INTRODUCTION: Medial patellofemoral ligament (MPFL) prevents lateral dislocation of the patella and acts as a major stabilizer. The tear could be associated with osteochondral avulsion fractures; however, intra-articular split fractures are not seen. Here, we present one unusual case of such a presentation.
CASE REPORT: A 16-year-old male presented with a history of recurrent patellar dislocation and inability to bear weight following the last episode. The patient was diagnosed with an MPFL tear with an unusual intra-articular femoral condyle split fracture. The case had significant improveme

Pelvic support osteotomy and femoral lengthening are sometimes employed in cases of hip instability. We assessed the efficacy of surgery following sepsis and congenital dislocation in relation to functional results. Between March 2005 and May 2007 we performed pelvic support osteotomy and femoral lengthening for 21 hips in 20 patients. Preceding pathology included congenital dislocation in 13 hips (12 patients) and sequelae of sepsis in 8 hips. All surgical procedures were performed as described by Ilizarov. The mean follow-up period was 33.5 months. There was no significant difference between

Nail-patella syndrome (NPS) or hereditary onycho-osteodyaplasia is a rare genetic condition involving a mutation in the LMX1B gene affecting nails, elbows, knees, and pelvis. Due to the regulatory functions of the gene in many developmental processes through the body, patients with NPS experience wide-ranging musculoskeletal problems including patellar instability, fingernail anomalies, iliac exostoses/horns, and elbow abnormalities. The patellar changes often involve aplasia, hypoplasia, and chronic dislocation. Due to these musculoskeletal involvement, arthritis of joints can occur in patien

Sixty knees presenting with chronic anterior pain were investigated. A diffusely increased uptake indicated sympathetically mediated pain provided other diagnoses such as synovitis or widespread arthrosis were excluded. Focal uptake was not specific to any one condition, but could occur in chondromalacia patellae or recent dislocations. However, the commonest appearance found in chondromalacia patellae was a normal scan. A characteristic scintigraphic appearance in symptomatic postpatellectomy knees, which has not been previously reported, was increased uptake in the femoral groove. In all the

Hinged implants for total knee arthroplasty (TKA) improve stability and joint function in a ligamentously compromised knee, but potentially at the cost of reduced implant longevity due to increased mechanical stress on the construct. This report details the case of an 83-year-old woman who underwent a revision total knee arthroplasty for a chronically dislocated hinged knee prosthesis in August 2024 at our institution. The patient presented with a several month history of pain and instability with reported self-reduction of the prosthesis to aid in ambulation. The revision surgery revealed tha

First patellar dislocation is a common injury of the knee, involving often adolescents and the active population. The consequences of the first episode can be various and potentially disabling. Among these, acute patellar dislocation can often result in recurrent patellar instability. Recurrent patellar instability is certainly multifactorial but depends primarily on the injury of the medial patellofemoral ligament (MPFL), the major soft-tissue stabilizer of the patella. Some classifications are extremely useful in establishing the diagnosis and therapy in patellofemoral disease, in particular

Objectives: To compare patient-reported outcome measures (PROMS) as the primary outcome following medial patellofemoral ligament reconstruction (MPFLR) using autograft versus allograft tendon. Secondary objectives were to assess re-dislocation rates and evaluate the effect of concomitant tibial tubercle osteotomy (TTO) on PROMs. Methods: Eighty-eight patients from two fellowship-trained orthopaedic surgeons operating at a single institution between 2018 and 2023 were identified. Eligible patients, including those who had undergone an MPFLR with either autograft or allograft +/- TTO, were conta

BACKGROUND: We aimed to prospectively investigate the clinical outcomes of medial patellofemoral ligament (MPFL) reconstruction using polyester high-strength suture tape with knotless anchors.
METHOD: Forty-three patients (65 knees) were followed for at least 1 year postoperatively, with 27 patients (43 knees) followed for 2 years postoperatively. All patients underwent MPFL reconstruction using polyester high-strength suture tape (FiberTape®; Arthrex) with knotless anchors (SwiveLock®; Arthrex). Repeated dislocation and residual patellar apprehension signs were recorded, and congruence and t

BACKGROUND: Rehabilitation protocols following medial patellofemoral ligament (MPFL) reconstruction were historically restrictive, with patients often immobilized and/or given weightbearing restrictions. However, more recently published protocols have been more aggressive. We compared patient-reported outcomes and recurrent dislocation risk between patients treated with a restrictive rehabilitation program (early post-operative bracing and weightbearing restrictions) and an accelerated rehabilitation protocol (no post-operative bracing or weightbearing restrictions) following MPFL reconstructi

