OBJECTIVES: To characterize the variability in the times catheters take to block with encrustation in patients who have Proteus in their urinary flora, and to identify factors responsible for modulating the rate of catheter encrustation and blockage.
PATIENTS AND METHODS: Twenty patients were followed prospectively for > or = 12 weeks, with a bacteriological analysis on weekly urine samples. The pH of the voided urine samples and the pH at which crystals formed in them (the nucleation pH) were determined. Catheters were collected and examined for bacterial biofilm and crystal deposition.

Brain abscesses are rare in infants and their clinical presentation is specific for this age group. Seven cases of brain abscess in infants aged 2-11 months are reported. The underlying cause was meningitis in four, sepsis in two, and unknown in one. Gram-negative organisms were cultured in 6 patients. The abscess size was 5 cm or more in five cases; in four there were multiple lesions. The primary aim of this study was to clarify the mechanism and significance of lipopolysaccharide (LPS)-induced translocation of intestinal bacteria. Female Swiss Webster mice were purchased from two different animal suppliers. Twenty-four hours following parenteral LPS, mice from both suppliers had noticeable morbidity and mortality. Although nonclostridial gas gangrene is not an unusual occurrence, relatively few cases including the distal lower extremity have been reported. Due to the serious nature of some of these infections, it is important for physicians to familiarize themselves with these nonclostridial crepitant infections, which are often confused with clostridial myonecrosis. Etiology, evaluation, and treatment is described by the authors. Also, a case study is presented.

Acetohydroxamic acid was administered in 5 patients to determine its effect in reducing urinary catheter incrustations. All patients had chronic indwelling catheters that required frequent changes because of severe incrustations and catheter occlusion. Incrustations were analyzed chemically for calcium, magnesium, ammonia nitrogen and phosphorus. A model was designed to study the relation between urinary infection and vesicoureteral, low-grade, total reflux. Surgical alteration of the ureterovesical junction produced grade 2 vesicoureteral reflux in seven of 13 animals. In the other six animals, surgery had rendered the vesicoureteral junction marginally competent and chronic bladder infection caused grade 2 reflux. It was found that grade 2 vesicoureteral reflux in the adult monkey neither prolongs urinary tract infection nor causes continued renal infection.
