The Children's National Hospital Outcomes Registry: a plea for a benchmarking tool that provides longitudinal outcomes for patients and families.

Surgical advancements in paediatric cardiovascular surgery have led to improved survival rates for those patients with the most complex CHDs leading to greater numbers of patients who are living well into adulthood. Despite this new era of long-term survival, our current reporting systems continue to focus largely on using short-term postoperative outcomes as the criteria to both rate and rank hospitals. Using such limited criteria to rate and rank hospitals may mislead the intended audiences: patients and families. The goal of this article is to describe the creation of a local benchmarking r

Atypical pathogen infection in community-acquired pneumonia.

Community-acquired pneumonia (CAP) is a world wide cause of morbidity and mortality. The etiology of CAP is different between countries and changes over time. With the increasing incidence, atypical pathogens are attracting more and more attention all over the world. In many countries, atypical pathogens are one of the main pathogens of CAP, and even could be the most prevalent etiology in China. Atypical pathogen infections can cause multi-system complications, which leads to a worse prognosis. Although still controversial, empirical antibiotic coverage of atypical pathogens in CAP may improv

Incompetency to stand trial. Appropriateness and outcome.

Of 85 persons (38% of those found incompetent to stand trial in Los Angeles County in 1983), 92% were currently charged with felonies and 62% with crimes of violence. Eighty-seven percent had a history of serious physical violence against persons and 68% had prior felony arrests. This study indicated that in this jurisdiction, incompetency to stand trial is not being used to divert mentally ill persons, charged with minor offenses, into intermediate or long-term psychiatric hospitalization to circumvent obstacles such as restrictive commitment laws and rapid hospital discharge policies. The la

Role of systemic immune-inflammatory index and systemic inflammatory response index in predicting the diagnosis of necrotizing pneumonia in children.

BACKGROUND: Necrotizing pneumonia (NP) is a rare serious complication of community-acquired pneumonia (CAP) in children, which is characterized by a protracted course of the disease and a prolonged hospital stay. This study aimed to assess the role of systemic immune-inflammatory index and systemic inflammatory response index in predicting early lung necrotization in children with CAP. METHODS: This study included all children hospitalized in Pediatric Pulmonology Unit, Tanta University, Egypt, with CAP between the ages of two months and 18 years. Systemic inflammatory indices, including the

Death of a Healthcare Member: A Systematic Guide for Hospital Systems.

The death of a healthcare team member often has a significant impact on co-workers, hospital systems, friends, and family. A large community can be affected by grief, shock, and stress when faced with the tragedy of death, and there is limited information about the resources and guidelines that should be initiated. In the face of inadequate guidelines, we present a flexible procedural framework designed to effectively manage the series of events after the death of a healthcare team member occurring within or outside the hospital premises. This framework was developed within a prominent level 1

Bridging the Gap in Chronic Disease Management: A Nursing Perspective on the Use of Predictive Tools and Telemedicine in the Hospital-Community Transition.

Chronic diseases represent one of the most complex, costly, and significant challenges for healthcare systems. The increase in chronic conditions and multimorbidity, together with the growing demand for continuity of care makes the vulnerability of the hospital-to-community transition increasingly evident. This phase is often characterized by delays, fragmented services, and insufficient support for patients and caregivers, leading to higher rates of early readmission and substantial clinical, social, and economic impacts. This paper was developed through a narrative synthesis of international

Management of frail older people with acute illness.

Frailty status is intrinsically related to every aspect of older patients' hospital journeys: the way in which they present to hospital, their health status at admission, vulnerability to complications in hospital and rate of recovery after an acute insult. In younger people, hospitalisation is usually the result of a serious illness or injury, such as sepsis or major trauma. Management can be underpinned by evidence-based algorithms relating to the precipitating insult and recovery usually follows a predictable trajectory. In older people who are frail, on the other hand, admission to hospita

The Ukraine War: Traumatic Brain Injury in a Front-Line Hospital.

The location close to the front lines of Mechnikov Hospital in the eastern Ukrainian city of Dnipro creates both opportunities and challenges. The same medical teams have worked closely together for years. They have learned how to provide the best possible care despite personnel shortages and overwhelming patient volumes, including many patients who are critically ill with systemic as well as neurological injuries. Outcomes are better than many might expect. International support and collaboration have been instrumental in achieving these good outcomes. Mechnikov Hospital neurosurgeons have de

Variation in Acute Care Rehabilitation and 30-Day Hospital Readmission or Mortality in Adult Patients With Pneumonia.

