BACKGROUND: Spasticity in patients with infantile cerebral palsy may lead to progressive subluxation and/or hip dislocation.
MATERIAL AND METHODS: Retrospective, cross-sectional trial of a cohort of patients with infantile cerebral palsy who underwent subtrochanteric osteotomy of the hip at the Shriner's Hospital for children in Mexico City, with a 5-year follow-up. X-ray evaluation was conducted pre- and postoperatively and at the 5-year follow-up using the Settecerri classification.
RESULTS: Twenty cases were analyzed; mean age was 8.8 years. They included spastic quadriplegia, spastic dip…

We report the outcome of closed reduction of traumatic hip dislocation in 28 children younger than 16 years of age (average age, 8 years 10 months). At an average follow-up of 8 years and 10 months (range, 2 years - 42 years), 18 patients were asymptomatic and enjoying a normal life, and some of them returned to practice sport activities. The remainder had complications related to associated injuries. There were no cases of avascular necrosis of the femoral head during follow-up. The majority of dislocations were attributable to low energy injuries in children younger than 10 years of age. Tra…

Proximal femoral fracture (PFF) has long become a target for intervention with pharmacologic agents and pharmacologic intervention has been shown to reduce the fracture risk at least by half, compared to placebo, but PFF still continues to increase in incidence. Causes of fracture include bone-related and other factors but data remains yet to be accumulated on the relationship between these causes and lifestyle-related factors, such as nutrition, exercise and lifestyle preferences, which should be essential to establishing osteoporosis as a lifestyle-related disease. Furthermore, despite the p…

Total hip arthroplasty (THA) is one of the most common surgical procedures performed by orthopedic surgeons. Frequent indications for THA include osteoarthrosis of the hip joint that can be primary origin or secondary to dysplasia or traumatic origin, as well as fractures of the femoral neck in active patients. Other common indications include rheumatoid arthritis and osteonecrosis of the femoral head. It is a surgical intervention giving very good results giving patients pain relief and mobility with a success rate in more than 90% of cases. The point of this article is to inform the medical …

The indications for hip arthroscopy are expanding as the understanding of hip disease increases. Improved instrumentation and technical skills also have facilitated the ability to treat some hip disorders arthroscopically. Femoroacetabular impingement (FAI) is increasingly recognized as a disorder that can lead to progressive intra-articular chondral and labral injury. Although FAI is usually treated through an open approach, limited-open and all-arthroscopic approaches have been described. Various arthroscopic techniques allow treatment of labral and acetabular rim pathology as well as periph…

The authors define adolescence and developmental dysplasia of the hip (DDH). Special attention is paid to pathological findings characteristic of DDH in adolescence (unrecognized and untreated DDH; treated DDH, but non-terminated treatment; DDH diagnosed with delay, inadequately treated, with complications). The authors emphasise that DDH treatment has to be successfully terminated well before the adolescence; possibilities are explained on management modes at the time of adolescence, and possible persons guilty for the persistence of later hip problems are indicated. Based on the authors' exp…

Mechanical impingement of the proximal femur against the acetabular rim is now recognized as a common cause of labral tears and degenerative changes in the hip. Open surgery is successful in treating this condition if damage to the articular cartilage surface is minimal.

We report a case of a 19 year old male with Down's syndrome who presented with severe functional disability and pain secondary to bilateral hip arthritis. He had longstanding developmental dysplasia of the hip (DDH) that had evolved into end stage arthritis. Following a comprehensive assessment and counselling, he underwent bilateral total hip replacements. At last follow up, he had excellent outcome for function and pain relief. This case highlights the good results achievable with aggressive management of musculo-skeletal conditions (especially hip arthropathy) in those with Down's syndrome …

BACKGROUND: This retrospective study was carried out to identify prognostic factors that can predict a better long-term outcome.
METHODS: Between 1 January 1993 and 31 December 2004, 97 patients were treated at King Fahd University Hospital, AlKhobar. Fifty-eight patients were analyzed. Hips were evaluated on functional basis as per the classification of Merle d'Aubigne.
RESULTS: There were 52 males and 6 females, with a mean age of 29.6+/-9.8 (18-57) years and follow-up of 59.5+/-18.8 (24-120) months. In 43 patients, the dislocations were due to road traffic accidents. In 57 patients, close…

Hip arthroplasty is performed nowadays according to the needs of the patients irrespective of their age. Tapered rectangular stems for cementless fixation are chosen in most cases in central Europe. They provide primary stability by press-fit implantation into a precisely rasped osseous bed and secondary stability by bone ingrowth into the highly biocompatible titanium alloy with a microrough surface. The 10-year survival of such devices is 92%. Typical radiographic patterns include cortical atrophy and radiolucent lines in Gruen zones 1 and 7. They are due to stress shielding with these dista…

Osteolysis and aseptic loosening are the major causes of failure of total hip replacements (THRs). These processes are induced by wear debris released from components of implants over time. Wear particles are a clinically relevant size (0.1-10 microm) that activates macrophages and induces a foreign-body response. Activated macrophages stimulate the production of proinflammatory cytokines (IL-1, IL-6, and TNF-alpha) to the synovial capsule, leading to chronic inflammation and bone resorption. Wear mechanics, including characteristics of wear particles from metal, polyethylene, and ceramic arti…

The recent advances in hip arthroscopy have made it a useful minimally invasive approach to the diagnosis and treatment of intra-articular hip pathology. It offers the benefits of being an outpatient procedure with short rehabilitation and few reported complications and does not hinder opportunities for future surgical interventions. The lateral approach has been widely accepted, and technical details are discussed. Patients typically present with mechanical symptoms and persistent pain resistant to a reasonable trial of conservative treatment and are without significant joint space narrowing.…

Instability after total hip arthroplasty (THA) is not a rare occurrence. Numerous factors have been associated with dislocation including surgical approach, implant design, failure to restore proper hip mechanics and soft tissue restraints, and patient variables such as early postoperative compliance, soft tissue integrity, and neurologic conditions such as poor proprioception. A thorough understanding of the mechanism of dislocation, timing, and direction of dislocation is mandatory in formulating an approach toward treatment. The radiographic evaluation should evaluate hip mechanics includin…

Dislocation is one of the most common complications after total hip arthroplasty (THA). Risk factors include neuromuscular and cognitive disorders, patient non-compliance, and previous hip surgery. Surgical considerations that must be addressed include approach, soft-tissue tension, component positioning, impingement, head size, acetabular liner profile, and surgeon experience. Recent improvements in posterior soft-tissue repair after primary THA have shown a reduced incidence of dislocation. When dislocation occurs, a thorough history, physical examination, and radiographic assessment help in…

Osteoporosis is characterized late in the course of the disease by an increased risk of fracture, particularly in the elderly. It occurs in … egular exercise, to maintain muscle strength. Bisphosphonates have been shown to reduce the risk of hip fracture. For elderly patients, the use of hip protectors may be used as a treatment of last resort. Regardless of the age of the patient, individual patient risk factors must be considered to target appropriate treatment and prevent fracture.

The authors report the importance of hip dislocation and pelvic imbalance in patients with cerebral palsy, as the cause of pain, decubitus, and severe secondary deformities of the vertebral column. They report their experience in the surgical treatment of spastic hip, based on 231 cases with a mean follow-up of 5.4 years. The authors emphasize the importance of obtaining a balanced pelvis to allow for walking and sitting in the severest patients, and in order to avoid severe deformity of the vertebral column. The surgical methods used, with particular interest in the importance of stabilizing …
