Historical versus contemporary medicinal plant uses in the US Virgin Islands.

ETHNOPHARMACOLOGICAL RELEVANCE: Hidden in the documents of the dark past of the trans-Atlantic slavery are gems of ethnomedicinal observations, supported by herbarium specimens, which tell of the traditional medicine of a by-gone slave society in the Caribbean. In the context of the former Danish West Indies (now US Virgin Islands), we identify pre-1900 medicinal plants and their historical uses, and trace their status in the traditional medicine of St. Croix today (2014). By a combined historical and ethnobotanical approach we assess the scale of loss and preservation of traditional medicinal

Do guidelines influence the implementation of health programs?--Uganda's experience.

BACKGROUND: A guideline contains processes and procedures intended to guide health service delivery. However, the presence of guidelines may not guarantee their implementation, which may be a result of weaknesses in the development process. This study was undertaken to describe the processes of developing health planning, services management, and clinical guidelines within the health sector in Uganda, with the goal of understanding how these processes facilitate or abate the utility of guidelines. METHODS: Qualitative and quantitative research methods were used to collect and analyze data. Da

Violations of human rights: health practitioners as witnesses.

For humanitarian health-care practitioners bearing witness to violations of human dignity has become synonymous with denunciations, human rights advocacy, or lobbying for political change. A strict reliance on legal interpretations of humanitarianism and human rights is inadequate for fully understanding the problems inherent in political change. With examples from the HIV/AIDS epidemic in the USA, the Rwandan genocide, and physician-led political activism in Nepal, we describe three cases in which health practitioners bearing witness to humanitarian and human-rights issues have had imperfect

Improving the current system for supplying organs for transplantation.

The United States currently relies on a voluntary, altruistic system for supplying organs for transplantation. It is now generally recognized that this system, as currently operated, produces a seriously inadequate supply of organs. A number of scholars have argued that some type of (generally unspecified) market system is necessary. Two articles appearing in the Journal of Health Politics, Policy and Law have proposed relatively specific market systems for increasing the supply of organs. In this paper we argue that market systems are at best premature. In particular, there is little to sugge

The impact of immunohistochemical detection of positive lymph nodes in early stage lung cancer.

BACKGROUND: Detection of micrometastatic disease is an interesting area in non-small cell lung cancer (NSCLC). We conducted a study to determine whether the detection of mediastinal lymph node spread by immunohistochemical (IHC) analysis offers some prognosis with respect to patients' disease-free survival or not. METHODS: Between 1997 and 2003, twenty-one early stage lung cancer patients underwent complete resection with mediastinoscopy and systemic nodal dissection. Four hundred and twenty-six paraffin-embedded lymph node sections from 21 patients were analyzed. Epithelial specific-antigen

[Disseminated intravascular coagulation in chickenpox. Report of a case in a child].

The case of a healthy and immunocompetent five-year-old boy, who developed a disseminated intravascular coagulation during chickenpox is described. Disseminated intravascular coagulation manifestations were extremely severe and included macroscopic hematuria, necrotic purpura and cerebrovascular thrombosis. The outcome in this patient was a complete recovery. Nevertheless, the possibility of a seriously complicated course of chickenpox even in low-risk children subgroups suggests that the Varicella-Zoster virus infection should not be underestimated. More accurate information about the impact

Foodborne disease surveillance in England and Wales: 1989-1991.

This review summarises reports of food poisoning, salmonellosis, campylobacteriosis and other acute foodborne illness to the PHLS Communicable Disease Surveillance Centre, and notifications of food poisoning collated by the Office of Population Censuses and Surveys, in the period 1989-1991. During this period there were continuing rises in notifications of food poisoning and reports of salmonellosis and campylobacteriosis. There was considerable success in the control of foodborne listeriosis. Newly emerging pathogens, such as Vero cytotoxin producing Escherichia coli, became more important. T

Fertility care in the age of algorithms: opportunities and risks of AI and social media integration in reproductive medicine.

Media representations have long influenced public understandings of infertility and assisted reproductive technologies (ART), with tradition … digital media analyses to examine infertility narratives, online patient engagement, and AI-driven information dissemination. Findings indicate that social media has expanded patient agency and reduced isolation, but AI-generated content and influencer marketing contribute to the rapid spread of inaccurate or misleading fertility information. Despite these risks, digital …

Identifying and overcoming barriers to providing sexuality information in the clinical setting.

While the scope of the clinician-patient relationship in rehabilitation includes the opportunity to provide information on sexuality, all too often this does not occur. This article identifies barriers to providing information on sexuality and makes suggestions for overcoming them. At a preconference workshop, "Sexuality After Spinal Cord Injury: Understanding the Effects of Knowledge and Attitude on Dissemination Practices," held during the Association of Rehabilitation Nurses 22nd Annual Educational Conference, participants explored barriers to providing sexuality information to their patien

Multicountry research on comprehensive abortion policy implementation in Latin America: a mixed-methods study protocol.

INTRODUCTION: Access to comprehensive abortion care could prevent the death of between 13 865 and 38 940 women and the associated morbidity of 5 million women worldwide. There have been some important improvements in Latin America in terms of laws and policies on abortion. However, the predominant environment is still restrictive, and many women, adolescents and girls still face multiple barriers to exercise their reproductive rights. This research will systematically assess comprehensive abortion policies in five Latin American countries (Argentina, Colombia, Honduras, Mexico and Uruguay). Th

Sleep and circadian biomarkers of postoperative delirium (SLEEP-POD): protocol for a prospective and observational cohort study.

