Computer-assisted contouring combined with bone ostectomy for dental implant placement of craniofacial fibrous dysplasia involving the right maxilla.

OBJECTIVE: Craniofacial fibrous dysplasia affecting the jaws can result in facial asymmetry, posing a unique challenge for dental implant placement in edentulous areas. This case report introduces an innovative approach for rehabilitating the posterior maxilla affected by craniofacial fibrous dysplasia using a computer-guided technique for maxillary recontouring and substitution bone grafting at implant sites. CASE PRESENTATION: A 58-year-old edentulous woman with a craniofacial fibrous dysplasia bone lesion affecting the right maxilla was referred for correction of asymmetry and dental impla…

Spread asymmetry to differentiate nasopalatine duct cysts from radicular cysts arising in the anterior maxilla on computed tomographic images.

OBJECTIVE: The aim of this study was to clarify numerical values for differentiating nasopalatine duct cysts (NPDCs) from radicular cysts (RCs) arising in the anterior maxilla on computed tomography (CT) or cone-beam CT (CBCT) images. METHODS: CT or CBCT images of histologically proven NPDCs (n = 30) and RCs (n = 33) beyond the midline of the maxilla were investigated to determine two asymmetry indices on axial images of the maximum lesion area. The lateral asymmetry index was calculated based on two distances from each of the lateral ends of the lesion to the midsagittal plane. The index was…

Comparative evaluation of dental implants in posterior maxilla placed using unicortical and bicortical anchorage-A split-mouth prospective study.

BACKGROUND: The use of dental implants has become a very predictive method of rehabilitation for patients with partial or complete edentulism. It is more challenging to treat the posterior quadrants of the maxillary ridges using dental implants due to their anatomical and physiological characteristics. So to overcome the limitations of other techniques, short implants were introduced recently as a new approach to simplify implant placement in compromised alveolar bone and to prevent possible damage to vital structures. PURPOSE: This study aims to compare the clinical outcomes of dental implan…

Lateral Sinus Floor Elevation in the Severely Atrophied Maxilla: Concentrated Growth Factors Versus Bone Substitutes. A Controlled Clinical Trial.

The present study clinically and radiographically compares the outcome of implants inserted in maxillary sinuses augmented with concentrated growth factors (CGFs) or demineralized bovine bone matrix (DBBM) in a one-stage lateral approach. In 20 patients with a residual bone height of 1 to 4 mm, lateral sinus floor elevation was performed, using CGFs or DBBM as the sole grafting material, with simultaneous implant placement. Outcome variables were implant and prosthesis failures, complications, subjective satisfaction, and radiographic changes in marginal bone level (MBL) 12 months after surger…

Soft Tissue Closure of Grafted Extraction Sockets in the Anterior Maxilla: A Modified Palatal Pedicle Connective Tissue Flap Technique.

Localized ridge resorption, the consequence of socket collapse, following tooth extraction in the anterior maxilla can adversely affect esthetics, function, and future implant placement. Immediate grafting of extraction sockets may help preserve natural ridge contours, but a lack of available soft tissue can compromise the final esthetic outcome. The presented modified rotated palatal pedicle connective tissue flap is a useful technique for simultaneous soft tissue coverage and augmentation of grafted sockets to improve esthetic outcome. This article delineates its advantages through the prese…

Tissue Engineering Therapy for Atrophic Maxilla Using Minimally Manipulated CD90 and CD105 Bone Marrow Stem Cells: A Case Report.

Bone deficiencies in the jaws of edentulous patients limit the possibility of successful placement and osseointegration of endosseous implants. Bone resorption is defined as a vertical and horizontal loss of quality and quantity in the residual bone ridges; lack of bone structure requires bone grafting procedures to enable implant placement and to improve the function, predictability, and longevity of implants. The objective of this case study was to demonstrate the feasibility of a novel cell therapy treatment for localized jaw bone defects with tissue repair cells composed of a mixture of bo

Anterior maxilla alveolar ridge dimension and morphology measurement by cone beam computerized tomography (CBCT) for immediate implant treatment planning.

BACKGROUND: Implants have been widely used to restore missing teeth. Limited information on applied anatomy at the anterior maxilla compromises the clinical outcome for implant placement in this region. In the current study, Cone Beam Computerized Tomography (CBCT) was used to measure alveolar ridge and buccal undercut dimension at the anterior maxilla to help develop treatment planning for immediate implant placement. METHODS: CBCT scans were screened to include 51 subjects with full dentition at right maxilla. Measurements were taken at the cross sectional views in the middle of the maxilla…

Soft tissue closure of grafted extraction sockets in the posterior maxilla: the rotated pedicle palatal connective tissue flap technique.

The sequelae of socket collapse and localized ridge resorption after tooth extraction in the posterior maxilla can adversely affect esthetics, function, and future implant placement. Immediate grafting of extraction sockets may help to preserve natural ridge contours; but lack of available soft tissue can prevent primary closure or exert tension that hampers graft turnover and compromises the visible gingival scallop. Exposed barrier membranes may also be susceptible to bacterial infection that may lead to secondary graft failure. The rotated pedicle palatal connective tissue flap is a relativ…

Long-term results of segmental repositioning of the maxilla in cleft palate patients without previously grafted alveolo-palatal clefts.

Eleven patients (9 UCLP, 2 BCLP) were treated with segmental osteotomies with or without osteotomies at the Le Fort I level and simultaneous bone grafting of the alveolo-palatal clefts at adult age. These patients were clinically and radiographically evaluated after a mean follow-up period of 59 months (range 39-110 months). One patient showed complete dentoalveolar relapse, whereas the skeletal stability after miniplate fixation proved to be adequate in all cases. Only one patient presented with a persisting oro-nasal fistula. In six cases, the alar base asymmetry had improved to such an exte

Synergistic effects of intermittent parathyroid hormone (1-34) and masticatory force for alveolar bone remodeling in the maxilla of dogs.

INTRODUCTION: Parathyroid hormone (PTH) is a key regulator of calcium homeostasis and bone remodeling. While intermittent PTH (1-34) (teriparatide) is an established treatment for osteoporosis, its effects on alveolar bone regeneration remain unclear. This study evaluated intermittent PTH (1-34) impact on the microstructure and remodeling of canine maxillary alveolar bone under physiological masticatory force. METHODS: In this exploratory pilot study, three male mongrel dogs were each assigned to a single dose group (n = 1 per group) and injected weekly for 20 weeks with vehicle (CON), 1.6 μg…

How Far Can We Push the Limit in Palliative Surgery? A Case Report of Massive Chondroblastic Osteosarcoma of the Maxilla.

Surgical management of head and neck sarcomas presents significant challenges, particularly in advanced stages where curative options are limited. In such cases, palliative care becomes essential to alleviate symptoms and enhance the patient's quality of life. When chemoradiation therapy fails to provide adequate symptom control, palliative surgery may be a viable option. We report the case of a 24-year-old male diagnosed with an extensive and aggressive maxillary chondroblastic osteosarcoma. Despite multiple chemoradiation regimens, the disease progressed rapidly. Due to the substantial deter…
