Is rotator cuff repair appropriate in patients older than 60 years of age? prospective, randomised trial in 103 patients with a mean four-year follow-up.

BACKGROUND: The objective of this randomised trial was to compare surgical rotator cuff repair to simple decompression by acromioplasty and … group, the acromio-humeral distance was significantly shorter (6.9 mm/7.8mm, P=0.03) and eccentric humeral head position was more common (44%/26%, P=0.01). DISCUSSION: Arthroscopic rotator cuff repair provides better functional outcomes than does simple decompression in patients older than 60 years and prevents cuff tear arthropathy with eccentric humeral he …

Shoulder deformities from obstetrical brachial plexus paralysis.

Abnormalities are described in the shoulders of 11 patients up to 17 years of age who have chronic brachial plexus paralyses from birth injuries. These abnormalities include a poorly formed and hypoplastic humeral head, a short abnormally formed clavicle, and a hypoplastic elevated scapula with a shallow glenoid fossa, inferiorly directed coracoid process, and abnormally tapered acromion. Four also had subluxated shoulders.

Shoulder arthroplasty for traumatic avascular necrosis: predictors of outcome.

Posttraumatic avascular humeral head necrosis may require salvage shoulder arthroplasty in symptomatic cases. We tested the hypothesis that associated tuberosity malunion negatively influences the final clinical outcome after shoulder arthroplasty. Thirty-eight patients with posttraumatic avascular necrosis of the humeral head were followed clinically and radiographically for a minimum of 5 years (mean, 8 years; range, 5-13 years). To quantify preoperative tuberosity malunion, two novel radiographic parameters were introduced: on the anteroposterior view, the greater tuberosity offset as a mea…

Ultrasonography of shoulders in spondyloarthritis and rheumatoid arthritis: a case-control study.

INTRODUCTION: Shoulders are often involved in spondyloarthritis (SpA) and rheumatoid arthritis (RA). The diagnosis of peripheral SpA and its differential diagnosis with RA could be challenging. A recent ultrasound study showed that ultrasonography (US) of the hands might differentiate psoriatic arthritis to RA. The aim of the study was to compare different US features in SpA, RA and healthy controls. METHODS: A total of 38 SpA and 43 RA patients with clinical involvement of shoulders were included and compared to 33 controls. One blinded rheumatologist performed US examinations. The following

Post-traumatic osteonecrosis of the proximal humerus.

Post-traumatic osteonecrosis of the proximal humerus represents a challenging problem to the surgeon. It is commonly seen following multi-fragmentary fractures of the proximal humerus which may affect the long-term functional recovery after such injuries. This review summarises the current evidence on risk factors, reasons why estimating its epidemiology is difficult, the vascular supply of the humeral head, classification, and management options.

Rhizomelic spondyloepimetaphyseal dysplasia.

Spondyloepipmetaphyseal dysplasias (SEMDS) are characterized by flattening of the vertebral bodies and epi-metaphyseal involvement of all the tubular bones. We report a 13 year-old boy with a novel variant of SEMD that presents with rhizomelic/mesomelic shortening of the extremities and normal head, hands and feet. Variable metaphyseal involvement and relatively large proximal humeral and knee epiphyses are further distinctive radiographic features in this patient.

The ultrasound diagnosis of posterior shoulder dislocation associated with Erb's palsy.

A case is presented of an infant with Erb's palsy whose posterior dislocation of the humeral head was identified using high-resolution ultrasound following non-diagnostic plain radiographs. Posterior dislocation is associated with Erb's palsy but the diagnosis can be delayed. We suggest the early use of ultrasound in patients with Erb's palsy, as this technique is a safe, quick and reliable method of excluding posterior dislocation.

Luxatio erecta.

Luxatio erecta or inferior dislocation of the shoulder joint is rare and is caused by a hyperabduction injury. The clinical appearance is characteristic, with the arm locked in an elevated position. Radiographically, the shaft of the humerus is directed upwards and the humeral head lies inferior to the glenoid fossa, although not in contact with it. Attention should be given to possible complication, particularly concurrent fractures, as well as injury to the brachial plexus or the axillary artery.

Bony defects in chronic anterior posttraumatic dislocation of the shoulder: Is there a correlation between humeral and glenoidal lesions?

