Recognition of secondary arterial hypertension is one of the main purposes of diagnosis in patients with arterial hypertension. Secondary hyperaldosteronism is one of the most frequent endocrinological causes of arterial hypertension. Because of the increased risk of cardiovascular diseases in these patients, the early diagnosis and proper treatment of primary hyperaldosteronism play a crucial role. The aim of the study was presentation of a patient with secondary arterial hypertension. During the diagnosis of the hypertension cause, a renal tumor was found and then primary hyperaldosteronism …

We report our experience with the initial six cases of transperitoneal laparoscopic adrenalectomy for primary hyperaldosteronism (Conn's syndrome). This is the first report from India of such a series. From 1997 to 2002, 6 patients with primary hyperaldosteronism (Conn's syndrome) were referred to the Department of Endocrine Surgery and underwent laparoscopic adrenalectomy through the transperitoneal approach. In all patients, the adrenal gland and the adenoma were successfully removed laparoscopically, and all patients were normokalemic and normotensive in the first follow-up after 6 months. …

INTRODUCTION: Primary aldosteronism is the most common form of mineralocorticoid hypertension. The disease has been described by Jerome W. C … ne-producing adenoma was detected in more than two thirds of patients (n = 135), whereas idiopathic hyperaldosteronism was found in 46 patients. Other subtypes of primary hyperaldosteronism occurred less frequently (unilateral primary adrenocortical hyperplasia in 5 patients and adrenocortical carcinoma in one patient). For the diagnosis of familial hyperaldosteroni …

AIMS AND OBJECTIVES: Adrenal adenomas are usually non-functioning, but can secrete aldosterone or cortisol. It has recently been suggested that many more adenomas than previously thought secrete more than one hormone. This has important implications for their clinical management. Our aim was to determine the frequency of cortisol co-secretion in primary hyperaldosteronism at our institution and investigate the difference in metabolic profiles and clinical outcomes between co-secreting and non-co-secreting patients.
DESIGN AND PATIENTS: A retrospective study of 25 patients with primary hyperal…

Two cases of phaeochromocytoma with hypokalaemia are reported. In the first, a carcinoma, hypokalaemia was potentially secondary to hyperreninaemia and hyperaldosteronism in association with probably paraneoplastic hypercorticism. The other was benign and the origin of isotopically demonstrated potassium deficiency was less clear, because hyperreninaemia, hyperaldosteronism and hypercortisolism were moderate.

The case of a woman with autonomous aldosteronism, hypertension, and malignant ovarian tumour is reported. Hormone studies revealed high plasma aldosterone levels, and low plasma renin concentration. Following surgical removal of a malignant sex cord stromal tumour, the hyperaldosteronism regressed, and the hypertension improved. Subsequently the tumour recurred, as did hyperaldosteronism.

A young Polynesian man presented with severe hypertension complicated by an intracerebral hemorrhage. The hypertension was found to be secondary to hyperaldosteronism from a well differentiated adrenocortical carcinoma. Surgical removal of the tumor has resulted in a near normal blood pressure with no evidence of tumour recurrence of hyperaldosteronism after one year.

ACTH alpha 1-24 was infused at incremental rates of 12.5-200 mIU/30 min in dexamethasone-suppressed hypertensive patients on a regular sodium diet. The plasma aldosterone response to this stimulus in 8 patients with hyperaldosteronism due to an adrenal aldenoma and 11 with adrenal hyperplasia was significantly greater at all infusion rates (P less than 0.05) when compared with the response in 6 normal subjects on a similar diet. This responsiveness to ACTH in the patients with primary hyperaldosteronism was similar to that of the normal subjects on a low sodium diet. Twelve patients with low r…

Both severe hypokalemia and persistent hypertension are clinical symptoms of hyperaldosteronism. Hyperaldosteronism may occur as a primary or secondary syndrome. Excess ACTH produced ectopically by tumors may induce hyperaldosteronism through the mineralocorticoid activity of glucocorticoids that are upregulated by ACTH. Licorice, with the active ingredient glycyrrhiza, is also a well-known inducer of hyperaldosteronism under specific conditions. In this report, we describe a case of severe hypokalemia caused by ectopic ACTH syndrome (EAS) elicited by an intrathoracic carcinoid tumor, which ha…

Resistant hypertension is an increasingly common problem faced by primary care physicians and specialists and will undoubtedly become even more common as the adult population ages and gains weight. In the Antihypertensive and Lipid-Lowering Treatment to Prevent Heart Attack Trial (ALLHAT), at least 8% of subjects were resistant to treatment based on the need for three or more antihypertensive agents. Characteristics of patients with resistant hypertension include being older, black, obese, and diabetic, and having chronic kidney disease as well as untreated sleep apnea. Hyperaldosteronism is c…

Clinical and humoral signs of postoperative stress ore partly related to temporary hyperaldosteronism. The action of an aldosterone-antagonist was therefore studied on 24 abdominal surgical patients. The aldosterone-inhibitor has shown a consistent pharmacological and clinical effect. The drug seems then to be useful and safe-with close clinical and humoral follow-ups for the treatment of the postoperative secondary hyperaldosteronism.

