Use of Preoperative Single Dose Ivabradine for Perioperative Hemodynamic Stabilization During Non-Cardiac Elective Surgery Under General Anaesthesia: A Pilot Study.

BACKGROUND: Peri-anesthetic hemodynamic fluctuations during non-cardiac surgeries are sometimes of serious consequences and associated with increased morbidity and mortality, especially in undiagnosed vulnerable patients. Currently used drugs like β-blocker, α2 agonist or sedative analgesic have their own limitations like combined negative ionotropic and chronotropic action, and unwanted bradycardia associated with hypotension, respectively. In this context, ivabradine has been used extensively in cases of cardiac failure, myocardial ischemia and cardiomyopathies for its funny channel associat

The median effective dose (ED50) of cis-Atracurium for laryngeal mask airway insertion during general Anaesthesia for patients undergoing urinary surgery.

BACKGROUND: In clinical practice, the laryngeal mask airway is an easy-to-use supraglottic airway device. However, the cis-atracurium dosage for laryngeal mask insertion has not been standardised. We aimed to determine the optimal dose of cis-atracurium using a sequential method for successful laryngeal mask insertion. METHODS: The cohort study protocol is registered at clinicaltrial.gov (NCT-03668262). Twenty-three patients undergoing elective urinary surgery were sequentially administered cis-atracurium doses as follows: 150, 100, 70, 50, 30, and 20 μg·kg- 1. The main outcome involved the d

[Case of ulnar nerve palsy in a patient with a history of total elbow arthroplasty after abdominal surgery under general anesthesia].

Ulnar nerve palsy is a well recognized complication of the general anesthesia. We experienced a case of ulnar nerve palsy after abdominal surgery under general anesthesia in a patient with a history of total elbow arthroplasty and ulnar nerve translocation. It appears that pressure on medial epicondyle caused the palsy and that the position of the ulnar nerve has to be carefully examined before surgery in a patient with a history of total elbow arthroplasty and ulnar nerve translocation.

Comparative study of stress response to central venous cannulation under local anesthesia and general anesthesia in patients undergoing open heart surgery.

The open heart surgery like coronary artery bypass graft (CABG), repair of ventricular septal defect (VSD) and atrial septal defect (ASD), valve replacement need a circulatory arrest, which is normally safe under conventional hypothermia. During this time not only the heart and lungs are bypassed but also a wide range of haemodynamic manipulation is necessary. For this reason continuous arterial and central venous pressure monitoring are mandatory. Arterial pressure is monitored by cannulation of the radial or femoral artery and central venous pressure is monitored from internal jugular, subcl

Cost minimisation and cost effectiveness in anaesthesia for total hip replacement surgery, in Belgium? A study comparing three general anaesthesia techniques.

The aim of the prospective randomised study is to compare the cost effectiveness of three general anaesthesia techniques for total hip replacement surgery and the cost minimisation by use of anaesthetics. For induction propofol was used in the three techniques. For maintenance, we used desflurane, or sevoflurane, or propofol. There was no significant difference in consumption of drugs for pain treatment, treatment of nausea and vomiting or cost of hospital stay or total cost for pharmacy. In terms of cost-effectiveness we can consider that the three techniques are similar. The cost of an i.v.

Postoperative Delirium and the Association to Continuously Monitored Severe Deviations of Vital Signs at the General Ward After Major Abdominal Surgery.

BACKGROUND: Delirium is the most frequent neurological complication after surgery and is associated with increased postoperative morbidity and mortality. The underlying pathophysiology is not fully understood, but is suggested to be multifactorial, including hypoperfusion and hypoxia; however, the role of deviating vital signs prior to postoperative delirium (POD) remains unclear. This study aimed to examine the association between severe physiological deviations detected by continuous wireless monitoring and the occurrence of POD within the first three postoperative days after major abdominal

Patient-Reported Perceptions of Surgical Informed Consent Communication in Elective General Surgery: A Cross-Sectional Study in a Chinese Tertiary Hospital.

BACKGROUND: Informed consent supports voluntary surgical decision-making, yet patients' routine perceptions of consent communication remain insufficiently described. In China, where family involvement and time-pressured care are common, understanding how patients retrospectively perceive written and oral consent communication is important. METHODS: We conducted a cross-sectional survey of 627 consecutive adult patients undergoing elective general surgery in a tertiary general hospital in Fuzhou, Fujian Province, China. Participants were interviewed face-to-face on postoperative days 2-7 using…

Association Between Intraoperative Perfusion Index and Mortality in Patients Undergoing General Anesthesia for Noncardiac Surgery: A Protocol for an Observational Study.

BACKGROUND: Hemodynamic derangement during general anesthesia is associated with postoperative morbidity and mortality. Current paradigms for hemodynamic management focus on optimizing macrocirculatory parameters, which may not adequately reflect or improve microcirculatory function. The peripheral perfusion index, a noninvasive measure derived from pulse oximetry, may provide an indication of tissue perfusion status. In smaller studies, the intraoperative perfusion index has been linked to postoperative complications and mortality. This retrospective cohort study will evaluate the association
