Caring for the dying in a developing country, how prepared are we?

End of life care is framework to allow for a peaceful, comfortable and dignified death while considering the patients' personal and religious values, bioethics and knowledge of the disease process. A well planned end of life pathway should allow for the flexibility to shift from an active (or aggressive) treatment approach to one of comfort and care when initial interventions have failed. The need for this pathway is most apparent in the intensive care setting. Implementation of a pathway will face various challenges due to religious and cultural beliefs, education of healthcare providers to c

Management of malignant bowel obstruction in patients with advanced cancer at the end of life.

BACKGROUND: Malignant bowel obstruction in patients with advanced cancer at the end of life is common. Patients may have substantial symptoms (pain, nausea and vomiting) and experience aggressive care at the end of life. Due to the lack of robust evidence, the care algorithm of treatment for patients at the end of life is not standardized. Patient's prognosis, clinical condition and patient's preferences related to goals of care discussion must be considered when opting between comfort-focused care (conservative/pharmacological treatment), palliative (nonsurgical) procedures and palliative sur

Making a measurable difference in advanced Huntington disease care.

Neurologists' role in the care of people with advanced Huntington disease (HD) (total functional capacity <7), often limited by a lack of clinical research to support good practice, includes the following: (1) provide comprehensive health records to an interdisciplinary care staff before admission to a more intense care setting (home health services, day program, assisted living, group home, long-term skilled nursing facility, palliative care); (2) consult with and refer to rehabilitation (occupational therapy, physical therapy, speech and language pathology), behavioral and psychiatric profes

Ethical aspects in the care of a child with infantile spinal muscular atrophy (SMA).

The pediatrician has a privileged relationship with a child with infantile spinal muscular atrophy (SMA). At all times, he/she must be the child's mentor, promoting a comprehensive approach and support in order to ensure the best possible solution for the patient's autonomy. In all circumstances, an ethical stance is essential. After a reminder on the notions of ethics of care, we will address various ethical questions encountered through three critical situations during the care of a child with infantile spinal muscular atrophy: the announcement of the diagnosis, the transmission of informati

Empowering community volunteers improves the well-being of family caregivers of musculoskeletal disorder patients: a randomized controlled trial.

BACKGROUND: Musculoskeletal (MSK) disorders cause major global disability and strain caregivers' mental health. In China, aging and limited rehabilitation amplify this burden. Empowering community volunteers through structured training may provide emotional and practical support to family caregivers, yet its effectiveness for MSK caregiving remains insufficiently studied. METHODS: A randomized controlled trial was conducted in a coal-mining community. Eighty-eight family caregivers were randomly assigned to an intervention group, which received regular home visits from trained community volun

Romanian male patients with the dual diagnosis of schizophrenia and alcohol use disorder: a prospective study of clinical, social, and treatment-related factors affecting quality of life.

BACKGROUND: Schizophrenia frequently co-occurs with alcohol use disorder (AUD), resulting in a complex clinical profile associated with poor functional outcomes and reduced quality of life (QoL). Although both conditions independently impair psychosocial functioning, few studies have examined the combined effects of clinical, social, and treatment-related factors on QoL in patients with this dual diagnosis. METHODS: This prospective observational study included 88 male inpatients diagnosed with schizophrenia and comorbid AUD and who were followed over a 6-month period. Quality of life was ass

High-Commitment Human Resource Management and Care Workers' Attitudes and Performance Through a Sustainable-Careers Lens: A Multilevel Analysis in Japan.

In aging societies, securing a stable long-term care workforce is a priority, yet workforce policies centered on financial incentives have demonstrated limited effectiveness. Drawing on the sustainable-careers perspective as an analytical lens, this study re-examines high-commitment human resource management (HCHRM), grounded in ability-motivation-opportunity (AMO) theory, by investigating how it relates to the work attitudes and work role performance of long-term care workers, with work-life balance (WLB) support incorporated as an opportunity-enhancing practice. Data of 875 full-time care wo

Stakeholder views on integrating prehabilitation in neoadjuvant therapy for breast cancer.

BACKGROUND: Neoadjuvant systemic therapy (NAST) for breast cancer can cause fatigue, neuropathy, and sleep disturbance, affecting treatment adherence and recovery. Early supportive care-including exercise, psychological support, and integrative therapies-may reduce side effects and improve outcomes. However, it remains unclear how best to deliver such care to optimise patient participation and effectiveness. This study explored the perspectives of women with breast cancer and healthcare professionals (HCPs) on the design and delivery of a multimodal supportive care program during NAST. METHOD

Effectiveness of Family-Focused End of Life and Bereavement Interventions Across Neonatal, Paediatric, and Adult Intensive Care: An Overview of Reviews.