A 6-week-old male with severe hypertension is presented. He was found to have hyperaldosteronism and a negative long polymerase chain reaction (PCR) test for glucocorticoid-remediable aldosteronism. His mother and maternal grandfather also had severe hypertension. A case is made for the possible diagnosis of familial hyperaldosteronism type II at a very early age.

The evaluation of computed tomography (CT) for detecting aldosterone-producing adenoma in primary aldosteronism was performed by comparison with adrenal scintiscan; determination of aldosterone in adrenal or renal veins, retroperitoneal pneumography and adrenal venography was reliable for diagnosis of adrenal tumors in pheochromocytoma or Cushing's syndrome, but not so effective for small adenoma of primary aldosteronism. An abdominal CT scan was performed on six patients with primary aldosteronism, one with idiopathic hyperaldosteronism and one with glucocorticoid responsive hyperaldosteronis…

Conn's syndrome, defined as unilateral aldosterone-producing adenoma, accounts for 35-40% of cases of primary hyperaldosteronism. Primary hyperaldosteronism typically occurs in younger patients with poorly controlled arterial hypertension due to extracellular fluid retention, in whom at least a triple combination of antihypertensives, including a diuretic, is needed to maintain normotension. The clinical picture of arterial hypertension may be complemented by symptoms associated with hypokalaemia, such as weakness, fatigue, palpitations, convulsions, polydipsia, or polyuria. In addition to art…

A 49-year-old female patient presented with decline of general health and muscle pain. Laboratory findings showed a combination of rhabdomyolysis and severe hypokalemia. Symptoms improved with potassium supplementation. Hyperaldosteronism was identified as the underlying cause of the hypokalemia. Sonography revealed an adrenal mass that was diagnosed as adrenal carcinoma following surgical removal. Hypokalemia is a rare cause of rhabdomyolysis and should prompt consideration of hyperaldosteronism.

We describe a 55 year-old woman with longstanding hypertension who developed hypokalaemia following diuretic treatment. Investigations revealed primary hyperaldosteronism due to an adrenal adenoma, and normal blood pressure was restored after surgical removal of the tumour. Primary hyperaldosteronism is a potentially curable cause of hypertension and should be considered in hypertensive patients who present with diuretic - induced hypokalaemia.

67 renal biopsies obtained in the course of adrenalectomy from patients with primary hyperaldosteronism, hypercorticism and pheochromocytoma were studied. Sclerotic affection of the arterioles and renal interstitium were found in long lasting adrenal arterial hypertension. This may cause residual postoperative hypertension. Endocrine nephropathy in primary hyperaldosteronism due to hypopotassemia and alcalosis manifests with vacuolar distrophy and atrophy of the epithelium, dilatation of tubular lumen, intratubular calcinosis and tubulointerstitial nephritis. Primary hyperaldosteronism was cha…

This study explores the potential relationship between 8 immunohistochemistry markers and 4 adrenal diseases-cushing syndrome, primary hyperaldosteronism, nonfunctioning adrenal adenoma, and hypercatecholamine-based on symptoms and laboratory findings. Understanding these associations can provide valuable insights into the diagnosis and management of adrenal tumors. We retrospectively analyzed cases of patients treated in the urology department of our hospital from April 2017 to May 2022, focusing on pathologic results of specimens that had undergone immunohistochemical staining following lapa…

The insulin sensitivity of five essential hypertensive patients was compared to five patients with renovascular hypertension, five patients with primary hyperaldosteronism, and five normotensive subjects, using the euglycemic hyperinsulinemic clamp technique. Essential hypertensive patients had significantly lower insulin sensitivity than patients with hyperaldosteronism and renovascular hypertensive patients (P = .0066, P = .004, respectively). Hyperaldosteronism patients also had less insulin sensitivity than renovascular hypertensive patients (P = .016). A significant negative correlation w…
