Five patients were admitted to the 1st and 2nd Division of General Surgery of Soronno Hospital between July '75 and September '78 suffering from fistulas complicating colon diverticulosis. A sixth patient, operated on for diverticulitis, developed a stercoraceous cutaneous fistula in the postoperative period. This was closed by conservative therapy. An 18-year review of 64 patients treated with 71 postoperative enterocutaneous fistulas of the stomach /4/, duodenum /21/, jejunum /9/ and ileum /37/ was carried out to identify the factors affecting morbidity and mortality. Chronic perianal fistulas are a common clinical condition. However, their evolution into adenocarcinoma is rare. We report the case of a 68-year-old man with perineal and perianal chronic fistulas, who developed a perineal mass that extended proximally as a pararectal tumor. At our hospital in Hamburg 106 patients underwent laparoscopic hysterectomies. 92 patients had no complications, but 6 had major and 10 minor ones. As major complications occurred: one vesico-vaginal fistula, one uretero-vaginal fistula, one ureter injury, one lesion of the small intestine and two bladder injuries. As minor complications could be found: six urinary tract infections, one anemia (Hb 7,1 g/dl) and one febrile morbidity. The overall complication rate is 13.2%.

As the etiology of Crohn's disease is still unknown conservative as well as surgical treatment has to be symptomatic. Medical therapy is performed by use of prednisolone, salazosulfapyridine, metronidazole and elementary diet depending on the localisation and extension of the disease. Surgical treatment means therapy of complications like stenoses, fistulae and abscess formation. Neither after conservative nor after surgical treatment is the course of the disease predictable.

Richter's hernia is due to the entrapment of a part of circumference of the bowel wall. As the bowel continuity is maintained, the patients usually do not have intestinal obstruction.
