Clinical characteristics of acoustic trauma caused by gunshot noise in mass rifle drills without ear protection.

One of the major occupational hazards of working in military service is being subjected to intense impulse noise. We analyzed the clinical presentation of acoustic traumas, induced by mass rifle gunshot noise during military training, in unprotected patients. We evaluated 189 soldiers who had otologic symptoms after rifle shooting exercises without using any hearing protection. All soldiers had been training on the K2 rifle. We took medical histories; conducted physical examinations and hearing evaluations (pure-tone audiometry, speech audiometry, and impedence audiometry); and distributed the

U-shaped sacral fracture and lumbosacral dislocation as a result of a shotgun injury: case report.

OBJECTIVE: Gunshot wounds to the spine and/or sacrum rarely cause spinal instability. Our goal is to report the first case of a U-shaped sacral fracture and lumbosacral dislocation caused by a gunshot injury to the spine. CLINICAL PRESENTATION: A 37-year-old man sustained a close-range shotgun wound to the abdomen. The blast partially destroyed the L4 and L5 vertebral bodies and fractured the S1 and S2 segments of the spine, resulting in severe neurological deficits with lumbosacral and spinopelvic instability. INTERVENTION: Debridement of devitalized tissues, proper antibiotic coverage, dec

Multiple cranial gunshot injuries without skull penetration.

We report on a 74 year old patient who was admitted to the Emergency Department of Cologne University after an attempt to commit suicide. Clinical examination showed no neurological deficits but two bleeding bullet holes in his head bilateral in the temporal areas initially suggesting a complete passage of a bullet through the head. The patient was able to answer questions but showed also a slight bleeding from his mouth while talking. Emergency CT showed a bullet between the internal and external lamina of the temporal bone on both sides and one bullet in the hard palate. With general anesthe

Neuro-cognitive activity during a self-paced visuospatial task: comparative EEG profiles in marksmen and novice shooters.

Log-transformed EEG power spectral estimates (6-7, 9,10-11,18-22, and 36-44 Hz), obtained from skilled marksmen and novice shooters at sites F3, F4, C3, C4,T3, T4, P3, P4, O1, and O2 during the aiming period (6 s) of a target shooting task for each of 40 trials up to the moment of trigger pull, were contrasted to determine regional differences in cortical activation. The EEG power obtained from both groups during the preparatory aiming period was also compared to that observed for a similar time period during the processing of standard verbal and spatial tasks. The marksmen exhibited less acti

Secure Storage Training to Reduce Veteran Suicide: Mental Health Clinicians' Needs Assessment Results.

Many Veterans see a clinician shortly before suicide. Clinicians report a lack of training in firearm secure storage conversations as a barrier to initiating these conversations. We aimed to determine the needs of Veterans Health Administration mental health clinicians for training in discussing secure storage. A needs assessment survey of attitudes, barriers, facilitators, and desired training topics was sent to mental health clinicians at 15 Veterans Administration medical centers in the South, including Texas. Of clinicians completing the needs assessment (N = 566), most (93%) endorsed "usu

Limb salvage and functional recovery following accidental shotgun injury in a young child: A case report.

BACKGROUND: Paediatric firearm injuries are rare in civilian settings but pose significant challenges due to high-energy trauma, small anatomical compartments, and limited physiological reserve. Shotgun wounds, in particular, produce complex skeletal and soft tissue destruction requiring staged multidisciplinary care. CASE PRESENTATION: We report a 3-year-old girl with an accidental close range shotgun injury to the right forearm, presenting with grossly contaminated wounds, soft tissue loss, and comminuted fractures of the shaft of radius and ulna. Emergent debridement and elbow-spanning ext

Delayed presentation of retained renal shrapnel: a case report and literature review.

Retained renal foreign bodies (FBs) following blast or firearm injury are rare and usually present early; delayed presentation is a challenging diagnostic scenario. The present report describes a case of retained renal shrapnel presenting with obstructive complications 18 years after the initial injury. Imaging revealed a 24 mm calculus encrusting a retained metallic FB at the left renal hilum, with severe hydronephrosis and reduced differential renal function (27.4%). Following ureteroscopic stenting, percutaneous nephrolithotomy was performed. The surrounding calculus was fragmented, but the

Expert consensus on the evaluation and diagnosis of combat injuries of the Chinese People's Liberation Army.

The accurate assessment and diagnosis of combat injuries are the basis for triage and treatment of combat casualties. A consensus on the assessment and diagnosis of combat injuries was made and discussed at the second annual meeting of the Professional Committee on Disaster Medicine of the Chinese People's Liberation Army (PLA). In this consensus agreement, the massive hemorrhage, airway, respiration, circulation and hypothermia (MARCH) algorithm, which is a simple triage and rapid treatment and field triage score, was recommended to assess combat casualties during the first-aid stage, whereas

Child Fatalities in Dammam: A Call for Child Fatality Reviews in Arab Countries.

Identifying children's risk exposure is the first step toward mortality prevention. This retrospective study determined the causes of child fatalities in Dammam, Saudi Arabia. Death reports of children and adolescents from 1999 to 2015 (N = 157) were analyzed. Boys represented most cases (69%) and there were two age peaks (1-5 years and 16-18 years). Accidents (typically immersion) defined the main death circumstance (51%) followed by homicide (25%). Only 33% of cases underwent autopsy, and the most common cause of death was head injury (27%) followed by firearm injury. Only one immersion deat

Providing medication therapy management for smoking cessation patients.