BACKGROUND: Intensive Care Units (ICUs) care for critically ill patients across all age groups, with many deaths occurring during or shortly after admission. For families, this can be associated with significant psychological distress, including prolonged grief, depression, and post-traumatic stress. Family experiences are influenced by the quality of end-of-life communication, involvement in decision-making, and the availability of bereavement support. Despite this, bereavement care across neonatal, paediatric, and adult ICUs remains inconsistent, highlighting the need for evidence-based stra

Amyotrophic lateral sclerosis (ALS): three letters that change the people's life. For ever.

Amyotrophic lateral sclerosis (ALS) is a neurodegenerative disease affecting the motor nervous system. It causes progressive and cumulative physical disabilities in patients, and leads to eventual death due to respiratory muscle failure. The disease is diverse in its presentation, course, and progression. We do not yet fully understand the cause or causes of the disease, nor the mechanisms for its progression; thus, we lack effective means for treating this disease. Currently, we rely on a multidisciplinary approach to symptomatically manage and care for patients who have ALS. Although amyotro

The role of palliative care for patients with left ventricular assist devices: a narrative review.

BACKGROUND AND OBJECTIVE: Left ventricular assist devices (LVADs) have revolutionized the care of patients with advanced heart failure (HF). Compared to guideline-directed medical and device therapies, LVAD technology improves quality of life and reduces mortality. Palliative care specialists have an important role to play in the pre-LVAD evaluation phase, in the post-operative longitudinal care phase, and at the end-of-life in patients with LVADs. The objective of this narrative review is to describe the evidence regarding the role of palliative care for patients with LVAD across the care con

A pharmacologic review of cardiac arrest.

Cardiac arrest is manifested by arrhythmias (ventricular fibrillation or pulseless ventricular tachycardia, pulseless electrical activity, or asystole) resulting in minimal to no forward blood flow to the body's oxygen-dependent tissues. Defibrillation and cardiopulmonary resuscitation (CPR) should be initiated immediately as they have been shown to increase return of spontaneous circulation and survival to discharge rates. Cardiac arrest in the surgical patient population has devastating consequences. Data specific to the surgical patient found that 1 in 203 surgical patients experienced card

Integrative Supportive Care Trial to Enhance Physical Activity in Malignant Pleural Effusion-Protocol for the ISC-TEAM/AMPLE-5 Randomised Controlled Trial.

BACKGROUND: Malignant pleural effusion (MPE) heralds advanced cancer with poor patient outcomes. High symptom burden of weight loss, fatigue, breathlessness, distress, and anxiety leaves many patients unable to participate in desired activities of daily life and is associated with poor outcomes. Freedom from symptoms and maintaining daily activities are the key wishes of patients and carers. For the best outcome, accessible services aimed at prevention and management of malnutrition, care of psychological well-being, and support for increasing physical activity are required. METHODS: The Inte

Implementing a quality improvement programme in palliative care in care homes: a qualitative study.

BACKGROUND: An increasing number of older people reach the end of life in care homes. The aim of this study is to explore the perceived benefits of, and barriers to, implementation of the Gold Standards Framework for Care Homes (GSFCH), a quality improvement programme in palliative care. METHODS: Nine care homes involved in the GSFCH took part. We conducted semi-structured interviews with nine care home managers, eight nurses, nine care assistants, eleven residents and seven of their family members. We used the Framework approach to qualitative analysis. The analysis was deductive based on th

Implementation outcomes of a dementia-focused intervention for family care partners and clinicians in home hospice care.

OBJECTIVES: End-of-life care for persons living with dementia in home hospice relies heavily on coordination between family care partners (FCPs) and clinicians (e.g., hospice social workers and nurses). FCPs and clinicians have reported support and knowledge gaps in end-of-life dementia care. Interventions are needed to improve FCPs' support and clinicians' educational gaps. METHODS: A pilot randomized controlled trial was designed to examine implementation outcomes for a dementia-focused end-of-life intervention for FCPs (n = 37) and clinicians (n = 15). [Rheumatic diseases in childhood and adolescence: importance of early multi-professional care].

Juvenile idiopathic arthritis (JIA) is the most common chronic inflammatory rheumatic disease in children. Both the disease, which is accompanied by pain and movement restrictions, and its usually complex, long-term therapy, are associated with burdens for those affected until adulthood. Impairments can affect many areas of life. Holistic care that considers the preferences and expressed needs of patients and their families is crucial in order to achieve the desired therapeutic goals of remission and the best possible quality of life. Since JIA begins in a very early phase of life, long-term c
