The authors present the results of a retrospective review of popliteal artery injuries associated with fractures and dislocations about the knee. They treated 41 patients with popliteal artery injuries associated with either fractures about the knee or knee dislocations. Thirty-five of the patients were males, 6 females; the mean age was 23 years. The delay before accessing the hospital was 17 hours (range: 3 hours to 10 days). Thirty-two fractures were open. There is evidence that rigid fixation of zygomaticomaxillary suture enhances the recovery of the infraorbital nerve compared with other means of surgical treatments. Six cases of old unreduced Monteggia fracture dislocation are reported. All cases were treated by surgical reconstruction. The author concludes that the essential feature of a stable result is complete correction of the ulna deformity. It is suggested that the natural history of unreduced Monteggia fracture dislocations is not benign and surgical treatment < or = 6 years after the initial injury appears justified. The surgical technique is described.

We report seven cases in which open or closed reduction of a shoulder dislocation associated with a fracture of the humeral neck led to displacement of the neck fracture. Avascular necrosis of the humeral head developed in all six patients with anatomical neck fractures. All five anterior dislocations also had a fracture of the greater tuberosity and both posterior dislocations had a fracture of the lesser tuberosity. The aim of this study was to evaluate the influence of primary displacement of fragments of the radial head on the clinical result after conservative therapy, also to see whether the extent of displacement could give valuable indications for operation. In non-displaced fractures the results are slightly but significantly better than in displaced fractures. On the other hand, the amount of displacement must be considered to be of minor importance. Injuries to all five carpometacarpal joints are rare. Only four cases have been reported in the English literature. A case of fracture-dislocations of all five carpometacarpal joints is presented. An excellent result has been obtained 2 years after open reduction and internal fixation. This case is the only one in the English literature so treated and with prolonged follow-up.

An exceptional case of posterior dislocation of the ulna at the elbow, associated with fracture of the radial shaft, is reported. Diagnosis was established with anteroposterior and lateral radiographs of the forearm, elbow and wrist. Treatment consisted of closed reduction of the ulnar dislocation and open reduction and internal fixation of the radius with an AO plate. A long arm cast was applied for 3 weeks with the elbow held at 90 degrees. Six months later full function of the limb had been achieved.

We reviewed the cases of fifteen children who were treated for a Monteggia injury. Transverse and metaphyseal fractures of the ulna were stable after closed reduction, but oblique fractures of the shaft redisplaced and required intramedullary pinning. Five patients who were seen late had open reduction and internal fixation of the fracture. Four of these children had reconstruction or repair of the annular ligament, with excellent results. But in the child in whom the ligament was not repaired the radius subluxated.

Fourteen cases of fracture of the shaft of ulna and radius with associated dislocation of the radial head in adults are presented. The dislocation was initially missed in 42 per cent of cases. Several closed and open therapeutic procedures were performed either for the dislocation or the shaft fractures. In a review of 22 patients with unstable fractures or fracture-dislocations of the thoracolumbar spine, treated by Harrington rod instrumentation and spinal arthrodesis, all arthrodeses became solid. All patients with incomplete neurologic lesions regained some function. No patient was made neurologically worse by the procedure. Solid arthrodesis was achieved whether laminectomy was performed or not. Harrington rods offer a method of reduction of compression fractures, offer partial stability to the spine, and serve as an adjunct to spine arthrodesis.

In fracture-dislocations of the cervical spine with associated injury to the supporting posterior elements, anterior interbody bone block or dowel fusion does not give sufficient stability to the involved area.
