Ridge augmentation using mandibular ramus bone grafts for the placement of dental implants: presentation of a technique.

Local bone grafts are a convenient source of autogenous bone in alveolar reconstruction. The mandibular ramus area provides primarily a cortical graft that is well-suited for veneer-grafting of ridge deficiencies prior to implant placement. The advantages of this method of augmentation include its intraoral access and low morbidity. Similar to bone harvested from the mandibular symphysis, these grafts require short healing periods, exhibit minimal resorption, and maintain their dense quality. Advantages of this donor site over the chin include minimal patient concern for altered facial contour

The use of medical modeling in microvascular free tissue transfer reconstruction with osseointegrated implantation in complex midface defects.

Osseointegrated implants in conjunction with microvascular free tissue transfer reconstruction has been increasingly adopted in our head and neck cancer population. However, the majority of the literature on dental rehabilitation in free flaps have been for mandibular defects. Midface and maxillectomy defects are challenging defects for reconstruction. The utilization of medical modeling technology has allowed for improved efficiency and accuracy of microvascular free tissue transfer reconstruction of these midface defects and opened the possibility of immediate osseointegrated implant placeme

Reconstruction of TORS oropharyngectomy defects with the nasoseptal flap via transpalatal tunnel.

The nasoseptal flap (NSF) has been described as reconstructive option for soft palate defects following transoral robotic surgery (TORS). As described, this technique is does not provide adequate coverage of the lateral oropharyngeal wall, parapharyngeal space, exposed vessels, or exposed mandibular bone. The NSF for TORS reconstruction has been limited to soft palate reconstruction, given the limitations on length when passed via the nasopharynx. In this article, we describe (1) a novel technique for TORS reconstruction using direct transposition of the ipsilateral NSF into the oropharynx via

Solitary plasmocytoma of the mandible--a combined approach for treatment and reconstruction.

A 44-year-old man presented with a solitary plasmocytoma in the left mandible. After neoadjuvant radiation therapy and chemotherapy, a bone marrow biopsy was taken for cryopreservation. In simulation surgery, tumour resection and primary reconstruction were planned on an individual stereolithographic 3-D skull model. Radical tumour resection and primary reconstruction using a microvascular iliac crest bone graft was performed. In case of extensive osteolysis and/or mandibular asymmetry due to tumourous bone enlargement, preoperative model planning has proved to be a useful tool for primary rec

Mandibular bone defect healing using polylactic acid-nano-hydroxyapatite-gelatin scaffold loaded with hesperidin and dental pulp stem cells in rat.

Addressing mandibular defects poses a significant challenge in maxillofacial surgery. Recent advancements have led to the development of various biomimetic composite scaffolds aimed at facilitating mandibular defect reconstruction. This study aimed to assess the regenerative potential of a novel composite scaffold consisting of polylactic acid (PLA), hydroxyapatite nanoparticles (n-HA), gelatin, hesperidin, and human dental pulp stem cells (DPSCs) in a rat model of mandibular bone defect. The PLA-HA-GLA composite was synthesized using solvent casting-leaching and freeze-drying methods and subs

Mandibular Osteoradionecrosis: Mandibular Preservation Using Humeral Periosteal Free Flap Wrapping.

BACKGROUND: Management of mandibular osteoradionecrosis (ORN) is challenging and often requires large surgical resection. Alternatively, mandibular preservation and wrapping with a periosteal free flap may achieve healing in selected patients. Our objective was to assess the success rate and morbidity of revascularization of mandibular ORN using a humeral periosteal free flap. METHODS: A retrospective study was performed using medical records of patients who had undergone humeral periosteal free flap reconstruction for mandibular ORN. Clinical data, including age, gender, ORN site, ORN grade

Early secondary closure of alveolar clefts with mandibular symphyseal bone grafts and beta-tri calcium phosphate (beta-TCP).

Alveolar reconstruction of bony defects in cleft lip and palate patients is a widely accepted treatment regimen for which multiple donor sites can be used. For 25 years, autogeneous bicortical mandibular symphyseal bone grafts have been used at the authors' centre. In cases in which the alveolar defect was too large to match the volume of the mandibular symphyseal bone transplant, beta-TCP granules were packed against the bone transplant to fill the defect completely. In a retrospective study, 18 patients, who were treated with mandibular symphyseal bone wrapped in beta-TCP granules, were comp

Mandibular fossa morphology during therapy with a fixed functional orthodontic appliance : A magnetic resonance imaging study.

OBJECTIVE: During therapy of distoclusion entailing a rigid, fixed orthodontic appliance, the mandibular fossa and condyle are ideally remodeled, while dentoalveolar effects occur through adaptive mechanisms. Adaptive processes, especially in the fossa region, have not been adequately investigated. Our magnetic resonance imaging (MRI) investigation aimed to assess the effects of therapy with a functional mandibular advancer (FMA) on mandibular fossa morphology. PATIENTS AND METHODS: We monitored via MRI the therapeutic course of 25 patients at three time points. Visual findings and metric ass

Imaging in traumatic mandibular fractures.

A fracture of the mandible is a common trauma presentation amongst young males and represents one of the most frequently encountered fractured bones within the viscerocranium. Historically, assault was the dominant contributing factor but now due to the increased number of vehicles used per capita, motor vehicle accidents are the primary cause. Mandibular fractures can be classified anatomically, by dentition, by muscle group and by severity. The fracture may also be closed, open, comminuted, displaced or pathological. It is important that the imaging modality used identifies the classificatio
