Current Status of the Diagnosis and Management of Osteoporosis.

Osteoporosis has been defined as the silent disease of the 21st century, becoming a public health risk due to its severity, chronicity and progression and affecting mainly postmenopausal women and older adults. Osteoporosis is characterized by an imbalance between bone resorption and bone production. It is diagnosed through different methods such as bone densitometry and dual X-rays. The treatment of this pathology focuses on different aspects. On the one hand, pharmacological treatments are characterized by the use of anti-resorptive drugs, as well as emerging regenerative medicine treatments

A case of multiple myeloma with navicular bone involvement.

Multiple myeloma is a haematological disease caused by proliferation of malignant plasma cells in bone marrow. It frequently has lytic bone lesions. However, involvement of the small bones of the hands and feet is extremely rare. We report a unique multiple myeloma patient with first recurrence in navicular bone after allogenic stem cell transplantation.

Atypical Location of Enchondroma and its Management - A Case Report.

INTRODUCTION: Enchondromas are most commonly found in the bones of the hand and feet. Although the location in the feet is not uncommon the enchondroma of the calcaneus is a rare finding. CASE REPORT: In this case report, we present a 45-year-old male patient who was diagnosed with enchondroma of the calcaneum. The power burr was used to perform an extended curettage, and the harvested cortico-cancellus bone from the iliac crest was used to fill the cavity; following which the calcaneal plate was applied. At the end of the 1-year follow-up, the patient was asymptomatic, and there was no evide

Chronic Recurrent Multifocal Osteomyelitis in a Child's Hand: Case Report and Review.

We present a 9-year-old girl with persistent pain and swelling of the left wrist. X-ray, magnetic resonance imaging (MRI) and bone biopsy led to the diagnosis of chronic recurrent multifocal osteomyelitis (CRMO), affecting phalangeal and metacarpal bases and distal carpal bones on the ulnar side of the wrist. She was treated with non-steroidal anti-inflammatory drugs and complete remission with no long-term sequelae was achieved. CRMO is a rare auto-inflammatory condition with infrequent involvement of the hand. The current literature is discussed. The aim of this report is to raise awareness

Primary osteosarcoma of the parietal bone.

Osteosarcoma is the most common primary bone tumor and usually involves the long bones. Osteosarcoma of the skull, on the other hand, is relatively rare. Here, we present a 29-year-old man with a growing mass in the skull he first noticed after a fall while skateboarding. The initial clinical diagnosis was hematoma. While undergoing an evacuation surgery for a hematoma, a suspicious mass was detected which was biopsied. Histopathological evaluation showed high-grade osteosarcoma. The patient was referred to our hospital where he underwent definitive resection followed by adjuvant chemotherapy.

Case Report: Multifocal Tubercular Osteomyelitis of the Spine and Bilateral Dactylitis.

We report a case of a 13-year-old immunocompetent male with multifocal tubercular osteomyelitis involving several spinal segments, small bones of the hands, and the scalp, who started with progressively back pain and enlarging painful swelling on the palms of hands, fatigue, and irregular fever. All the hand lesions were firm, mildly tender, and covered by ulcerated skin with serous discharge from the site. Magnetic resonance showed lesions of the right fifth metacarpal, of the right intermediate phalanx of the fourth finger, of the left second metacarpal, and of most vertebral bodies of the c

Treatment of neglected, severely displaced, open malunion of the distal radius and ulna in an elderly person with Alzheimer's disease - a case report.

The case of a patient suffering from Alzheimer's disease who sustained epiphyseal, severely dislocated fractures of the distal radius and ulna is reported. As the patient was incapacitated and attempts to contact her curator failed, the decision was made on a conservative treatment of the fractures. This treatment eventually failed and resulted in malunion with exposure of bone fragments above the skin. After obtaining informed consent from her curator, a corrective osteotomy was performed using an original method, followed by fixation of bone fragments in almost anatomical position by intrame

Age estimation of unaccompanied minors. Part I. General considerations.

In recent years many countries have experienced a sharp increase in the demand for forensic age estimates of unaccompanied minors. In many countries the age thresholds of relevance to criminal prosecution lie between 16 and 22 years. In line with recommendations issued by the Study Group on Forensic Age Diagnostics, for determining the age of live subjects a forensic age estimate should combine the results of a physical examination, an X-ray of the hand and a dental examination which records dentition status and evaluates an orthopantomogram. To assess the age of persons who are assumed to be

The rectus abdominis free flap as an emergency procedure in extensive upper extremity soft-tissue defects.

Stable wound coverage after extensive soft-tissue loss of the upper extremity remains a difficult problem in the management of large defects of the upper limb. To prevent further tissue loss owing to infection or inadequate cover when important structures such as vessels, tendons, nerves, joints, and bones are exposed, various free flaps have been introduced into the therapeutic armamentarium of acute plastic surgical management options. Emergency or delayed early reconstruction has been proposed to prevent chronic infection and further tissue loss. We report a series of 12 emergency and delay

Osteoid osteoma of the phalanx: enlargement of the toe--two case reports.

After a review of the literature, two cases of osteoid osteoma involving phalanges of the toes are reported. Physical findings of osteoid osteoma vary with the site of the tumor; in the foot, particularly the toes, it presents various atypical and characteristic features. Its more frequent cancellous and subperiosteal localization in the small bones of hand and foot, showing atypical roentgenographic findings, poses particular problems in diagnosis. In the osteoid osteoma of the phalanges, characteristic enlargement of the toe or finger seems to be frequent and with a typical clinical history

[Traumatic axial dislocation injury of the carpus and metacarpus with triquetrum fracture. Rare entity or typical injury].

