The relationship between life adjustment and parental bonding in military personnel with adjustment disorder in Taiwan.

The aim of the present study was to examine the characteristics of military personnel with adjustment disorder to give them more appropriate treatment. The participants were 36 military personnel who met the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition, criteria of adjustment disorder as diagnosed by a psychiatrist at a teaching hospital in southern Taiwan. Another 24 persons were recruited as an age-matched control group. Each individual completed the clinical interview and the Wechsler Adult Intelligence Scale-Revised and then completed the questionnaires which inclu

Increased early life stress and depressive symptoms in patients with comorbid substance abuse and schizophrenia.

Early adverse events have been associated with increased rates of substance abuse and depression. To investigate the association between early adverse events and comorbid substance abuse in schizophrenia patients, early life stress, depressive symptoms, positive and negative symptoms, anxiety, and cognitive function were measured in an age-, sex-, and race-matched sample of 40 schizophrenia patients with and without comorbid substance abuse. Compared to patients without substance abuse, patients with schizophrenia and a history of substance abuse had a significantly higher incidence of early l

Chronic illness as a family process: a social-developmental approach to promoting resilience.

This paper describes a social-developmental approach to interventions in chronic illness using naturally occurring processes of change during family life-cycle transitions to promote more positive developmental outcomes. Clinical interventions can help build resilience by creating a therapeutic collaboration designed to help patients improve their use of existing and new resources in multiple systems. They can then better meet demands of the illness as it impacts on shared development. A case example of a 13-year-old daughter with complex, chronic health problems and developmental disabilities

[Psychosomatic aspects of idiopathic spasmodic torticollis. Results of a multicenter study].

Idiopathic spasmodic torticollis (IST) is one of the most frequent dystonic movement disorders. Its classification as a focal dystonia, as well as its treatment with botulinum toxin resulted in groups of patients being regularly seen by neurologic specialists. In a multicentre study, we investigated psychosocial changes, coping and psychopathology, and their interrelations with signs, symptoms and course. 256 patients were included in the study (59.3% women, 40.7% men). The mean age was 49.1 years. Rotating torticollis occurred more often than latero-retrocollis and antero-retrocollis. A famil

Gender differences in the onset of depression following a shared life event: a study of couples.

BACKGROUND: Gender differences in clinically relevant depression are well established, appear to be greatest in childbearing years and may be the result of gender differences in social roles. METHODS: A community sample of 100 couples who had recently experienced at least one threatening life event that was potentially depressogenic for both of them was studied using a semi-structured interviewer-rated interview. Onset of depression was assessed using the Present State Examination, and, rather than assuming that a gender difference in roles existed uniformly across the couples, they were char

Fear of personal death in adulthood: the impact of early and recent losses.

This study examines the association between the loss of significant others and fear of personal death in adulthood. The sample consisted of a group of 86 Israeli adults who had experienced a significant loss in childhood or adolescence and a matched group of 86 subjects who had not experienced any early loss. Participants in each group were further divided according to the experience of a significant loss during the previous 2 years and completed a multidimensional scale measuring fear of personal death. Whereas early loss was positively related to the interpersonal component of fear of death,

Life events and stress in patients with functional dyspepsia compared with patients with duodenal ulcer and healthy controls.

BACKGROUND: Life events and stress may be important for functional dyspepsia and duodenal ulcer. METHODS: The perception of life events in the preceding 6 months was recorded in 100 patients with functional dyspepsia, 100 patients with duodenal ulcer, and 100 healthy controls. In addition, psychologic and social factors were assessed. RESULTS: Patients with functional dyspepsia experienced significantly more life events than patients with duodenal ulcer and healthy controls. The difference in life events between the groups was due to the difference in stressful life events. The patients with

Treatment of posttraumatic stress disorder with eye movement desensitization.

This case report describes the successful treatment of a posttraumatic stress disorder (PTSD) using eye movement desensitization (EMD). The client, a 40-year-old male, presented with an 8-year history of PTSD following an incident in which he was shot with a hand gun and left dying. Using EMD treatment, this trauma was quickly desensitized. Two earlier traumas with similar themes then emerged and they too were desensitized. Test results, taken pre-treatment and posttreatment, along with the client's verbatim account of cognitive and behavioral changes 8 months later, converged to document the

Psychosocial aspects of hyperprolactinemia.