Nearly 50 years ago, the Surgeon General of the US Public Health Service released the first report of the Surgeon General's Advisory Committee on Smoking and Health. The report concluded that cigarette smoking caused lung and laryngeal cancer as well as bronchitis. Today, smoking is one of the leading preventable causes of deaths in the United States. Research has shown that it potentially causes more deaths than human immunodeficiency virus, illegal drug use, alcohol use, motor vehicle injuries, and firearm-related incidents. Health care providers play a critical role in guiding and directing

Pseudo-gunshot wound injury from perforating rib fracture: a cautionary case report.

We report a case of a young woman who was found unresponsive in a garbage dumpster beneath the balcony of her 9th floor apartment residence. Initial investigations by the police and coroner raised concerns regarding the circumstances of the death. Initial examination of the body, revealed a single penetrating injury on the chest with a wide abrasion collar--the injury pattern was similar to a gunshot entry wound or a shored exit wound. Autopsy revealed the abraded skin perforation along with major chest injuries with bruising of the chest wall, extensive comminuted rib fractures, and pleural a

Traumatic common carotid-jugular fistula: report of 2 cases.

Traumatic common carotid artery-to-internal jugular vein fistula is a rare entity that is not usually detected during the acute injury phase. We present 2 cases of traumatic carotid-jugular fistula. Both involved the right side of the neck, and both were due to injury from a firearm. The patients exhibited signs of high cardiac output. In Patient 1, the diagnosis was made by clinical examination and radiography. After diagnosis, the patient's parents requested his discharge from the hospital because of an inability to pay for treatment. The patient did not return. Patient 2 had a brachial plex

Psychiatric and neurological characteristics of murder defendants referred for pretrial evaluation.

After literature review, this paper presents the largest study to date (n = 270) of psychiatric and neurological characteristics of accused murderers in the United States. This retrospective record review of pretrial detainees undergoing competency to stand trial and criminal responsibility evaluations examined demographic characteristics, psychiatric diagnosis, substance use patterns, Intelligence Quotient (IQ), and results of electroencephalogram (EEG), neuroimaging (MRI or CT) and neurological examination. Substance use and mood/adjustment disorders were common. Neuroimaging was abnormal in

Characteristics of acute acoustical trauma in the Finnish Defence Forces.

In spite of strict safety regulations concerning firearm shooting, several hundred conscripts still suffer acute acoustical trauma (AAT) in the Finnish Defence Forces (FDF) every year, resulting in symptoms such as tinnitus and/or hearing impairment. The causes leading to AAT, causative weapons and mode and level of hearing protection were analysed to find out why so many AATs still occur in the FDF. The material consisted of 119 patients of the total 163 AAT patients treated at the Central Military Hospital during the year 2000. In 87.5% of cases, the AATs occurred in unprotected ears. The mo

Criminal deterrence as a public health strategy.

Deterrence is an established theme in criminal justice, but its role in prevention of assault has been treated with ambivalence and even hostility in medicine. The extent to which offenders can be persuaded, through knowledge of criminal and health risks, not to injure others is emerging from studies of the health effects of firearm and other crime legislation, and from macro-level studies and controlled experiments of police interventions. There is convincing evidence that motorists can be deterred from alcohol-impaired driving, and recognition that specific, targeted, and visible police work

Burden of Racial Disparities in Pediatric Traumatic Brain Injury Among American Indian/Alaska Native Children: A Scoping Review of the Literature.

BACKGROUND: Traumatic brain injury (TBI) is a leading cause of morbidity and mortality in children worldwide. In the United States, and in the state of Alaska in particular, American Indian/Alaska Native (AI/AN) children experience a disproportionate burden of healthcare disparities. Understanding the epidemiology, mechanisms of injury, access to care, and long-term clinical outcomes in this vulnerable population is crucial for guiding management strategies and prevention initiatives. OBJECTIVES: This scoping review aimed to characterize the epidemiology of pediatric TBI among AI/AN children

Cytokine changes in nasal secretions after acute acoustic trauma.

BACKGROUND: Occupational noise-induced hearing loss poses a significant health risk, especially for military and law enforcement. Unlike steady, prolonged noise, impulse noise, such as gunfire, leads to more immediate and severe hearing impairment. This study explores the link between sensorineural hearing loss and cytokine levels in nasal secretions of male and female military cadets exposed to high-intensity gunfire noise. METHODS: 105 cadets (75 males and 30 females, aged 19-25) undergoing regular firearms training were included. Audiometric testing and measurement of IL-1b, TNF-a, GM-CSF,…

Suicide or Homicide? A Case of Self-Inflicted Triple Gunshot Wound by Double-Barrel Shotgun.

Differential diagnosis between suicide and homicide by firearms is still a major challenge in forensic practice, especially when multiple gunshot wounds are present. A case of self-inflicted triple wound by double-barrel shotgun is presented. A 73-year-old White male patient was found dead in a wooden area. According to investigations, he had disappeared from home the day before. The corpse was covered with clothing in a prone position on a gravel road, a short distance from a car with driver-side-door open and keys in the ignition. Next to the body, a 12-gauge double-barrel shotgun was found,…

Estimation of all-cause noise exposure for U.S. adults from national survey data.

Millions of adults are at risk of hearing loss resulting from exposure to occupational and recreational noises. Data from the combined National Health and Nutrition Examination Survey (NHANES) 2011-2012 and 2015-2016 datasets were used to establish the prevalence of occupational and recreational noise exposures through self-report questions. For recreational noise exposures, NHANES asked about the use of firearms, including the use of hearing protection devices (HPDs) while shooting, and off-work exposures to very loud noise. For work exposures, NHANES asked about exposures to loud and very lo…
