The changing epidemiology of infections at cancer hospitals.

Intensive chemotherapy regimens, widespread prophylactic and therapeutic administration of antibiotics, reliance on intravascular catheters, increased use of immunomodulators, and increased outpatient management have altered the epidemiology of infections in patients with neoplastic disease. At many centers, bacteremias due to gram-positive organisms have replaced bacteremias due to gram-negative organisms as the most frequent infections in patients with cancer. Isolation of gram-negative bacilli other than Enterobacteriaceae and Pseudomonas aeruginosa has increased in frequency, and these org

Bone and the immune system.

There are several lines of evidence which provide support for an important relationship between immune cells and bone. Clinical studies of immunodeficiency syndromes have shown that abnormalities in bone shape are evident on x-rays, and peculiarities in the structure of the growth plate have been identified by histopathology. Studies of bone histology, and quantitation of cellular abnormalities, are scarce. Abnormalities in bone turnover, have, however, been identified in the nude mouse model. Many lines of evidence derived from in vitro bone studies have shown that lymphokines and monokines c

Fatal infectious mononucleosis with staphylococcal pyoderma in a girl with hereditary immunological dysregulations.

We describe a 14-year-old girl with fatal infectious mononucleosis with high fever, pancytopenia, and multiple skin ulcers due to Staphylococcus aureus. Immunological studies revealed low serum IgM, low natural killer (NK) activity, and high CD4/CD8 ratio. Her father had also low NK activity and high CD4/CD8 ratio. It is suggested that she had a dominantly inherited immunodeficiency predisposing to severe Epstein-Barr virus infection.

[Deficiencies in humoral immunity].

B cell immune deficiencies are characterized by inadequate production of antibodies and/or low levels of one or more classes of immune globulins (IgG, IgA, IgM) or IgG sub-classes. They include: 1) severe deficiencies involving all the immune globulins (Bruton agammaglobulinemia, variable hypogammaglobulinemia); 2) selective deficiencies in immune globulins (IgA deficiency, IgM deficiency, deficiencies in IgG sub-classes, dysgammaglobulinemias); 3) transient infantile hypogammaglobulinemia; 4) B cell immune deficiencies with normal gammaglobulin levels. Symptoms of B cell immune deficiencies a

Modified immune globulin: its use in the prophylactic treatment of patients with immune deficiency.

Patients with antibody deficiency syndromes provide excellent models to study safety and efficacy of new immune globulin (IG) preparations for intravenous use. In a long term study of such patients we showed that a reduced and alkylated IG preparation given intravenously was as effective in preventing infections as Cohn fraction II given intramuscularly. Mild side effects occurred in 50% of infusions; addition of maltose reduced side effects to 3%. Adverse reactions appeared to be related to IG concentration: they were slightly higher if the IG concentration was increased from 5% to 10%. The s

[Results of bronchial scrapings in interstitial pulmonary diseases in immunodeficient children (author's transl)].

Endobronchial scraping was used in 53 immunodeficient children, aged 4 months to 15 years, and divided into three categories (37 receiving immunosuppression treatment, 8 with marasmus, and 8 with immunodeficiency), in order to determine the etiology of their interstitial pneumopathy. The examination was made under blind conditions in 21 cases using an intubation tube (under assisted ventilation), and with bronchoscopy under general anesthesia in the other 32 cases. Three scrapings were required for cytological, bacteriological, and virological and mycological examinations. In 32 cases (60%), t

Isolation of an adenovirus from an Arab foal with a combined immunode ficiency disease.

Some details of the clinical and postmortem findings of an Arab foal that died as a consequence of adenoviral pneumonia superimposed on a combined immunodeficiency disease are provided. The foal was the 17th in a series of similar deaths that occurred on a farm since 1959. An adenovirus, which by haemagglutination inhibition and serum neutralisation tests was antigenically similar to 2 other equine adenoviruses isolated in Australia, was isolated from a nasal swab taken from the foal when it was 23 days of age.

A TREATMENT FOR SEBORRHEA CAPITIS (CRADLE CAP).