INTRODUCTION: Surgical patients over 70 experience postoperative delirium (POD) complications in up to 50% of procedures. Sleep/circadian disruption has emerged as a potential risk factor for POD in epidemiological studies. This protocol presents a single-site, prospective observational study designed to examine the relationship between sleep/circadian regulation and POD and how this association could be moderated or mediated by Alzheimer's disease (AD) pathology and genetic risk for AD. METHODS AND ANALYSIS: Study staff members will screen for eligible patients (age ≥70) seeking joint replac

Clinical and pathologic characteristics of disseminated cutaneous coccidioidomycosis.

BACKGROUND: Cutaneous lesions are a common manifestation of disseminated coccidioidomycosis, which has increasing prevalence in endemic regions. The clinical and histologic features may be more variable than is currently recognized. OBJECTIVE: To characterize the clinical and histologic findings from a large series of disseminated coccidioidomycosis cases in the southwestern United States. METHODS: Histologic materials and clinical records for all biopsy-positive cases (1975-2006) from three medical centers in Arizona were reviewed. RESULTS: Patterns of inflammation were generally granuloma

Disseminated histoplasmosis in renal allograft recipients.

Histoplasmosis, an opportunistic fungal infection endemic in the Ohio and Mississippi river valleys, is caused by a dimorphic fungus Histoplasma capsulatum. Most infections are asymptomatic or self-limited febrile illness. Immunosuppressed renal transplant recipients are susceptible to a disseminated disease. We report 5 cases of disseminated histoplasmosis seen in our institute over a period of 25 years amongst 1074 renal transplant recipients. The duration of immunosuppression prior to the diagnosis of infection ranged from 84 days to 14 years. All patients had pulmonary involvement. Three p

Infections due to Cunninghamella bertholletiae in patients with cancer: report of three cases and review.

Limited information is available regarding the pathogenesis and clinical manifestations of infection with Cunninghamella bertholletiae. In this report, we describe the clinical courses of three patients with leukemia and fatal C. bertholletiae infection and review the literature. In all patients, the infection developed in the setting of severe neutropenia following high doses of cytotoxic chemotherapy. Clinical presentation always consisted of fever and pneumonia and could be associated with dissemination to numerous organs. The course of infection was very rapid, and the diagnosis was made a

How communication of genetic information within the family is addressed in genetic counselling: a systematic review of research evidence.

Supporting consultands to communicate risk information with their relatives is key to obtaining the full benefits of genetic health care. To understand how health-care professionals address this issue in clinical practice and what interventions are used specifically to assist consultands in their communication of genetic information to appropriate relatives, we conducted a systematic review. Four electronic databases and four subject-specific journals were searched for papers published, in English, between January 1997 and May 2014. Of 2926 papers identified initially, 14 papers met the inclus

Exploring the use and usefulness of living guidelines for consumers: international online survey of patients' and carers' views.

BACKGROUND AND OBJECTIVE: Living guidelines contain continually updated, and potentially changing, clinical recommendations. The implications of living guidelines for consumers (eg, patients, carers, and people with lived experience) - particularly how living guidelines should be developed and disseminated - are yet to be established. The objective of this study was to explore consumers' views about how best to support the use and usefulness of living guidelines to consumers. METHODS: This study used a qualitative (online survey) design. We invited consumers who were familiar with guidelines

DENV-1 genotype V linked to the 2022 dengue epidemic in Southern Brazil.

Even though Brazil is a country where the dengue virus (DENV) is endemic, until recently, Southern states did not have significant viral circulation, such as Rio Grande do Sul (RS), and some municipalities were even considered dengue-free. During 2022, these places have shown a sharp increase in the incidence of the disease, apparently following a worldwide growth pattern. Therefore, in this study, we monitor and characterize the genetic diversity of DENV circulating in southern Brazil through next-generation sequencing during an outbreak in 2022. We generated 70 DENV-1 genome sequences, all c

International high-risk clone of fluoroquinolone-resistant Escherichia coli O15:H1-D-ST393 in remote communities of Brazilian Amazon.

The global dissemination of multidrug-resistant Escherichia coli lineages belonging to high- risk clones poses a significant public health threat. Herein we report the identification and genomic profiling of two multidrug-resistant E. coli strains [BL-II-03(2) and BL-II-11(3)] belonging to the O15:H1-D-ST393 (clonal complex 31) worldwide spread clone, isolated from fecal samples of indigenous peoples belonging to two different ethnic groups of remote communities of Brazilian Amazon. Genomic analysis revealed genes and mutations conferring resistance to β-lactams [blaTEM-1], aminoglycosides [aa

From engagement to dissemination: a WiseAge patient and public involvement and engagement event on dementia with Lewy bodies.

BACKGROUND: Dementia with Lewy bodies may account for up to 20% of dementia cases, yet public awareness remains limited. Public engagement may help communicate dementia research to wider audiences, but little is known about whether information spreads beyond event attendees. This study explored how attendees evaluated information about dementia with Lewy bodies and the ongoing Ambroxol in New and Early Dementia with Lewy Bodies (ANeED) clinical drug trial presented at a WiseAge public engagement event, and how they reported sharing this information afterwards. METHODS: This descriptive cross-