IMPORTANCE: Pneumonia often leads to functional decline during and after hospitalization and is a leading cause of hospital readmissions. Physical and occupational therapists help improve functional mobility and may be of help in this population. OBJECTIVE: To evaluate whether use of physical and occupational therapy in the acute care hospital is associated with 30-day hospital readmission risk or death. DESIGN, SETTING, AND PARTICIPANTS: This cohort study included the electronic health records and administrative claims data of 30 746 adults discharged alive with a primary or secondary diagn

Healthcare conflict management in resource-constrained settings: evidence from two hospitals in Ghana.

BACKGROUND: Conflict is a recurring feature of healthcare delivery, especially in hospitals that operate with limited staff, inadequate supplies, and heavy workloads. Although conflict among healthcare workers in Ghana has been documented, little research has explored how it emerges within the everyday routines of resource-constrained hospitals or how staff interpret and respond to it. This study examined the causes, effects, and management of institutional conflict in two public hospitals in the Effutu Municipality, using Organizational Information Theory and the Institutional Logics Perspect

A hospitalist-led short-stay rehabilitation unit reduced inpatient discharge delays.

Hospitalized patients requiring post-acute rehabilitation often remain in acute care while awaiting skilled nursing facility placement. In July 2023, the San Francisco VA Health Care System opened a hospitalist-led short-stay rehabilitation unit within its Community Living Center, called the Lotus. Mean referral-to-discharge time, excluding referral hold periods, decreased from 6.89 days in fiscal year (FY) 2023 to 4.58 days in FY2024 and 3.35 days in FY2025. The Lotus became the lowest-delay post-acute care destination and the facility's most frequently used individual rehabilitation destinat

[Implementing infection prevention and control: principles and barriers.].

Healthcare-associated infections (HAIs) are infections that occur during hospitalization or healthcare treatment, representing a major public health threat. They have serious consequences: clinical (deterioration of conditions, increased mortality and disability), economic (costs to families and healthcare systems), and legal (professional liability). A substantial proportion of HAIs are preventable. Antimicrobial resistance further complicates management, with multidrug-resistant strains on the rise in Europe and globally. A fundamental pillar of effective programs against HAIs and antibiotic

Scarcity in the Covid-19 Pandemic.

As we write, U.S. cities and states with extensive community transmission of Covid-19 are in harm's way-not only because of the disease itself but also because of prior and current failures to act. During the 2009 influenza pandemic, public health agencies and hospitals developed but never adequately implemented preparedness plans. Focused on efficiency in a competitive market, health systems had few incentives to maintain stockpiles of essential medical equipment. Just-in-time economic models resulted in storage of only those supplies needed then. At the same time, global purchasing in search

[How to reduce the risk of Legionnaires' disease?].

Legionella pneumophila is a microorganism present in water and able to develop pneumoniae through inhalation of contaminated aerosols. Legionnaires'disease is rare (1000 cases per year in France) concerning particularly people with risk factors, but potentially with severe prognosis (lethality 15-20%). The diagnosis was recently facilitated by the introduction of urinary antigen test. Hospital is the most frequent place where L. pneumophila were acquired; however the proportion of nosocomial cases is decreasing since 2001. The associated mortality is higher for hospital acquired cases than for

Analysis of the nature and contributory factors of medication safety incidents following hospital discharge using National Reporting and Learning System (NRLS) data from England and Wales: a multi-method study.

INTRODUCTION: Improving medication safety during transition of care is an international healthcare priority. While existing research reveals that medication-related incidents and associated harms may be common following hospital discharge, there is limited information about their nature and contributory factors at a national level which is crucial to inform improvement strategy. AIM: To characterise the nature and contributory factors of medication-related incidents during transition of care from secondary to primary care. METHOD: A retrospective analysis of medication incidents reported to

Preparedness of dentists to treat patients with intellectual and learning disabilities: a survey in Northern Thailand.

BACKGROUND: Individuals with intellectual and learning disabilities (ILD) face significant barriers to dental care and experience disproportionately poor oral health outcomes. Evidence on dentists' preparedness, confidence, and stress in providing care for this population across public hospital settings in Thailand remains limited. METHODS: A cross-sectional survey was conducted among 306 dentists in public-sector hospitals across 17 provinces of Northern Thailand (November 2024 to May 2025). A validated online questionnaire assessed individual-, institutional-, and system-level preparedness