Although there are only 39 reports of compression injuries with axial dislocation of the carpus and metacarpus, there are similar patterns of injury. In all cases the dislocation is located either between the first and second or the third and fourth metacarpals and the corresponding carpal bones so that weak points of the carpal arch can be assumed in these regions. Clinical and radiological findings in a 22-year-old patient with dislocation of the third and fourth metacarpals and a fracture of the os triquetrum of the right hand after compression injury of both forearms with severe soft tissu

The lateral arm fascial free flap for resurfacing of the hand and fingers.

The lateral arm free flap can be harvested as a fascial flap or fasciocutaneous flap. In this report we describe the use of the lateral arm fascial flap for degloving injuries of the fingers and for skin loss on the dorsum of the hand with exposure of tendons and bones. Concomitant reconstruction of a missing phalanx with a portion of the distal humerus is also described. The use of the fascial flap allows a large area of tissue to be harvested, and still, the donor site can be closed primarily. The fascia is thin and pliable and so conforms well to the contour of the fingers. Its bulk does no

Injuries to the ulnar carpometacarpal region: are they being underdiagnosed?

Thirteen cases with radiographic evidence of injury to the ulnar side of the hand are reviewed. These injuries included intraarticular fractures and/or dislocations of the fourth and fifth carpometacarpal joints and associated bones collectively discussed here as the ulnar carpometacarpal region. In nine the extent of the injury was not appreciated on first presentation. Injuries to this region are often difficult to assess clinically and radiographically even in experienced hands. In our experience computerized tomography (CT) scans in the longitudinal and longitudinal oblique axis best displ

Diagnostic value of hand X-rays in Turner's syndrome.

Previously described radiological signs of Turner's syndrome were evaluated in X-rays of the left hand from 17 patients with Turner's syndrome (age 10.4--15.3 years) and 17 age-matched girls with constitutional short stature. While none of the signs clearly distinguished between the two groups, ballooning of the tips of the terminal phalanges with a high ratio between the tip and mid-shaft diameters seemed to be the most useful of Turner's syndrome. Disproportionately long phalanges in the 4th finger and the presence of a coarse reticular pattern in the carpal bones appeared to be the next mos

[Nuclear spin tomography of the hands of a patient with Maffucci's syndrome].

Maffucci's syndrome is associated with tumours of the soft tissues and enchondromas. The hands are involved in the majority of patients. Magnetic resonance imaging (MRI) enables the demonstration of the soft tissue structures and the bone marrow of the hand. The hands of 19-year old patient with Maffucci's syndrome were examined via MRI and the results were compared with radiographs, angiograms and skeletal scintiscans of the hands. MR imaging shows a more expansive infestation with enchondromas than the radiographs. Haemangiomas were also visualised.

One-bone forearm reconstruction using vascularized fibular graft for massive forearm soft-tissue and bone defect: case report.

The authors report a case of recurrent malignant fibrous histiocytoma with extensive bone- and soft-tissue involvement, successfully treated with one-bone forearm reconstruction using a vascularized fibular graft and multiple tendon transfer. Twenty-four months after surgery, elbow and hand functions were maintained, and the patient had no disturbance of hand function in daily activity, although rotation of the forearm was sacrificed. No local recurrence or metastasis was noted. The procedure is a useful reconstructive option for complex tissue defects following wide excision of sarcoma of the

[Upper limb lengthening. 47 clinical cases].

Between 1989 and 1993, the authors treated 39 patients by progressive bone lengthening in a total of 47 hands (17 post-traumatic and 30 congenital deformities). The Hoffman distractor was used in 34 cases and the Orthofix device was used in 13 cases. The percentage of lengthening obtained, compared to the initial bone length, was 87% for the hand skeleton and 47% for the forearm bones in congenital malformations while only 51% was obtained in post traumatic cases due to their older age and frequently associated skin problems. In 4 (8.5%) of these post-traumatic cases, the skin conditions had t

Application of in vivo activation analysis to the compartmental study of sodium in the hand.

Sodium kinetics were studied in the hands of 14 subjects by local in vivo neutron activation analysis. Hands were irradiated with 252 Cf sources giving absorbed doses of 8 cGy. The variation in 24Na radioactivity was plotted against time, and each curve fitted to a function that was the sum of two exponentials. Two pools were identified, one corresponding to fast turnover, with a half-life of about one hour, the other to a slower turnover process with a half-life averaging 79 h if the calcium concentration of the hand bones is normal, and 35 h if it is subnormal. The mean ratio of calcium to s…

HR-pQCT in vivo imaging of periarticular bone changes in chronic inflammatory diseases: Data from acquisition to impact on treatment indications.

Imaging is essential for the assessment of bone and inflammatory joint diseases. There are several imaging techniques available that differ regarding resolution, radiation exposure, time expending, precision, cost, availability or ability to predict disease progression. High-resolution peripheral quantitative computed tomography (HR-pQCT) that was introduced in 2004 allows the in vivo evaluation of peripheral bone microarchitecture and demonstrated high precision in assessing bone changes in inflammatory musculoskeletal diseases. This review summarizes the use of HR-pQCT for the evaluation of

Antibiotic-Loaded Calcium Sulfate as an Adjunct in the Acute Management of a Traumatic Segmental First Metacarpal Defect.

Traumatic segmental bone defects of the hand are uncommon injuries that may require complex reconstruction to restore skeletal stability and function. Calcium sulfate is widely used as a biodegradable bone void filler and local antibiotic carrier, although its use during the acute management of traumatic metacarpal defects has been rarely reported. We present the case of a 55-year-old man who sustained a severe open injury to his dominant hand caused by a radial saw, resulting in dissociation of the first and second rays, segmental bone loss involving approximately 40% of the first metacarpal
