BACKGROUND: Neglected or inadequately treated rigid congenitally deformed feet in older children are a nightmarish challenge for the child, the parents, and the orthopaedic surgeon. Because of the multiplicity of spatial deformities exhibited by these feet and legs, it was hypothesized that correction using the Taylor spatial frame (TSF) would decrease morbidity, facilitate correction, and minimize treatment time in children from remote regions with extremely rigid deformed feet.
METHODS: Recent experience with the management of 11 such feet (Dimeglio type IV) in 9 children with an average ag

BACKGROUND: A second staged operation using temporary bridging external fixation (TBEF) has been widely used in patients with periarticular complex fracture, yet few papers have been published on the related complications. The purpose of this study was to report the complication rate and pitfalls directly related to TBEF through a retrospective study and to suggest some solutions.
METHODS: Fifty-nine cases that were treated by using TBEF were studied among 195 periarticular complex fractures. We retrospectively collected the clinical and radiological data and then the study data was evaluated

Non-union of femoral neck fractures may occur due to mechanical and biological factors. Valgus intertrochanteric osteotomy (VITO) alters hip biomechanics and enhances fracture union. The double-angled 120° plate is usually used for internal fixation of the osteotomy. It allows the osteotomy to heal with medialisation and verticalisation of the femoral shaft. This deformity causes medial ligament strain of the knee joint, genu valgum and ultimately osteoarthritis. This work presents our experience in treating vertical fractures and non-unions of the femoral neck by VITO and fixation by a single

BACKGROUND: Progressive valgus deformity after proximal tibial metaphyseal fractures in pediatric patients has been well described, although the pathophysiology is not clear. We performed a retrospective study to evaluate the occurrence of progressive tibia valga after curettage of benign bone lesions of the tibia.
METHODS: During a 6-year period, we performed curettage and bone grafting of benign bone lesions of the tibia in 20 patients aged 16 years and younger. Radiographs and clinical notes were reviewed. The diagnosis, age, sex, location of lesion, surgical approach, physeal arrest, limb

AIM: Low-intensity pulsed ultrasound has been proven to accelerate fracture healing both clinically and experimentally. In this study the influence of low-intensity pulsed ultrasound during distraction-osteogenesis in case of delayed callotasis was investigated.
METHOD: 20 patients could be included in this study. 16 patients initially were treated because of fractures of the lower leg, 2 because of fractures of the femur with resulting shortening of the afflicted limb. One patient suffered from chronic osteitis at the thigh and one from chronic osteitis at the upper arm without previous trau

Limb preservation surgery compares favorably with amputation in oncologic and functional outcomes. However, an increased incidence of complications and reoperations is associated with limb preservation. This study compared the outcome of a group of young patients who required at least one reoperation after limb preservation surgery because of a complication or need for prosthetic lengthening with the outcome of a similar group who did not require additional surgery. The functional scores before and after reoperation are compared to determine the impact on reported function and patient satisfac

Congenital pseudarthrosis of the tibia (CPT) is one of the most challenging conditions in paediatric orthopaedics. The history of CPT treatment is characterised by repeatedly ineffective surgical interventions with the inevitable outcome of severe disability. Most procedures failed to account for the complexity of the problem. The past two decades have witnessed more experience having been gained with the Ilizarov technique. This method provides a comprehensive approach to all aspects of CPT problem, allowing the surgeon to simultaneously address the problems of union as well as those of defor

Coxa vara in childhood may be clinically classified as developmental, congenital, dysplastic, or traumatic and may occur at the physis or in the trochanteric or subtrochanteric area. Evaluation should include a search for a family history of similar deformity, a history of trauma or infection, and evidence of associated skeletal abnormality. Radiographs will illustrate whether the deformity is unilateral or bilateral and whether it occurs at or below the physis. With this information, coxa vara can be classified, and the optimal treatment can be selected. Surgical treatment of coxa vara in chi

A 33-year-old white man with a total hip arthroplasty presented with a malunion deformity of his femur following a fracture. Careful reapproximation of normal anatomy was achieved through a corrective angled osteotomy procedure. An attempt was made to reduce anticipated risks associated with future revision arthroplasty, and the patient's difficulties in ambulation were alleviated.