BACKGROUND: The prevalence of combined humeral and glenoid defects varies between 79 and 84 % in case of chronic posttraumatic anterior shou … y contralateral shoulder. The following measurements were taken: D/R ratio (Hill-Sachs lesion depth/humeral head radius) on an AP X-ray in internal rotation and the D1/D2 ratio [diameter of the involved glenoid articular surfaces (D1)/the healthy one (D2)] on a comparative Bernageau glenoid profile view. [Research progress on intra-articular screw penetration in proximal humeral fracture treated with locking plate].

OBJECTIVE: To review the research progress on intra-articular screw penetration in proximal humeral fracture treated with locking plate. ME … dary screw penetration is related to the loss of reduction and varus, collapse, and necrosis of the humeral head. The risk factors for intra-articular screw penetration include the bone mass density, the fracture type, the quality of fracture reduction, the applied location, number, and length of the plate and screws, and whether medial column buttress is rest …

Is the humeral stem useful in anatomic total shoulder arthroplasty?

PURPOSE: Traditionally and since Neer, the humeral side of shoulder arthroplasty consisted of a stemmed component but the real need for stem fixation in total shoulder arthroplasty (TSA) has barely been investigated. The current study evaluated the clinical and radiological outcomes with a stemless TSA. METHODS: Forty-seven patients, 20 female and 27 male patients with an average age of 63, were selected in four orthopaedic centres during a four year period, and implanted with a humeral head prosthesis with a three-fin design and titanium coating. Aetiologies were: primary osteoarthritis (29)…

[Progress in the research of medial column reconstruction of proximal humerus fractures in the elderly].

The proximal medial column of the humerus is a continuous cortical region in the inner and lower part of the humerus head, which has attracted more and more attention in clinical and scientific research since it was proposed. It has been shown to increase the stability of internal fixation, maintain the height of the humeral head to prevent varus, and reduce the risk of screw penetration. Biomechanical studies have also shown that the medial column has an outstanding performance in increasing the stiffness, torsion resistance, and shear resistance of the locking plate. Although it has many ben…

[MRI in dislocation and instability of the shoulder joint].

The value of magnetic resonance imaging (MRI) in shoulder instability depends on its ability to demonstrate pathologic lesions within the joint. Traumatic dislocation in particular often results in capsulo-labral-avulsion, stretching of the joint capsule and impression fracture at the humeral head. Various MR weightings, including spin echo and gradient echo, are useful at the shoulder. A surface coil should be used. Image planes are oriented at the scapula. A capsulo-labral-avulsion can not be reliably demonstrated on non-contrast imaging in the absence of intra-articular effusion. With indir…

Shoulder function after displaced fractures of the proximal humerus.

It is difficult to predict the outcome of comminuted fractures of the proximal humerus and give guidelines for optimal treatment. Different treatment modalities are available, and there are various scoring systems for postoperative evaluation. The aim of this study was to assess the functional outcome after three- and four-part fractures of the humeral head in 38 patients, with a 3-year follow-up. Twenty-eight patients were treated conservatively, seven were operated on with open reduction and internal fixation, and three received a hemiarthroplasty. All patients were examined clinically and r…

Gorham's disease: treatment with an autologous iliac bone graft and a reverse total shoulder arthroplasty.

BACKGROUND: Gorham's disease (GSD) is a rare osteolytic disease with unclear etiology, and no known prevention or effective treatment. Here we report a new surgical treatment for a case of GSD in September 2017. CASE PRESENTATION: We report GSD in a 52-year-old woman. She had disappearance of her humeral head and a defect of the glenoid bone in her left shoulder joint, which were serious obstacles to joint function. We used an autologous iliac bone graft to repair the glenoid bone defect and a reverse total shoulder arthroplasty. After surgery, humeral osteolysis did not continue, and her sho…

Biceps tendinitis caused by an osteochondroma in the bicipital groove: a rare cause of shoulder pain in a baseball player.

Tendinitis of the long head of the biceps brachii muscle is commonly seen in athletes who do repetitive overhead motions. Common causes of biceps tendinitis include impingement syndrome, subluxation of the biceps tendon, and attrition tendinitis, whereas biceps tendinitis secondary to a bone neoplasm is rare. A case of biceps tendinitis caused by an osteochondroma arising in the left humeral bicipital groove in a 25-year-old male baseball player is reported. The tumor was hook-shaped, originated from the inferomedial portion of the humeral lesser tubercle, and surrounded the biceps tendon. Sym
