Peritonitis and paralytic ileus due to COVID-19 in a 29 weeks pregnant woman: a case report.

BACKGROUND: COVID-19 infection has caused a global health crisis with over 6,956,900 deaths. Extra-pulmonary forms of the infection are common, including digestive symptoms. Pregnant women are at greater risk of developing severe forms of the infection. They are also affected by extra-pulmonary symptoms. CASE PRESENTATION: We report the case of a 28week + 4 pregnant woman with paralytic ileus caused by inflammatory peritonitis related to COVID-19 leading to premature birth. Pulmonary deterioration was secondary to abdominal symptoms. CONCLUSIONS: Therefore, the presence of gastrointestinal (GI

Impact of gastrointestinal involvement on mortality and malnutrition in systemic sclerosis: an observational cohort of 135 patients.

To describe the characteristics and outcomes of gastrointestinal involvement (GI) in patients with systemic sclerosis (SSc). We also assessed the impact of GI on mortality, hospitalizations, and undernutrition and identified the subgroup affected by chronic intestinal pseudo-obstruction (CIPO). We conducted an observational monocentric study including 135 consecutive patients with SSc and evaluated clinical and paraclinical data, laboratory data and nutritional data at disease onset and at the last follow-up over a median follow-up of 89 months. We assessed hospitalizations and their main caus…

The IgSF Cell Adhesion Protein CLMP and Congenital Short Bowel Syndrome (CSBS).

The immunoglobulin-like cell adhesion molecule CLMP is a member of the CAR family of cell adhesion proteins and is implicated in human congenital short-bowel syndrome (CSBS). CSBS is a rare but very severe disease for which no cure is currently available. In this review, we compare data from human CSBS patients and a mouse knockout model. These data indicate that CSBS is characterized by a defect in intestinal elongation during embryonic development and impaired peristalsis. The latter is driven by uncoordinated calcium signaling via gap junctions, which is linked to a reduction in connexin43

A Novel Mutation in an MNGIE Patient Presenting with More Prominent Neurological Symptoms than GI Symptoms.

Mitochondrial neurogastrointestinal encephalopathy (MNGIE) is an autosomal recessive disease associated with the mutation of the TYMP gene. MNGIE causes gastrointestinal and neurological symptoms, and the gastrointestinal symptoms are usually notable, which may lead to misdiagnosis. However, we herein report a 29-year-old female who presented with prominent neurological symptoms, while her gastrointestinal symptoms were mild. Brain MRI revealed prominent diffused leukoencephalopathy and peripheral neuropathy was confirmed by the nerve conduction velocity test. Biochemical tests showed elevated

Compound heterozygous loss of function variants in MYL9 in a child with megacystis-microcolon-intestinal hypoperistalsis syndrome.

Megacystis-microcolon-intestinal hypoperistalsis syndrome (MMIHS), or "visceral myopathy," is a severe early onset disorder characterized by … s of MMIHS was made based upon the presence of megacystis, lack of urinary bladder peristalsis, and intestinal pseudo-obstruction. After initial testing of ACTG2 was negative, further sequencing and deletion/duplication testing was performed on the LMOD1, MYH11,MYLK, and MYL9 genes. We identified two heterozygous loss of function variants in MYL9: an exon 4 deletion and a nine …

Placental Fetal Thrombotic Vasculopathy Occurring in Association with Megacystis-microcolon-intestinal Hypoperistalsis Syndrome: A Case Report.

Megacystis-microcolon-intestinal hypoperistalsis syndrome (MMIHS) is a rare disease of functional obstruction affecting the bladder and intestines, characterized by a markedly distended bladder, microcolon, and decreased or absent intestinal peristalsis. Afflicted neonates have very poor prognosis, usually with fatal outcomes in first days to months of life. Placental fetal thrombotic vasculopathy (FTV) is a thrombo-occlusive disorder of the chorionic plate and fetal circulation. Herein, we describe an undocumented association of MMIHS and placental FTV. We present the case of 32-year-old fema

Use of neostigmine in capecitabine-induced paralytic ileus.

Paralytic ileus is a recognised side effect of the oral agent capecitabine. We present this report on a patient with metastatic colorectal cancer who was treated with capecitabine and presented with persistent paralytic ileus which did not respond to standard conservative measures. Neostigmine was administered safely, resulting in resolution of the paralytic ileus. This approach merits further investigation.

