Osteoplastic Flap Without Obliteration: How I Do It.

Background The osteoplastic flap provides access to pathology of the frontal sinus and is often performed with obliteration, which can result in significant long-term complications. Objective To describe the authors' approach to osteoplastic flap without obliteration. Methods Descriptive analysis. Results Osteoplastic flap without obliteration preserves frontal sinus function while permitting direct access to the frontal sinus. In this study, we demonstrate a combined nonobliterated approach, accessing the frontal sinus with a Draf III approach to remove a large frontal sinus osteoma. Conclusi

[Axillary flap approach to the frontal recess in 20 patients].

OBJECTIVE: To discuss the method and efficacy of Wormald technique application in opening frontal recess surgery. METHOD: Twenty cases with chronic frontal sinusitis underwent surgery with Wormald technique were subjected to this study. Preoperative CT scan and especially sagittal reconstructing were used to indicate the extent of lesion, source and key adjacent structure. RESULT: All patients were followed up for more than half a year with an average of nine months. The effective rate was 90% and no recurrence appeared except for two cases. CONCLUSION: Application of Wormald technique for …

Indications and methods for performance of osteoplastic-obliterative frontal sinusotomy with a description of a new method and some remarks upon the present state of the are of external frontal sinus surgery.

The Goodale-Montgomery osteoplastic obliterative frontal sinusotomy has been the ultimate resource in America for the most difficult and intractable cases of frontal sinusitis. This operation respects the integrity of the frontal floor and is based upon a concept with which the author disagrees: that the frontal sinus may be treated as an entity quite separate form the ethmoid. Macbeth, who does not obliterate, avails himself of the excellent transfrontal approach to the ethmosphenoids, but breaches the frontal floor in so doing. American surgeons have not followed his lead. The pros and cons …

Functional endoscopic sinus surgery in chronic sinusitis--a series of 237 consecutively operated patients.

From 1989 to 1999, 237 consecutive patients with chronic sinusitis and/or nasal polyposis entered a prospective study on the effect of functional endoscopic sinus surgery (FES). Nasal stenosis associated with massive nasal polyposis was the most frequent problem found in 61% of the patients. The rest had long-lasting symptoms of chronic sinusitis. Duration of symptoms averaged 9.3 years. Most frequent symptoms preoperatively were: nasal stenosis, chronic secretion, anosmia, frontal pain, headache and maxillary pain. All patients had the operation performed under general anaesthesia. 86% of the

Benign lesions of the frontal sinus.

Benign tumors of the frontal sinus including fibro-osseous lesions and inverted papillomata present unique problems for the rhinologist. Because of the bony cavity surrounding the frontal sinus and relatively small portal into the nose, conventional techniques including the osteoplastic flap, frontal trephination, and lynch procedure have provided useful approaches to these tumors. The advent of endoscopic techniques in the management of chronic inflammatory disease of the paranasal and, specifically, frontal sinuses, has led to these traditional techniques being augmented successfully, and, i

Frontal lobe abscess secondary to head trauma and nasal polyposis.

Cerebral frontal lobe abscess may be a frequent intracranial complication of paranasal sinusitis. An unusual case of cerebral abscess complicating massive polyposis is described in a patient with history of previous severe head trauma 30 years ago. Imaging evaluation using both computed tomography and magnetic resonance imaging is important to diagnose such rare entities.

Early Practice: External Sinus Surgery and Procedures and Complications.

External approaches to the paranasal sinuses are rarely used in the endoscopic era. However, their indications for use have not changed, and in every surgeon's career those indications may present themselves. For residents training in the endoscopic era, these procedures are also very rarely seen. In this article, the external approaches to the maxillary, ethmoid, and frontal sinuses are described: their original descriptions, modern use, and potential complications. It is hoped that this article will serve to instruct residents and practitioners alike in these techniques.

Lung adenocarcinoma metastasis to frontal sinus mimicking Pott's puffy tumor.

Metastasis of the lung adenocarcinoma to the paranasal sinuses is a rare clinical entity. We present a 75-year-old male patient who presented with swelling of the forehead and left upper eyelid with proptosis in left eye due to metastasis from lung adenocarcinoma. It appears as a puffy swelling of the forehead like a Pott's puffy tumor. Pott's puffy tumor is a subperiostal abscess of the frontal bone associated with osteomyelitis and usually occurs as a complication of sinusitis or trauma.

Open surgical management of an asymptomatic giant frontal sinus osteoma.

