Frontal sinus obliteration using a laterally based pedicled pericranial flap.

OBJECTIVE: Fractures of the frontal sinus represent one of the more uncommon injuries of the maxillofacial skeleton. In an effort to avoid potential mucocele formation, frontal sinus obliteration has been put forward as the treatment of choice when there has been significant disruption of normal frontal sinus drainage. Traditionally, sinus obliteration has been accomplished with nonvascularized free adipose tissue or bone grafts and a variety of alloplastic materials. We developed a laterally based pedicled pericranial flap to accomplish frontal sinus obliteration. STUDY DESIGN/METHODS: A tot

A novel approach to frontal sinus surgery: treatment algorithm revisited.

BACKGROUND: Access to the frontal sinus remains a challenging problem for the craniofacial surgeon. A wide array of techniques including min … tly outline and then cut the anterior table of the frontal sinus at its junction to the surrounding frontal bone. The outcomes are evaluated by postoperative three-dimensional computed tomography scan. RESULTS: Eight patients sustaining frontal sinus fractures were treated with the aid of medical modeling. Three patients (37.5%) had isolated anterior table fr …

Out of the North Sea: the Zeeland ridges Neandertal.

In 2001, a portion of human frontal bone was discovered in sediments extracted from the bottom of the North Sea, 15km off the coast of the Netherlands. The extraction zone is located in the so-called Zeeland Ridges area located at 51 degrees 40' northern latitude and 3 degrees 20' eastern longitude. The specimen was dredged up from sediments containing Late Pleistocene faunal remains and Middle Palaeolithic artefacts, including well-finished small handaxes and Levallois flakes. The details of the supraorbital morphology, as well as the quantitative assessment of the shape of the external surfa…

A Giant Maxillary Mucocele Presenting Left Cheek Swelling.

A paranasal sinus mucocele is an epithelial-lined, mucus-containing sac that completely fills the sinus and forms an expandable cystic structure. It most commonly affects the frontal and ethmoidal sinuses, and rarely the maxillary and sphenoid sinuses. Orbital displacement or external disfigurement resulting from the expansion of the frontal or ethmoid sinuses is common; however, facial asymmetry caused by maxillary bone remodeling is rare. We describe a case of large maxillary sinus mucocele that destroyed the maxillary sinus bony wall, resulting in notable left cheek swelling and disfigureme

Spontaneous epidural haematoma in sickle cell anaemia: case report and literature review.

Non traumatic spontaneous epidural Haematoma is a rare and often unmentioned complication of sickle cell disease. It is often associated with skull bone infarction. We report an eighteen year old boy with sickle cell anaemia who developed persistence headache during a vaso-occlusive crisis. Brain computed tomography (CT) revealed a right frontal epidural Haematoma (EDH) compressing on the brain. No other etiologic factor was identified. A right frontal craniotomy and evacuation of the Haematoma was performed and he made good recovery. The possible pathogeneses of this rare condition are discus

[Pathological changes of frontal sinus in the materials of the Otolaryngological Clinic of Medical Academy in Lódź (1986-1996)].

Authors report 25 cases of frontal sinus diseases treated surgically at the ENT Department Medical University of Lodz. There were three groups of pathologies: 13 cases of inflammatory disease, 9 osteoma cases and 3 trauma cases. The total number of 30 surgical procedures has been performed (13 Beck's method, 6 radical method, 6 classical method, 2 Ogston-Luc method and 3 reconstructions of anterior wall of the frontal sinus using bone sutures). In each group the type of surgical method and the causes of the possible postoperative pitfalls and recurrence of the disease were analyzed.

Revisiting a Rare Cause of Plagiocephaly: Frontosphenoidal Craniosynostosis.

Frontosphenoidal craniosynostosis remains a rare cause of anterior plagiocephaly, with only 49 cases reported worldwide. Delayed diagnosis m … w flattening and preserved brow height. Imaging confirmed frontosphenoidal suture fusion with right frontal bone and supraorbital ridge flattening. Due to the severity of deformity, both underwent fronto-orbital advancement (FOAR) with the affected side bandeau and frontal bone removed, remodeled, and advanced to match the contralateral side. Both were dischar …

Pott's puffy tumour: A report of 2 cases highlighting tuberculosis as a microbiological cause.

Pott's puffy tumour (PPT), characterized by subperiosteal abscess formation secondary to frontal bone osteomyelitis, is a rare clinical entity. This case series highlights tuberculosis as an unexpected cause of PPT. Two cases of PPT, diagnosed histologically and surgically managed within a one-year period, were retrospectively analyzed. Clinical, radiological, operative, and histopathological findings were evaluated. Both cases, aged 12 and 26 years respectively, presented with forehead swelling, headache, and low-grade fever. Imaging revealed subperiosteal abscesses involving the frontal bone…

A case of restitution ad integrum in Gorham-Stout disease.

Gorham-Stout disease (or "vanishing bone" disease) is a rare mono or polyostotic disease of unknown etiology, characterized by intraosseous angiomatous proliferation leading to bone resorption. We report the case of a 17-year-old woman presenting with symptomatic osteolytic lesions of the frontal vault. Imaging was suggestive of Gorham-Stout disease without argument for other diagnoses. An unusual evolution of the "vanishing bone" lesions was observed on the scan after one year, with a full recovery of the lytic lesions. This report shows for the first time a spontaneous restitutio ad integrum

Frontal sinus giant osteoma with radiologically unusual component suggesting blood supply: A case report.

Osteoma is a common, slow growing bone tumor, and often affects the paranasal sinus. Typically, it shows a very hyperdense osseous lesion on computed tomography (CT) scan and low-intensity change on T2-weighted image on magnetic resonance imaging (MRI). No report has mentioned osteomas in blood supply on MRI. A 57-year-old male patient presented with a prolonged declined activity and a gigantic osseous tumor that originated from the frontal sinus, which markedly compressed the bilateral frontal lobe. MRI revealed a slightly enhanced front basal part of the tumor by gadolinium, with blood suppl

Modified approach for ethmoid and anterior skull base surgery.

The use of combined craniofacial resection is well established for tumors of the ethmoid bone and the anterior aspect of the base of the skull. Mobilization of the medial fronto-orbital ridge improves the transbasal approach and can be performed with a monobloc bone flap. We describe an en bloc bifrontal craniotomy including the supraorbital ridges and the nasal bones. This provides a wider angle of approach to the anterior aspect of the base of the skull than any other method and avoids retraction of the frontal lobes. This type of bone flap procedure can be performed after a wide periosteal

Disseminated Coccidioidomycosis With Orbital Osteomyelitis and Periorbital Abscess.

Coccidioidomycosis osteomyelitis involving the orbital bones is exceedingly rare and is often misdiagnosed initially as other inflammatory or infectious conditions. No clear guidelines currently exist regarding appropriate management. The authors present an atypical presentation of disseminated coccidioidomycosis in an immunocompetent child with frontal bone superotemporal orbital rim osteomyelitis and associated periorbital abscess, successfully managed with surgical debridement through an upper eyelid crease incision.