OBJECTIVES: Assessment of cartilage lesions and osteoarthritis (OA) of the patellofemoral joint in patients following lateral patellar dislocation using magnetic resonance imaging (MRI).
METHODS: MR images of 129 knees (mean age 26 years, range 11-56) grouped as acute (A), recurrent (B), and chronic (C) dislocators were analysed regarding the prevalence and severity of patellofemoral cartilage lesions. Grades of OA were assessed using modified WORMS.
RESULTS: In groups A, B, and C the prevalence of cartilage lesions was 71%, 82%, and 97%, respectively. Most lesions were located on the centra

Clinical and arthroscopic findings after knee joint injury were evaluated in 35 children aged 4-15 years. Arthroscopy revealed a rupture of the anterior cruciate ligament in 9 patients; two lesions were only partial. Eight children suffered from patellar dislocation. Eleven patients showed minimal synovial and cartilaginous lesions as the only cause of hemarthrosis, hydrarthrosis and clinical symptoms. Three had chondral lesions due to a dysplastic patella. The clinical diagnosis was confirmed in only 12 of 35 cases. Arthroscopic therapy was performed in 15 and open surgical intervention in 16

Fractures of the distal femur typically occur in the axial and sagittal planes. A Hoffa fracture is a coronal plane fracture of the femoral condyle, which accounts for 8.7% to 13% of distal femoral fractures. It is usually associated with other injuries around the knee joint and hence is often missed. We conducted a comprehensive systematic review of papers published in the English language using PubMed, Web of Science, Scopus, and the Cochrane Database, which reported Hoffa's fracture associated with other injuries around the knee joint. We selected 11 eligible papers for final analysis and r

Nail patella syndrome is a rare dysplasia with characteristic findings of finger nail abnormalities, hypoplastic patellas, radial head dislocations, and iliac horns. We reviewed the problems and treatment of 44 patients with this syndrome from 13 Shriners Hospital units. The inheritance pattern is autosomal dominant, with foot abnormalities as the chief presenting complaint. All patients were ambulatory. Twenty of the patients underwent knee operations, and 24 underwent operations to correct foot and ankle deformities. Knee extensor realignments and foot posteromedial releases had overall good

Follow-up study of nine patients with patellar dislocation in combination with osteochondral fractures. Three of these patients with special problems in relation to knee pathology are discussed in detail. This group of knee injuries is often overlooked because of frequent spontaneous reposition. Etiology, diagnosis and treatment are discussed. Arthroscopy is recommended to visualize chondral defects. Results are presented in combination with follow-up. Four patients still complain of instability of the knee after surgery. Reconstruction is the treatment of choice. The kind of fixation depends

Acute patellar dislocation is a common knee injury that occurs most often in adolescents, frequently associated with sporting and physical activities. Patellar re-dislocation after the first episode appears to depend primarily on the medial patellofemoral ligament injury which represents the primary ligamentous restraint, providing about 50-60 % of the restraining force against lateral patellar displacement. Clinically, up to 94-100 % of patients suffer from medial patellofemoral ligament rupture after first-time patellar dislocation. Controversy regarding how patients with first patellar disl

CASE: This case report aims to highlight a rare injury of a combined right tibiofemoral and patellofemoral joint dislocation. The patient was found to have a partial quadriceps tendon rupture, lateral and medial-sided soft-tissue injuries, and avulsion of the anterior and posterior cruciate ligaments. Treatment involved lateral and medial-sided repairs, as well as repair of his partial quadriceps tendon tear, allowing eventual return to daily activities following extensive rehabilitation.
CONCLUSION: Successful surgical repair and rehabilitation of a combined tibiofemoral and patellofemoral d

BACKGROUND: The effect of osteoarthritis (OA) with tibiofemoral (TF) subluxation on patients undergoing total knee arthroplasty (TKA) has been less studied, and there have been no studies on sagittal knee subluxation which is a component of a three-dimensional problem. We aim to analyze the influence of preoperative coronal and sagittal knee subluxation with OA on other radiological parameters and the functional outcome in patients undergoing TKA.
METHODS: We retrospectively reviewed the 179-consecutive primary TKA in 151 patients from January 2017 to June 2017. The radiological parameters an