Forty-eight infants with seborrhea capitis were treated with a preparation containing a polypeptide-fatty acid complex and tyrothricin in a propylene glycol vehicle, to determine if this material was non-toxic and if it was effective in the treatment of infantile seborrheic dermatitis. In this uncontrolled study, the preparation was effective in 45 of 48 infants so treated; a possible mild sensitivity reaction was observed in one infant.

Co-occurrence of Griscelli Syndrome Type 2 and Systemic Juvenile Idiopathic Arthritis in a Pediatric Patient: A Case Report.

Griscelli syndrome type 2 (GS2) is a rare autosomal recessive immunodeficiency disorder characterized by silver-colored hair, fair skin, and an increased risk of developing hemophagocytic lymphohistiocytosis (HLH). Systemic juvenile idiopathic arthritis (sJIA) is an autoinflammatory condition distinguished by daily fevers, transient rash, and arthritis, with macrophage activation syndrome being a known complication. We present a case involving an 18-month-old female with persistent fever, rash, and joint inflammation. Laboratory findings revealed elevated inflammatory markers and hyperferritin

Neurological findings may be initial presenting symptom in children with inborn errors of immunity.

INTRODUCTION: Neurological manifestations in patients with inborn errors of immunity (IEI) may occur during the disease course or, in some cases, represent the initial presenting feature. Increased awareness of these manifestations may facilitate earlier diagnosis and more effective management of IEIs, thereby preventing irreversible neurological morbidity. This study aimed to investigate the relationship between IEI subtypes and neurological involvement and to highlight the role of neurological symptoms in early diagnosis. METHODS: This study included patients younger than 18 years who were

Fatal Systemic Granulomatous Disease Associated with Vaccine-Derived Rubella Virus in AIOLOS-Associated Immunodeficiency.

We present a case of disseminated granulomatous disease associated with live attenuated vaccine-derived rubella virus (VDRV) in an immunodeficient patient who subsequently developed fatal multi-organ failure, including neurological deficits, with VDRV detected in the cerebrospinal fluid. Posthumous genetic analysis found a heterozygous missense mutation in IKZF3, a gene where haploinsufficiency is linked to immunodeficiency and immune dysregulation. Functional studies demonstrated that this mutation decreases AIOLOS protein stability and half-life, and family members carrying the same mutation

Imaging of Immunodeficiency-Associated Lymphoproliferative Disorders.

Immunodeficiency-associated lymphoproliferative disorders represent a heterogeneous group of conditions characterized by abnormal lymphoid tissue proliferation in the context of immune dysregulation. These disorders can range from benign reactive hyperplasia to overt malignant lymphoma and are strongly associated with conditions such as congenital and acquired immunodeficiencies, chronic immunosuppressive therapy, and viral infections, particularly Epstein-Barr virus. This article provides a comprehensive overview of the most common immunodeficiency-associated lymphoproliferative disorders sub

Systemic auto-inflammatory diseases, auto-immune diseases and immune deficiencies: From independent to overlapped diseases approach.

Systemic AutoInflammatory Diseases (SAIDs), AutoImmune Diseases (AIDs), and Primary Immune Deficiencies (PIDs) were formely considered independent, even opposing, conditions. SAIDs were associated with the innate immune system, involving dysregulation of inflammation and recurrent episodes of systemic inflammation without an infectious process. Their original definition did not involve autoreactive cells or autoantibodies. In contrast, PIDs were characterised by defects in the adaptive or innate immune system, leading to recurrent infections and immune dysfunction. AIDs were inflammatory disea

Management of the child with primary immunodeficiency.

PURPOSE OF REVIEW: The purpose of this review is to provide an update on monitoring, prevention, and treatment of infectious and noninfectious complications of phagocytic, humoral, and cellular immune deficiencies. RECENT FINDINGS: The recognition that immune dysregulatory features are hallmarks of many genetic defects leading to immune deficiency have led to a new name for all congenital disorders that primarily impact immune function - inborn errors of immunity (IEI). While controlling infections is critical for treatment of patients with IEI, recognition and management of noninfectious com
