Domain specific multimodal large language model for automated endoscopy reporting with multicenter prospective validation.

Accurate endoscopy reports are crucial for the diagnosis and management of patients with upper gastrointestinal (UGI) diseases, yet errors and omissions are common. The preparation of routine reports for common diseases is labor-intensive and time-consuming. To address this, here we developed Report-Angel, an integrated AI system based on a multi-modal large language model (MLLM) and conventional deep learning models and trained it on 20,617 image-text pairs to automatically generate detailed draft reports for UGI endoscopy. Report-Angel achieved a clinically acceptable report rate of 79.3% (9

Reasoning-Driven Design of Single-Atom Catalysts via a Multiagent Large Language Model Framework.

Large language models (LLMs) are becoming increasingly applied beyond natural language processing, demonstrating strong capabilities in complex scientific tasks that traditionally require human expertise. This progress has extended into materials discovery, where LLMs introduce a new paradigm by leveraging reasoning and in-context learning, capabilities absent from conventional machine learning approaches. Here, we present a Multi-Agent-based Electrocatalyst Search Through Reasoning and Optimization (MAESTRO) framework in which multiple LLMs with specialized roles collaboratively discover high…

Large language model linguistic perplexity in childhood onset psychosis: unique features and developmental trends.

OBJECTIVE: Child and early adolescent onset psychosis (COP) is associated with subtle changes in language linked to thought disorder, a key contributor to functional impairment. Large language models (LLM) can detect deviations from expected language patterns by jointly analyzing sentence structure and word choice. This integrated information is captured by measures such as (pseudo)-perplexity, which quantify how difficult it is for an LLM to predict individual words given the surrounding linguistic context. The objective was to determine if perplexity measures change with age and whether they…

A reproducible optimisation protocol for calibrating prompt-based large language model workflows in evidence synthesis.

This methods article presents a reproducible workflow for optimising and testing improving large language models (LLMs) prompts in evidence-synthesis tasks with defined inputs and outputs. The method separates the fixed rules that define the scientific task from the editable prompt instructions that frame and apply them. It optimises those instructions against labelled or reference examples and an explicit task metric, then saves the final workflow with its specification, metric, settings, and evaluation traces so others can inspect and reuse it. The example code uses DSPy and GEPA tools, but …

Large Language Model-Based Text-to-Cypher Generation for Question-Answering on Biomedical Knowledge Graphs.

INTRODUCTION: Knowledge graphs are widely adopted due to their flexible structure and ability to represent large-scale, heterogeneous data. However, as these graphs grow, the queries used to retrieve information become more complex, making efficient information retrieval and question answering more challenging. OBJECTIVE: This work investigates (1) the impact of few-shot versus zero-shot prompting on Cypher query generation, and (2) performance differences between small (∼7B) and mid-size (∼32B) large language models (LLMs) on both of these approaches. METHODS: We propose a system that trans…

ProMem-agent: Procedural memory-augmented large language model agents for clinical trajectory reasoning.

BACKGROUND AND OBJECTIVE: Clinical large language model (LLM) agents can interpret current clinical context but have limited mechanisms for converting longitudinal experience into compact, reusable units. We developed ProMem-Agent, a procedural-memory framework that represents recurrent early intensive-care trajectories as provenance-linked observational patterns for retrospective mortality-risk estimation. METHODS: Adult ICU stays with at least 24 hours of observable data were represented as six consecutive four-hour state-action-response intervals. Memory candidates were extracted exclusive

Large Language Model-Assisted Thematic Coding in Medical Education Research: Comparative Methodological Study.

BACKGROUND: While large language model (LLM)-assisted qualitative analysis could improve the efficiency and scalability of feedback-driven curricular refinement in medical education, how best to leverage LLMs for qualitative analysis while ensuring quality outputs remains an open question. Prior work has demonstrated the feasibility of using LLMs for inductive and deductive coding tasks, but more needs to be known about how LLM-assisted thematic coding can best be deployed in a medical education context to maximize its strengths and guard against its weaknesses. OBJECTIVE: Our study evaluated

Large language model driven multicenter prediction and explainable risk attribution of acute kidney injury.

Acute kidney injury (AKI) represents a life-threatening condition among hospitalized patients, where early prediction enables prevention. Despite advances in existing models, clinical implementation remains hindered by excessive false positive rates (70%-94%) and lack of actionable clinical insights. We conduct a multi-center retrospective cohort study and develop a two-model large language model framework: AKI-PM (Prediction Model) for predicting AKI occurrence within 24 hours and AKI-RAM (Risk Attribution Model) for providing explainable risk attribution. Using a cohort of 140,637 hospital a

Accuracy, completeness and hallucinations in large language model generated statements and references for common rhinologic disorders.

This article critically discusses whether accumulation of lactic acid, or in reality lactate and/or hydrogen (H+) ions, is a major cause of skeletal muscle fatigue, i.e. decline of muscle force or power output leading to impaired exercise performance. There exists a long history of studies on the effects of increased lactate/H+ concentrations in muscle or plasma on contractile performance of skeletal muscle. Evidence suggesting that lactate/H+ is a culprit has been based on correlation-type studies, which reveal close temporal relationships between intramuscular lactate or H+ accumulation and