Fourteen mildly hyperprolactinemic women and 14 matched controls were compared with respect to biographical data, self-reported emotional and somatic well-being, and cortisol production. The patients had significantly more often experienced a separation from their parents in childhood. No differences were found for cortisol levels and emotional and somatic well-being. Future studies also should focus on personality, coping, and defense mechanisms in order to explain these contrasting findings.

Causal attributions in first-degree relatives of depressed and agoraphobic inpatients.

Causal attributions in 77 first-degree relatives of major depressed, agoraphobic, major depressed and agoraphobic (comorbid), and dysthymic inpatients were studied. The hypothesis that the relatives of comorbid and dysthymic patients should exhibit more "depressogenic" causal attributions for bad events than the relatives of major depressed and agoraphobic patients was partly supported. Relatives of dysthymic patients attributed bad events to more internal causes and assigned more importance to the events themselves than did relatives of major depressed patients. Relatives of patients with maj

[Psychogenic reaction: course and prognostic factors].

A follow-up was made of ninety patients with a diagnosed psychogenic reaction (adjustment disorder) fourteen years after the index hospitalisation. Contrary to expectation, at the follow-up only half of the patients showed no symptoms of the illness. Approximately a quarter of the patients suffered from a more serious illness (drug dependence, schizophrenia or organic mental disorder) than the index diagnosis. The other quarter showed symptoms of a psychogenic disorder (neurotic disorder, personality disorder, adjustment disorder). A number of factors which describe the course of the illness l

Relation of criminal victimization to health perceptions among women medical patients.

The relation of criminal victimization to health perceptions (self-rated current health) was determined among women health maintenance organization patients. Data were survey responses from 2,291 women (45% response rate), 57% of whom had experienced crime. Reliability was evaluated by assessing 241 respondents both by survey and by interview. Data were analyzed by hierarchical multiple regression, which indicated that criminal victimization was an important predictor of health perceptions even after accounting for the contributions of demographics and other stressful life events with known li

Differentiating major depression from adjustment disorder with depressed mood in the medical setting.

Although the psychiatric consultant in the general hospital setting is frequently called on to distinguish major depression from adjustment disorder, no studies to date have examined whether the two diagnoses are in fact distinguishable. Analysis of computerized data base records from 944 cases seen by psychiatric consultants from 1981-1987 revealed 59 cases of major depression and 130 cases of adjustment disorder with depressed mood. Patients with major depression were more likely to be older (p less than 0.001), widowed (p less than 0.001), and living alone (p less than 0.005). Patients with

The course of depression in elderly outpatients.

Sixty-four elderly outpatients diagnosed according to the criteria of DSM-III as having affective disorders were submitted to follow-up study for 6-24 months (mean 15 months). Twenty patients (31%) were in good health throughout follow-up, fourty-four patients (69%) remained more or less chronically ill. The prognosis was better in patients diagnosed as having "adjustment disorder with depressed mood" and "dysthymic disorder"; when onset was late, and when there were no signs of organic brain C.N.S. damage.

Assessing life stressors and social resources: applications to alcoholic patients.

A growing body of evidence points to the importance of life stressors and social resources in the development and course of alcoholism and other substance abuse disorders. This article describes the Life Stressors and Social Resources Inventory (LISRES), which provides an integrated assessment of life stressors and social resources in eight domains: physical health, home/neighborhood, financial, work, spouse/partner, children, extended family, and friends. The indices were developed on data obtained at two points in time 18 months apart from four demographically comparable groups: alcoholic pa

Life events in neurological patients with headache and low back pain (in relation to diagnosis and persistence of pain).

The series comprised 253 hospitalized patients with 3 groups of patients with low back pain differing in respect to the presence of nerve root compression, and 2 headache groups, i.e., migraine and other non-organic headache. Comparison of the frequency of single life events within the previous 12 months revealed no statistically significant differences among the diagnostic groups. Nor were there any statistically significant differences among the means of the total number of life events, the number of life events with transient distress and the number of life events with enduring distress. Long-term effects of losing a spouse or child in a motor vehicle crash.

In this article we examine the long-term effects of the sudden, unexpected loss of a spouse or child. In the spouse study, interviews were conducted with 39 individuals who had lost a spouse in a motor vehicle crash 4 to 7 years ago and with 39 matched controls. In the parent study, interviews were conducted with 41 parents who had lost a child in a crash and with 41 matched controls. Control respondents were matched to bereaved respondents case-by-case on the basis of sex, age, income, education, and number and ages of children. Significant differences between bereaved spouses and controls we
