excision of the anterior abdominal wall. Postoperatively, renal failure in both children was managed by continuous arteriovenous hemofiltration (CAVH). One child survived; the other died of sepsis. Serum lactate levels, which were 10 to 15 times the normal levels in both infants postoperatively, decreased rapidly in the survivor, but never approached normal levels in the infant who died. Although the best prospect for survival in neonatal NF remains prompt, radical, surgical excision and

A significant increase in the frequency of necrotizing fasciitis caused by streptococci of group A has recently been noted. The disease usually appears in individuals without obvious risk factors. The initial lesion is often quite ordinary, and the evolution towards a toxic shock very swift. This evolution can be summarized as follows: 1) localized infection; 2) bacteraemia with circulating toxins and soft tissue necrosis; 3) production of cytokines by the immune cells of the host, leading to a rapidly irreversible toxic shock Serotypes 1, 3, 12 and 28 of group A streptococcus are usually invo

Mediastinal and thoracic extension of head and neck infections are rare but occur even in the modern antibiotic era. Early intervention is paramount to successful clinical outcome. 111Indium-labeled white blood cell (WBC) scan, a new imaging modality, appears to play a role in complementing computed tomographic (CT) findings, demonstrating early extension of disease, and following therapeutic efficacy. Together, the CT scan and WBC scan aid in directing early intervention and extent of surgery in this disease of high-potential mortality. We report 3 cases of deep head and neck infection compli

Necrotizing fasciitis is a severe infection with a mixed bacterial population involving a single tissue plane. It is best diagnosed by the history of minor trauma, extreme toxicity, anesthesia of the skin and roentgenologic evidence of soft tissue gas. Initial treatment consists of antibiotics, intravenously administered fluids, blood transfusions, calcium and general patient support. Surgical procedures include extensive debridement as soon as possible and as needed for continued necrosis, secondary closure and skin grafting. The mortality of this disease has not been altered by antimicrobial

Necrotizing fasciitis, a serious infection requiring prompt antibiotic and surgical therapy, previously has not been associated with Haemophilus aphrophilus. We report a case of necrotizing fasciitis associated with methylphenidate abuse from which H aphrophilus was isolated. Surgical and antibiotic therapy based on in vitro susceptibility testing resulted in eradication of the infection.

The Laboratory Risk Indicator for Necrotizing Fasciitis (LRINEC) score is widely used to distinguish between necrotizing fasciitis and cellulitis. However, LRINEC scores are not as sensitive or specific as initially reported, possibly due to differences in patient backgrounds in different countries. Here, we examined the validity of LRINEC scores in Japanese patients. We also investigated the possibility of developing a new scoring system. Patients with necrotizing fasciitis (n = 56) and cellulitis (n = 209) were retrospectively evaluated. The data were split into training (n = 199) and valida

BACKGROUND Necrotizing fasciitis, a subtype of necrotizing soft-tissue infection, is a rare but life-threatening condition that requires a prompt and decisive approach. Early surgical debridement remains a key treatment; however, after debridement, the management of complex wounds, particularly in high-risk patients, remains a major clinical challenge and often requires prolonged hospitalization and multiple surgical interventions. These situations may warrant alternative techniques, such as negative pressure wound therapy, to support effective wound healing. This case demonstrates that a beds

OBJECTIVE: Necrotizing fasciitis (NF) is a severe and potentially life-threatening soft tissue infection characterized by rapid necrosis, primarily affecting the fascia and deep soft tissues. Despite its low incidence, NF poses significant challenges due to its aggressive nature and high mortality rates. This retrospective study aimed to assess clinical characteristics and prognostic factors influencing mortality rates among patients diagnosed with NF at a tertiary care facility.
METHODS: Patients diagnosed with NF at the emergency department of tertiary care hospital in Turkiye between Augus

PURPOSE: Necrotizing fasciitis of the upper extremity is a life-altering and often life-threatening condition. Surgical outcomes depend on factors that include delay in presentation and concurrent medical comorbidities. The impact of health care disparities among underrepresented populations has been increasingly highlighted across various surgical specialties. The present study sought to evaluate whether racial or ethnic variables influence outcomes after necrotizing fasciitis of the upper extremity on a population-wide basis.
METHODS: The TriNetX database was queried for patients diagnosed

Streptococcus constellatus is usually a benign, commensal bacteria but has increased incidence in blood cultures and abscesses. This pathogenic involvement is most prevalent in individuals with underlying medical conditions, such as solid tumors and type 2 diabetes mellitus, as well as in cases of community-acquired infections. We report a 43-year-old male with a right medial thigh ulcer and necrotic scrotal skin. The wound culture from surgical debridement grew Streptococcus constellatus, and histology was consistent with stage III necrotizing fasciitis. Regardless of etiology, the mortality

Necrotizing fasciitis is a severe, life-threatening soft tissue infection that presents as a surgical emergency. It is characterized by a rapid progression of inflammation leading to extensive tissue necrosis and destruction. Nonetheless, the diagnosis might be missed or delayed due to variable and nonspecific clinical presentation, contributing to high mortality rates. Therefore, early diagnosis and prompt, aggressive medical and surgical treatment are paramount. In this review, we highlight the defining characteristics, pathophysiology, diagnostic modalities, current principles of treatment,