Osteomas are benign, slow-growing tumors that arise most frequently in the craniomaxillofacial region, and in particular in correspondence of the nose and paranasal sinuses. Osteomas may be asymptomatic for a long time until they cause deformation of the bone or compress adjacent structures. The treatment of osteomas of the frontal sinus may involve endoscopic approach or open approach. Although endoscopic treatment is the most common approach for frontal sinus osteomas, a coronal approach combined with the use of titanium mesh may still constitute a valuable option for the reconstruction of t

Diffusion-weighted imaging of cerebritis.

Restricted water diffusion has been used to distinguish pyogenic abscess from other rim-enhancing brain masses; however diffusion-weighted imaging of cerebral infection before capsule formation has rarely been described. We report a case of fungal cerebritis in which water diffusion was more restricted than that of normal contralateral brain and the measured diffusion coefficient was in the range of that reported for pyogenic brain abscess. In the proper clinical setting, cerebritis should be considered in the differential diagnosis of an ill-defined focal brain mass associated with markedly r

[Reconstruction of the frontal bone after trauma (author's transl)].

Reconstruction of the frontal bone after trauma has to be performed in a different manner according to whether the frontofacial lesions are associated or not with neurosurgical disturbances. The lesions in the first case mainly affect all the orbital borders and the anterior walls of the frontal sinuses. Reconstruction of the frontal bone usually requires osteosynthesis to the metallic threads of the various fragments applied, but the treatment of the frontal sinus region must be modulated in relation to the possible existence of posterior wall lesions. Frontofacial lesions associated with neu

Dr. Paul Tessier and facial skeletal masculinization.

The male facial skeleton is larger and more angular than that of the female. The male skull has bossing in the area of the frontal sinuses (there is bossing even without presence of the sinuses--5% of people do not have a frontal sinus) and a small flat spot in the mid forehead between the areas of bossing and usually slightly above them. Also there is bossing in the superior lateral orbital angle. The chin on an average basis is 17% vertically higher in the male and there is more fullness laterally. The angle of the mandible is larger inferioro-posteriorly and generally flares out more latera

Pneumosinus dilatans frontalis: two case reports.

INTRODUCTION: Pneumosinus dilatans was described first in 1898 by Meyes. Up to now only 60 cases have been published worldwide dealing mainly with young men between 20 and 30 years of age. The aetiology remains unknown. Whilst frontal sinuses are affected most often other sinuses can also be involved. The expansion of the pneumosinus is not only an aesthetic problem but may also have functional consequences. There may be a bulging of the roof of the orbit with consequent global displacement, ocular alterations and ptosis. Other features are headache and anosmia. PATIENTS: Two male patients wi

Extent of Nasal Irrigation in Non-operated Sinuses: A Cadaveric Trial.

BACKGROUND: Nasal irrigation is among the most widely used treatment modalities for sinonasal diseases. The extent of irrigation is well studied in operated sinuses but not for non-operated sinuses. Moreover, the preferred head positioning during irrigation is not known. Herein, we studied the extent of nasal irrigation in cadaveric heads with non-operated sinuses. METHODS: Nine cadaveric heads were used. Normal saline solution mixed with radiological contrast material was used to irrigate the nasal cavities with the heads in 2 different positions: standing-leaning-forward (SLF) and Head-down

Distribution of CGRP and TRPV2 in Human Paranasal Sinuses.

Immunohistochemistry for protein gene product 9.5 (PGP 9.5), calcitonin gene-related peptide (CGRP) and the transient receptor potential cation channel subfamily V member 2 (TRPV2) was performed on human paranasal sinuses. It was found that in the paranasal sinuses, mucous membranes contain PGP 9.5-immunoreactive (PGP 9.5-IR) nerve fibers. Such nerve fibers terminated around large blood vessels as fine varicosities. Isolated PGP 9.5-IR nerve fibers were scattered beneath the epithelium. Glandular tissues were also innervated by PGP 9.5-IR nerve fibers. These fibers were numerous in the maxilla

Crumple zone effect of nasal cavity and paranasal sinuses on posterior cranial fossa.

OBJECTIVES/HYPOTHESIS: Examine a protective crumple zone effect of paranasal sinuses and nasal cavity on skull base fractures. STUDY DESIGN: Randomized-control, cadaveric study. METHODS: In the experimental group (n = 4), the nasal cavity and bilateral sinuses of cadavers were obliterated with bone cement, whereas the control group (n = 4) had native sinus architecture. Increasing frontal, glabellar impacts were introduced. Each impact event was examined with a high-speed video camera and sphenoid sinus pressure sensor. After each impact, computed tomography scans were performed and fracture
