Nasal Airway Evaluation After Le Fort I Osteotomy Combined With Septoplasty in Patients With Cleft Lip and Palate.

Septal deviation constitutes an important component of both esthetic deformity and airway compromise in patients with cleft lip and palate (CLP). The posterior parts of the nasal septum presented greater deviation than the anterior parts in patients with complete unilateral CLP. Le Fort I down-fracture provides better access to the nasal septum than intranasal incision during rhinoplasty, especially to the posterior part. This study objectively and subjectively evaluated the nasal function after Le Fort I osteotomy combined with septoplasty in patients with complete unilateral CLP. Twenty-thre

Does Collagen Membrane Coverage Offer any Advantage for Lateral Window Sinus Augmentation? A Histologic and Histomorphometric Analysis.

BACKGROUND: We aimed to determine whether collagen membrane coverage in maxillary sinus floor elevation surgery, provides an advantage regarding bone regeneration. METHODS: This randomized clinical trial included all healthy adults presented for dental implant placement in the posterior edentulous maxilla at the Maxillofacial Surgery Department of Mashhad Dental School, Mashhad, Iran from 2021-2022. Participants were candidates for sinus floor elevation surgery. Patients were randomly allocated to control (with membrane) and test (without membrane) groups. Surgery was performed through the la

Are We Correctly Measuring Mandibular Stability After Surgery-First Approach? A Comparative Study Based on Novel 3-Dimensional Measurements.

Using traditional measures to assess mandibular stability after the surgery-first approach (SFA) may produce inaccurate results because unlike the conventional orthodontic-first approach (OFA), the main dental movements occur after surgery in SFA, which produce unavoidable mandibular movements, especially in cases with postsurgical premature dental contact. As these movements are part of the surgical-orthodontic plan, they should not be considered an actual relapse. In this study, to avoid postsurgical dental movement effects, the authors used the relationship between proximal and distal mandi

Associated relaps factors in Le Fort I osteotomy. A retrospective study of 54 cases.

PURPOSE: The Le Fort I osteotomy (LFI) procedure is commonly used to restore morpho-functional balance. The goal of maxillofacial surgeons with this procedure is to achieve occlusal stability. To identify factors associated with relapse after maxillary advancement in cleft lip and palate patients, the one-year post-operative stability of Le Fort I osteotomy was evaluated. METHODS: Horizontal and vertical relapse were analysed on lateral cephalograms by retrospectively using tracing paper in an orthonormal landmark in 54 patients undergoing unilateral cleft lip and palate surgery who were moni

Revisiting major anatomical risk factors of maxillary sinus lift and soft tissue graft harvesting for dental implant surgeons.

The anatomical variations of the maxillary sinus septa, greater palatine artery, and posterior superior alveolar arteries might cause unexpected complications when they are damaged. Dentists who know these structures well might hope to learn more practical knowledge to avoid and assess injury preoperatively. Therefore, this review paper aimed to review the reported anatomy and variations of the maxillary sinus septa, greater palatine artery/nerve, and posterior superior alveolar artery, and to discuss what has to be assessed preoperatively to avoid iatrogenic injury. To assess the risk of inju

Management of the nasolacrimal system during transnasal endoscopic medial maxillectomy.

BACKGROUND: During transnasal endoscopic medial maxillectomy (TEMM) for sinonasal tumors, the nasolacrimal duct (NLD) is resected as it forms part of the medial wall of the maxillary sinus. This disrupts the nasolacrimal drainage system. To ensure that epiphora does not develop after such resection, an endoscopic dacryocystorhinostomy (DCR) is traditionally performed. This is based primarily on prior experience with open medial maxilloethmoidectomy. It is unclear whether a concurrent DCR during endoscopic medial maxillectomy is necessary, however. This study was designed to compare patients un

Stability of large maxillary advancements using a combination of prebent and conventional plates for fixation.

OBJECTIVE: The purpose of this study was to determine the stability of one and two jaw maxillary advancements fixed with prebent plates and "L"-shaped plates. STUDY DESIGN: Twenty-six patients met the inclusion criteria (group 1: one jaw [14]; group 2: two jaws [12]). Radiographs were digitized before surgery (T1), immediately after surgery (T2), at 6 weeks (T3), and at 6 months (T4). Stability was assessed comparing A-point movement and change in palatal plane. t tests were used for comparisons between groups, and linear regression analysis was used to assess the stability of A-point T2-T4 v

Evaluation of the Pterygomaxillary Separation Pattern in Le Fort I Osteotomy Using Different Cutting Instruments.

PURPOSE: Pterygomaxillary separation (PMS) is considered the main reason for serious complications associated with Le Fort I osteotomy. The aim of this study was to evaluate whether a piezo surgery, ultrasonic bone scalpel, or conventional bur used in Le Fort I osteotomy has an influence on PMS patterns. MATERIALS AND METHODS: Using a retrospective cohort study design, we enrolled a sample composed of patients who underwent orthognathic surgery. The primary predictor variables were the cutting instruments (ultrasonic bone scalpel, piezo surgery, and Lindeman bur) used to perform Le Fort I ost

The importance of the anterior nasal spine in predicting soft tissue movement in maxillary orthognathic surgery.

BACKGROUND: The aim of this study was to compare and analyze the correlation between changes in maxillary hard tissue landmarks and nasolabial soft tissue points in patients with skeletal Class II and Class III malocclusion following Le Fort I osteotomy. METHODS: From 2017 to 2022, patients who underwent Le Fort I osteotomy were recruited in this retrospective study. Three hard tissue landmarks on the maxilla and twelve soft tissue landmarks on the nose and lips were selected for measurement. A Pearson correlation coefficient was used to analyze the coefficient correlations between hard- and

Technical aspects of nasal cavity surgery through the Le Fort I down-fracture approach: An otolaryngologist's point of view based on 90 patients' experience.

BACKGROUND: The downfracture access to septoand turbinoplasty during maxillary osteotomy may be recommended in many cases. One or both of these laryngological interventions may be necessary when, after the patient's clinical evaluation, either an impaired function of nasal breathing or a deviated septum are present. The main postsurgical risk of the procedure is the destabilization of the cartilaginous septum position and its relation to adjacent anatomical structures, a change in the shape of the nose and the presence of a supratip break. OBJECTIVES: In this paper, the authors present their …

A Combined Approach To Crooked Nose Deformity.

BACKGROUND: Crooked nose deformity is a complex anatomical deformity; therefore, repair remains a challenge. Here, we introduce a new technique for crooked nose deformity repair. MATERIALS AND METHODS: Twenty-two patients underwent intraoperative evaluations and measurements, followed by surgery using the open rhinoplasty technique. Our method involves three major steps. First is the creation of an asymmetric medial oblique osteotomy using piezoelectric ultrasonic surgery and double lateral osteotomies, wherein the asymmetric bony ascending part of the maxillary bone at the base of the nasal

Unusual late vascular complications of sagittal split osteotomy of the mandibular ramus.

Intraoperative or early postoperative vascular complications are not uncommon problems in sagittal split osteotomies of the mandibular ramus; however, reports of late complications are considerably rarer. Here, we present two patients who sustained late vascular complications after the sagittal split osteotomy. The first patient had a delayed bleeding, which presented itself as a rapidly expanding swelling of the left cheek from the left external carotid artery 18 days postoperatively. During exploration, a 2 mm laceration of the external carotid artery located just proximal to the bifurcation
