Nonunion of long bone fractures in children: a review of 30 cases.

Nonunions after diaphyseal fractures of long bones in children are rare. Thirty diaphyseal nonunions in 30 children are reported. The sites of nonunion were tibia (15), femur (5), ulna (4), humerus (3), radius (2), and fibula (1). Nonunions in children tend to occur after high-energy trauma, particularly when the fracture is compound, there is soft tissue loss, and infection develops. Open reduction and internal fixation may contribute to nonunion, particularly when the fixation is inadequate or holds the fracture apart. Repeated manipulation of the fracture after open reduction may also contr

A review of long-bone fractures in patients with spinal cord injuries.

In a retrospective investigation of 38 long-bone fractures in 30 spinal cord injured patients (mean follow-up, 22 months), fractures were classified acute (occurring at the time of spinal cord injury) or pathologic (occurring in the chronically injured patient). The methods of treatment and complications were recorded and evaluated. The incidence of long-bone fractures was 6.7%. Complication rates were high, ranging from 20% to 40%, with open or closed treatment of extremity fractures. Strict adherence to established operative fracture management principles in paraplegic patients irrespective

Fractures of the malar bone. Follow-up study based upon a questionnaire.

During the years 1969-1976 214 patients with malar bone fractures were treated. A retrospective study was carried out and a follow-up by the aid of a questionnaire. The clinical judgement at 1 week after surgery showed good results. The follow-up study revealed, however, a poorer picture. Among the surgically treated patients who answered the questionnaire, 89 of 143 reported different complaints. Anaesthesia in the check and diplopia were the most common complaints. It seems anyhow that the process of regeneration of the sensible nerve takes longer than predicted. The pre-operative evaluation

[Treatment of open fracture bone defect of distal femur with antibiotic cement column and bone graft].

OBJECTIVE: To investigate the efficacy of antibiotic cement column combined with iliac bone graft in the treatment of open fracture with bone defect of distal femur. METHODS: From October 2014 to March 2021, 16 patients of open fracture bone defect of distal femur were treated with antibiotic bone cement column and iliac bone graft, including 12 males and 4 females. The age ranged from 28 to 68 years old. There were 11 cases of traffic accident injury, 5 cases of falling injury, 3 cases as Gustilo type Ⅰ, 5 cases as type Ⅱ and 8 cases as type ⅢA. AO classification was used:9 cases of C2 type

Decreased bone mass in adolescents with bone fragility fracture but not in young children: a case-control study.

BACKGROUND: The incidence of distal forearm fracture due to minimal/moderate trauma shows a bimodal distribution for age at event, with one peak occurring during early adolescence, in both boys and girls and the other one in postmenopausal females. The aim of this study was, therefore, to document whether the relationship between bone mineral density and fracture is different in young children compared with adolescents. METHODS: A matched-pair, case-control study has been conducted to evaluate bone mineral density in 469 young children and 387 adolescents of both sexes, with/without fracture

Reliability of temporal bone high-resolution CT in patients with facial paralysis in temporal bone fracture.

OBJECTIVE: This study aimed to investigate the reliability of temporal bone high-resolution CT (HRCT) in patients with traumatic facial paralysis. METHODS: HRCT with cross-sectional scanning and multi-planar reformation (MPR) was performed on 26 cases with traumatic facial paralysis, and the preoperative imaging manifestations were compared with surgical findings. RESULTS: Preoperative HRCT revealed fallopian canal damage at the posterior genu in 1 case, geniculate ganglion in 22 cases, labyrinthine segment in 4 cases, tympanic segment in 13 cases and mastoid segment in 0 case, while surgica

[Secondary osteoporosis or secondary contributors to bone loss in fracture. The relevance of immune system to bone metabolism].

Bone homeostasis is maintained by a balance between bone resorption by osteoclasts and bone formation by osteoblasts. It is also regulated by immune system and its imbalance often results in pathological processes such as secondary osteoporosis. For instance, Th1 and Th17 are involved in activation of immune system and also induce maturation of monocytes to osteoclasts, which leads to osteoporosis. Contrarily, Treg (regulatory T cells) suppresses immune function, but suppresses osteoclast differentiation. Proinflammatory cytokines such as TNF cause an imbalance in bone metabolism via direct an

Fat and bone in children: differential effects of obesity on bone size and mass according to fracture history.

Fat mass predicts bone accrual in prepubertal children, but obese children have increased fracture risk. We hypothesised that bone size and mass would vary according to prior fracture in obese children. One hundred and three children (52 obese) underwent dual-energy X-ray absorptiometry (DXA) scanning of the lumbar spine, total body, and radial metaphysis and diaphysis. We derived body size-adjusted bone mineral density (BMD) estimates for each site using commonly employed procedures. Following adjustment for either age, age(2) and weight, or height and weight based on a reference group of non

Bone mineral density and bone loss measured at the radius to predict the risk of nonspinal osteoporotic fracture.

Low bone mineral density (BMD) and, probably, the rate of bone loss (RBL) are associated with the risk of osteoporotic fractures. To estimate the risk of nonspinal fracture in osteoporotic women, we measured BMD and RBL in a prospective study (average follow-up, 5.38 years) in 656 postmenopausal women. The women were considered in three groups: group A (whole population), group B (women under the age of 65 years) and group C (women over the age of 65 years). At the beginning of the study, BMD was measured at the distal radius (DR) and at the proximal radius (PR) using a single-energy densitome

Ultradistal and cortical forearm bone density in the assessment of postmenopausal bone loss and nonaxial fracture risk.

Forearm bone mineral density (BMD) was measured by single-energy photon absorptiometry in 360 healthy females without known axial fractures, 202 of whom were postmenopausal. The three sites addressed included an ultradistal (U) region containing approximately 60% trabecular bone. The other sites, distal (D) and shaft (S), were progressively more cortical. Reproducibility was 1.7-1.9% CV. The earliest evidence of a significant correlation between BMD and years since menopause was seen in trabecular bone in subjects aged 45-55 years. Fractional decrease in BMD, relative to the premenopausal valu

Beyond bone density: toward an integrated bone- muscle-function framework for fragility fracture prevention.

Fragility fractures are a major clinical and public health challenge in aging societies. Current prevention strategies are commonly organized around skeletal risk assessment and osteoporosis management, yet fracture occurrence and post-fracture outcomes in older adults are also influenced by muscle dysfunction, frailty, fall propensity, physiological reserve, and rehabilitation capacity. This Perspective proposes a bone-muscle-function framework to complement existing osteoporosis-centered approaches to fragility fracture prevention. In this framework, bone represents skeletal strength and str
