[Treatment of clozapine-induced agranulocytosis using granulocyte colony-stimulating factor].

A 20-year-old woman had for the preceding 11 weeks been receiving clozapine (225 mg/d) for an endogenous psychosis when she developed a urinary tract infection with fever. The blood count showed 2100 white cells/microliter without any neutrophils, the count having been normal 5 days previously. Physical examination was normal except for a fever of 39 degrees C and parodontitis. The red cell count was 3.9 mill/microliters, platelet count 443,000/microliters. Bone marrow biopsy revealed almost complete stop of proliferation and maturation in granulocytopoiesis so that granulocyte colony-stimulat

[Treatment of methimazole-induced agranulocytosis with granulocyte-macrophage colony stimulating factor].

The authors treated a patient with methimazol (Metothyrin)-induced agranulocytosis with human recombinant granulocyte-macrophage colony stimulating factor (GM-CSF). On day seven, after combined antibiotics, corticosteroid and at a dose of 270 ug daily subcutaneous GM-CSF therapy the septic state of the patients rapidly cured and the leucocytes reached the peripheric blood. No side effects were found. The publication of this case history might help to determine the place of human GM-CSF-s therapy in the treatment of agranulocytosis of different origin.

[Treatment of neutropenia in Shwachman's syndrome with granulocyte growth factor (G-CSF)].

BACKGROUND: Patients with Shwachman syndrome have neutropenia and depressed neutrophil chemotaxis and are therefore susceptible to recurrent … 0/mm3) and profound depressed chemotaxis. He was given subcutaneous injections of recombinant human granulocytes colony stimulating factor (rhG-CSF), 1 microgram/kg/day, for 15 days. The polymorphonuclear count increased above 1000/mm3 during the second week of treatment, and this effect was seen again during a second course of rhG-CSF. The benefit was not sus …

Clozapine-induced agranulocytosis treated with granulocyte macrophage colony stimulating factor.

Clozapine-induced agranulocytosis is usually reversible after discontinuation of the drug. A patient who developed agranulocytosis after termination of clozapine therapy responded to treatment with granulocyte macrophage colony stimulating factor.

Granulocyte colony-stimulating factor treatment of leucopenia during fractionated radiotherapy.

11 patients suffering from an isolated leucopenia during fractionated radiotherapy were treated with granulocyte colony stimulating factor (G-CSF). 4 of the patients received radiotherapy alone, and 7 patients received concomitant chemotherapy. G-CSF treatment was initiated at the occurrence of leucopenia and maintained for the duration of radiotherapy. The applied daily dose was 5 micrograms/kg subcutaneously. 10 of the 11 treated patients reacted with an increased leucocyte count, from an average of 1342 leucocytes per microliter (+/- 502/microliters) to 24,568 leucocytes per microliter (+/-

Recurrent granulocytic aplasia as clinical presentation of a persistent parvovirus B19 infection.

We report a case of persistent infection with human parvovirus B19 (PVB19), manifesting clinically as recurrent agranulocytosis and in bone marrow biopsy as recurrent pure granulocytic aplasia. Persistence of parvovirus infection was documented by the presence of PVB19 DNA and anti-PVB19-IgM antibodies in the serum for a period of 19 months. Granulocytic aplasia occurred only when anti-PVB19-IgG antibodies were not detectable in the serum and granulopoiesis showed immediate recovery with high dose intravenous immunoglobulin treatment. This case report suggests that the erythroid precursor cell

Granulocyte-macrophage colony stimulating factor for the treatment of drug induced agranulocytosis.

Drug induced agranulocytosis is an uncommon but potentially fatal complication. In some cases, it may be associated with hypoplasia and depletion of granulocytic precursors in the marrow, leading to prolonged neutropenia. We report on the use of granulocytic-macrophage colony stimulating factor (GM-CSF) in two such cases, at a dose of 10 micrograms per kilogram per day subcutaneously. The absolute neutrophil count rose above 500/mm3 in 3 days in both cases. We believe that GM-CSF expedited the recovery of granulocyte counts in our patients and warrants further study in the management of drug i

Indium-111-granulocyte scintigraphy in brain abscess diagnosis: limitations and pitfalls.

The scintigrams and records of 28 patients referred for indium-111-granulocyte scintigraphy (111In-GS) because of a suspected brain abscess were studied retrospectively. The final diagnosis was brain abscess in 8 patients, brain tumor in 18 patients, and infarct and hematoma in 1 patient each. Five patients not on corticosteroid treatment showed intense focal 111In accumulation in abscesses, whereas an abscess patient receiving a high steroid dose showed no uptake. Two patients studied twice showed intense uptake in abscesses when not on steroid therapy or on a low dose, whereas no uptake was

The role of cortisol in the peripheral granulocyte response to insulin-induced hypoglycaemia in man.

Peripheral blood leukocyte counts and plasma hormonal changes in response to acute insulin-induced hypoglycaemia were examined in 16 patient … early rise in lymphocytes at 15 minutes after the acute hypoglycaemic reaction, and a later rise in granulocytes. Free radical and granulocyte-mediated injury during myocardial ischemia and reperfusion.

The success of thrombolytic/reperfusion therapy in limiting the extent of myocardial infarction may be limited by reperfusion injury. Damage from acute ischemia is not due solely to the interruption of blood flow; rather, ischemia initiates a cascade of reactions involving partially reduced oxygen, inflammatory mediators, mechanical capillary obstruction by granulocytes and other events that lead to irreversible injury. A surprising consequence is that reperfusion by delivering oxygen and granulocytes may counteract some of the benefits of restoring flow. Mechanisms of neutrophil and free radi…

Experimental murine schistosomiasis mansoni: inhibition of neutrophil granulocyte inflammatory reaction.

1. The mobilization of neutrophils into the peritoneal cavity following stimulation with sterile mineral oil was studied in normal and schistosome-infected mice. This response was correlated with changes in blood neutrophil counts and with the production and release of medullar granulocytes. 2. The peritoneal neutrophil inflammatory reaction was considerably reduced in schistosome-infected mice. The medullar pool of mature neutrophils was totally depleted, in spite of an increased number of immature neutrophil precursors. 3. We conclude that the decreased inflammatory response of neutrophils d…

Cytogenetic patterns following bone marrow transplantation for chronic granulocytic leukemia.

Allogeneic bone marrow transplantation (BMT) is the only treatment with a potential to cure chronic granulocytic leukemia (CGL). Cytogenetic markers, including the Philadelphia chromosome (Ph), t(9;22)(q34;q11), allow follow-up of these patients after BMT for engraftment and remission status. A variety of cytogenetic patterns have been observed after BMT for CGL. A number of patients experience cytogenetic relapse or mixed chimerism with normal cytogenetics. Furthermore, these types of complications may increase after T-cell depleted BMT. We have closely followed six patients with Ph-positive

The effects of transplantation of granulocyte-macrophage progenitors on bone resorption in osteopetrotic rats.

Osteopetrosis in the ia (incisors absent) rat is the result of reduced bone resorption due to abnormal osteoclasts. This mutant and others have been used to determine the precursor(s) to osteoclasts. Hemopoietic stem cells, isolated from bone marrow of normal littermates, cure the skeletal sclerosis and result in the formation of normal osteoclasts when transplanted into ia rats. These studies were conducted to define further the precursor to the osteoclast by evaluating the effects of the transplantation of granulocyte-macrophage progenitors on bone resorption in the ia rat. Granulocyte-colon
