BACKGROUND: Apocrine cells are a common finding in female mammary cysts, while only rare cases of apocrine metaplasia in gynecomastia have been found in surgical specimens.
CASE: A 65-year-old male presented with painful, monolateral gynecomastia. Fine needle aspiration biopsy showed sheets of large, eosinophilic epithelial cells. On immunocytochemistry these cells were positive for apocrine marker GCDFP-15. The patient had ischemic heart disease and was under treatment with spironolactone.

The patient was a 60-year-old male. He had undergone orchiectomy 5 years earlier because of prostatic cancer. During this interval, he has taken the estrogen. Prepubertal gynecomastia is a rare disorder that may be associated with adrenal or testicular tumors. An adrenal tumor causing a mixed feminizing-virilizing syndrome in a 6-year-old boy was identified by ultrasonography, whereas excretory urography failed to show any abnormality. Highly elevated estrone concentrations were found in the plasma, as were more modestly increased levels of estradiol, testosterone, androstenedione, and 11-deoxycortisol. After removal of the tumor, all hormonal concentrations returned to normal prepubertal levels.