[Megacystis-microcolon intestinal hypoperistalsis syndrome (MMIHS) as a rare differential diagnosis of foetal megacystis on ultrasonography].

INTRODUCTION: Foetal megacystis (incidence 1:1 500) occurring in the first trimester may already be a sign of congenital malformation. Often, urethral valves are causally responsible in male foetuses far more frequently than urethral atresia. As a further differential diagnosis, the "prune-belly syndrome" needs to be distinguished. Far more difficult to classify prenatally is the rare MMIHS which, in contrast to the diagnosis of urethral valves, is associated with an unfavourable prognosis. CASE REPORT: This is a report on a 28-year-old IG/0P, whose foetus of 26+4 gestational weeks was found

Variants of Hirschsprung disease.

Variants of Hirschsprung disease are conditions that clinically resemble Hirschsprung disease, despite the presence of ganglion cells in rectal suction biopsies. The characterization and differentiation of various entities are mainly based on histologic, immunohistochemical, and electron microscopy findings of biopsies from patients with functional intestinal obstruction. Intestinal neuronal dysplasia is histologically characterized by hyperganglionosis, giant ganglia, and ectopic ganglion cells. In most intestinal neuronal dysplasia cases, conservative treatments such as laxatives and enema a

Accuracy of increased large-intestine wall thickness during ultrasonography for diagnosing large-colon torsion in 42 horses.

Large-colon torsion is a common cause of colic in horses and has a worse prognosis and higher cost than other causes of surgical colic of the large colon. During large-colon torsion, the colon wall becomes thick due to vascular occlusion. Therefore, we hypothesized that detecting increased colon wall thickness during ultrasonography would be an accurate preoperative test for large-colon torsion. The sample population consisted of 42 horses that were admitted for surgical treatment of colic localized to the large colon. The diagnosis was confirmed at surgery or necropsy examination. Twelve (29%

Response to octreotide of intestinal pseudoobstruction and pneumatosis cystoides intestinalis associated with progressive systemic sclerosis.

Intestinal pseudoobstruction and pneumatosis cystoides intestinalis are uncommon complications of progressive systemic sclerosis. We report a 26-year-old woman with this disorder who responded poorly to conventional treatment. Subcutaneous administration of octreotide, a long-acting somatostatin analogue, at a dose of 50 micrograms/day for 3 weeks, relieved symptoms such as nausea and bloating. There was also a marked decrease of intestinal gas accumulation, as documented on X-ray films.

Enteric cholinergic neuropathy in patients with diabetes: Non-invasive assessment with positron emission tomography.

BACKGROUND: 11 C-Donepezil positron emission tomography (PET) allows non-invasive assessment of cholinergic innervation of visceral organs. We aimed to compare cholinergic innervation in the gut in patients with diabetes mellitus (DM) and in healthy controls (HC). METHODS: 11 C-Donepezil PET and computed tomography (CT) were performed in 19 patients with type 1 DM and gastrointestinal symptoms and in 19 age- and sex-matched HC in a cross-sectional design. KEY RESULTS: All patients had severe gastrointestinal symptoms when assessed by standard questionnaires. DM patients had significantly inc

Variant Hirschsprung's disease.

There are many clinical conditions that resemble Hirschsprung's disease despite the presence of ganglia cells on rectal biopsy. This group has focused its research interest into delineating variant Hirschsprung's disease based on specific histochemical, immunohistochemical, silver staining and electron microscopic studies. Between 1981 and 1996, full thickness rectal biopsy or resected surgical specimens from 66 patients with clinical symptoms suggesting Hirschsprung's disease were examined. Various functional bowel disorders diagnosed using different histological techniques included, intestin

Malnutrition and Sarcopenia in Gastrointestinal Motility Disorders: Mechanisms, Assessment, and Management.

PURPOSE OF REVIEW: Malnutrition and sarcopenia are increasingly recognized as clinically meaningful but underappreciated components of gastrointestinal (GI) motility disorders. This review examines the prevalence, pathophysiology, assessment, and clinical care of malnutrition in the context of patients with GI motility disease. RECENT FINDINGS: Emerging data demonstrate a high burden of malnutrition in GI motility conditions, including gastroparesis and chronic intestinal pseudo-obstruction. Mechanisms include reduced oral intake, restrictive eating patterns, malabsorption, inflammation, and …
