Extensile triradiate approach in the management of combined acetabular fractures.

INTRODUCTION: We investigated the results of combined acetabular fractures that were treated through the extensile triradiate approach in this study. MATERIALS AND METHODS: Between January 1996 and January 2001, a total of 48 acetabular fractures were treated surgically (mainly combined fractures). Twenty-five of the combined acetabular fractures that were surgically treated through the triradiate approach with a minimum of 2 years' follow-up were included in the study. The mean patient age was 42 years. There were 8 both-column, 6 T-shaped, 2 anterior column/posterior hemitransverse, 4 trans

Do functional deficits result from radial head resection?

The treatment of comminuted fractures of the radial head with primary or secondary head resection is controversial. To assess the outcome of patients after radial head resection, a retrospective study with clinical and radiologic follow-up including isokinetic testing was performed. Between 1981 and 1992, 151 patients underwent radial head resection for comminuted fractures. Fifty-nine patients were operated on during the first 2 weeks after injury (primary treatment), 47 patients were operated on between 3 weeks and 6 months after injury (early secondary), and 45 patients were operated on mor

[Plantar Lisfranc dislocation fracture].

We report a 30-year-old patient suffering a plantar dislocation fracture after he dropped a heavy weight on his foot. The patient was treated immediately after diagnosis was secured by CT scan. Median approach and dermatofasciotomy of the foot were followed by anatomic reduction of the fractures and the Lisfranc dislocation and fixed by internal osteosynthesis. After 3 months the patient was able to ambulate pain free without walking aids. Plantar dislocation is a very rare direction of comminuted Lisfranc dislocation fractures. The outcome may be favorable with early reduction and stable inte

[External fixation of the lower limb in children].

18 children (13 boys, 5 girls) with displaced fractures of the femur (13 cases) and tibia (5) after road accidents were treated with Wagner external fixation. 5 fractures were open and 13 closed. 9 also had head injury and were unconscious. In 3 fractures the popliteal artery was torn, requiring arterial reconstruction. 15 were examined an average of 2.3 years after injury and there was full range of motion in 13. In 2 after severe injury, range of motion was 80% of normal. There was refracture after removal of external fixation in 1. We conclude that external fixation is stable and has minima

Surgical prosthetic splints as an adjunct in treating facial fractures.

Surgical splints are a valuable adjunct in managing certain mandible and maxillary fractures. Of the various splint materials, acrylic is the easiest, fastest, and least expensive. Acrylic splints are rigid, strong, easily adjusted and repaired, translucent, lightweight, and tolerated well by the oral mucosa. Splints are beneficial not only in the edentulous adult, but also in the child with deciduous dentition and in the patient with a partial dentition. Splints are helpful in managing fractures of the symphysis, parasymphyseal region, body, and alveolar ridges of the mandible, sagittal fract

Ensuring bone-to-bone contact reduces interfragmentary strain in forearm shaft plating: A finite element study.

BACKGROUND: Forearm diaphyseal fixation requires a mechanical environment appropriate for healing, but effects of residual gaps and cortical contact patterns remain unclear. This study used finite element analysis to quantify how gap size and contact pattern affect fracture-site strain and plate stress and to inform fixation strategy. METHODS: Computed tomography scans from uninjured contralateral forearms of five adults with forearm diaphyseal fractures were used to create subject-specific models of the central 4/6 radius fixed with a small-fragment Ti-6Al-7Nb low-contact locking compression

[Distal femoral periprosthetic fractures: classification and therapy].

PURPOSE OF THE STUDY: Periprosthetic fracture is one of the most serious complication of total knee arthroplasty. In our retrospective clinical study we designed our classification with rules for treatment of those fractures. MATERIAL AND METHODS: During the last thirty years we treated 53 distal femoral periprosthetic fractures in our orthopaedic department. In our clinical study we reviewed our group of distal femoral periprosthetic fractures with on the basis of X-ray findings, the treatment method used and treatment outcomes. RESULTS: According to our findings we divided distal femoral p

Medial Submuscular Plating of the Distal Femur.