Twenty hip fractures in 19 children aged 2 1/2 to 18 were analyzed in a ten-year retrospective review at the University of Alabama Hospitals in Birmingham, Ala. There was one epiphyseal, one intertrochanteric, 4 subtrochanteric, and 14 intracapsular fractures. The mechanism of injury was usually severe trauma with 31% of the patients sustaining other associated major injuries. Avascular necrosis occurred in three intracapsular fractures (23%), all seemingly related to inadequate internal fixation. Coxa vara occurred in 30% of the patients; all were treated by closed methods, and delayed union

Subtrochanteric fractures are relatively rare in children and usually result from severe trauma. The unique biomechnical forces about the hip following fracture tend to complicate treatment. Remodeling and growth stimulation generally ensure good results by nonoperative means in children under 10 years of age. Open reduction should be considered in older children when good alignment cannot be achieved by closed methods.

BACKGROUND: Proximal tibial resection and reconstruction with a hinged knee megaprostheses may result in severe limb length discrepancy (LLD) in very young children because of the removal of the distal femoral and proximal physis. An alternative reconstruction using a proximal tibial hemiarthroplasty reconstruction has been proposed and reduces the degree of LLD because the distal femoral physis is preserved. However, there are very few reports on the results from this reconstruction, and it is not certain that the disadvantages of a more unstable knee are outweighed by the possibility of redu

BACKGROUND: Crowe type IV developmental dysplasia of the hip (DDH) represents the most severe form of hip dislocation and poses major technical challenges for total hip arthroplasty (THA). This study aimed to evaluate outcomes and complications of THA in Crowe type IV hips across multiple international centers, highlighting outcomes that may be relevant across practices in technique and implant selection.
METHODS: A retrospective review of 37 patients (49 hips) who had Crowe type IV DDH who underwent THA between 2013 and 2022 at four centers in three countries was conducted. Clinical and radi

INTRODUCTION: Total hip arthroplasty for developmental hip dysplasia is a challenging surgery due to anatomic abnormalities. Crowe III and Crowe IV hip dysplasia generally necessitates a subtrochanteric shortening osteotomy. Transverse and step-cut osteotomy are the most common procedures for shortening of femur although there is still no consensus which one is a superior method. The objective of this study was to demonstrate whether transverse or step-cut osteotomy is superior in hips who undergo arthroplasty for high riding hip dysplasia.
HYPOTHESIS: Our hypothesis was that higher rates of

Externally magnetic-controlled intramedullary telescopic nails for bone lengthening have recently gained popularity due to many advantages when compared to more traditional lengthening methods. Patients requiring lengthening often present with a clinical history of previous multiple surgeries increasing the risk for complications of further procedures. However, studies regarding the treatment of complications following implantation of these devices remain scarce in literature. Therefore, we report our experience with revision surgery after lengthening with a telescopic intramedullary lengtheni

BACKGROUND: Past reports have shown the tendency of posterior pelvic tilt to increase over time after total hip arthroplasty (THA). We have hypothesized that the volume and extent of released joint capsule may influence the change in postoperative pelvic tilt. This study was conducted to ascertain whether surgical approach during THA (posterior or anterior) has an effect on postoperative changes in pelvic tilt.
METHODS: We conducted a retrospective review of 81 hips in 60 patients. Patients classified as Crowe group 2, 3, and 4, those who had multiple previous compression fractures, and those

PURPOSE: Chipping and lengthening over nailing (CLON) technique was developed to treat femoral shaft nonunion with shortening more than 10 mm. The purpose of the current retrospective case series was to clarify the effectiveness of the CLON technique on the femoral shaft nonunion following intramedullary nailing.
METHODS: Clinical and radiological outcomes in the patients receiving operative treatment for femoral shaft nonunion between August 2012 and December 2016 were retrospectively reviewed using the Refractory Fracture Data Registry at the authors' institution. The CLON technique was ind

BACKGROUND: The conservative treatment of pediatric femoral fractures can be socially and financially burdensome for patients and families due to the long period of immobilization. New operative techniques have consistently gained significance in the treatment of such fractures due to a shorter period of immobilization. Elastic stable intramedullary nailing (ESIN) in particular has been proven to lead to better outcome in comparison to conservative treatment. This article presents the first study that compares the outcome of three versus two ESINs in the treatment of pediatric femoral shaft fr

This study investigated perspectives of 58 parents on leg length discrepancy (LLD) of their children using a questionnaire survey. There were significant differences in the pain/comfort (P=0.021) and global function subscales (P=0.013) of the pediatric outcomes data collection instrument between the idiopathic LLD and posttraumatic LLD groups. Significant differences in the items with regard to the satisfaction with appearance (P=0.012), preference for surgical treatment (P<0.001), effect on happiness (P=0.004), and concerns over possible operations (P=0.010) were observed between the LLD of 2