Distal femur fractures are often treated with laterally based plate constructs. The use of medial plates is less common because of the concern for the proximity to vascular structures. With a solid understanding of the anatomy, the technique can be used more frequently to optimize fixation in medial distal femur fractures and osteoporotic, comminuted, bone loss, and periprosthetic fractures. This video demonstrates a safe technique for the medial submuscular plating of a distal femur fracture. The video covers indications for surgery and provides detailed anatomy of the structures at risk, esp

Fractures of the Lateral Process of the Talus.

Lateral talar process fractures (LTPF) are uncommon injuries but have become more relevant with snowboarding. Currently the fractures are classified according to McCrory-Bladin into 3 types, with advice for treatment that is not ideal anymore. This article proposes modifying the existing classification by differentiating the multifragmented type III into 3 subtypes: IIIa, articular multifragmented but metaphyseal simple; IIIb, articular and metaphyseal multifragmented but reconstructable; and type IIIc, comminuted and nonreconstructable. A treatment-algorithm is presented. Undisplaced fracture

Humeral nonunion.

Humerus fractures comprise 5% to 8% of all fractures. Although uncommon, nonunions can present a frustrating challenge to the orthopedic surgeon. Various risk factors that may predispose patients to nonunion include obesity, osteoporosis, alcoholism, smoking, poor bone quality, and scar tissue. Many methods of treatment have been described with various degrees of success. The rates of healing of humeral nonunions by traditional means of internal fixation with bone graft range from 70% to 92%, although in cases of infection, poorly vascularized beds, and open, segmental, or severely comminuted

[Results of treatment of patellar fractures].

In a retrospective study the long-term results after treatment of 140 fractures of the patella were rated with a knee score. Compared with other studies our results were slightly better, although we did not have a smaller rate of comminuted fractures. Most cases were treated with tension band wiring. K-wires, partial and total patellectomy--compared with other studies--were rarely used, even in severe fracture types. The AO-classification in our study did not satisfy at all. For that reason, we suggest a new classification which should be useful to predict the outcome of patella fractures and

Hoffa fracture: should precautions be taken during fixation and rehabilitation?

A coronal fracture of a femoral condyle (Hoffa fracture) is an unusual injury and there are only a handful of reports discussing it. We report a case of a 52-year-old worker who fell from a height, suffering lower limb injuries, including a Hoffa fracture with comminution, and had problems with malunion during the postoperative period. Clinicians should be aware that rehabilitation programmes need to be tailored to the method of fixation used to manage this uncommon fracture pattern.

Subcapital fracture of a congenitally dislocated hip: revival of Girdlestone resection arthroplasty.

A twenty-year-old, active male with a high congenital dislocation of the hip sustained a displaced subcapital fracture (accompanied by a comminuted transarticular fracture of the ipsilateral distal femur). Such a combination of injuries poses a challenging problem; the English literature does not cite any report describing the occurrence of subcapital fractures in congenitally dislocated hips. This article reviews the treatment options of the fractured neck of the femur, and the reasons for electing to perform a Girdlestone resection arthroplasty.

[Osteosynthesis of extra-articular proximal tibial fractures].

The treatment of extra-articular proximal tibial fractures is a therapeutic challenge due to the frequently significant soft tissue injury, the effect of the deforming forces and the need for an exact restoration of the bony alignment. Various methods of osteosynthesis are available for surgical stabilization. The locking plate osteosynthesis is the most frequently used procedure because of its good biomechanical stability, especially in osteoporotic bones, and the protection of the periosteal blood flow. Depending on the extent and stability of the defect zone, especially in the case of a med

[Femoral shaft fractures].

Femoral shaft fractures after completion of growth predominantly affect young people with healthy bones. The causes are mostly high-velocity traffic accidents, crushing or running over mechanisms and falls from a great height. Gunshot wounds are relatively rare in Germany but have a certain importance internationally and in military medicine. Accompanying injuries in local or other regions are frequent. The predominant fracture types are transverse, wedge, segment and comminuted fractures. Spiral fractures are a sign of indirect force and are therefore frequently found in older patients with o
