The administrative organizations of the Ernakulam and Malappuram Districts' family planning programs during 1970-74 and the ways they dealt with typical problems of program organization are examined. Lack of personnel, poor staff morale, inadequate supplies, and political and religious opposition to various contraceptive methods, especially sterilization, existed to varying degrees in both districts' programs. The Ernakulam experience, involving mass sterilization camps that were part of an overall district development program, documents the effectiveness of a strong central leader. The Malapp…

There is a move away from a market economy in health care in the United States and a move towards such a market in Germany. This article tries to make explicit what underlies the moral intuition that there is a tension between a market economy and health care. First, health care is analyzed in terms of the economic theory of the market and incompatibilities are described. The moral problem is identified as the danger of liquefying the distinction between persons and things. The basic moral intuition seems to be the classical social contract: as a functioning market is governed by the principle

Contemporary nursing practice increasingly takes place within culturally, religiously, and ethically plural healthcare environments. Although professional standards provide a shared ethical foundation, limited conceptual attention has been given to how nurses' religious moral identities influence ethical reasoning during cross-gender care. Existing discussions of professionalism remain largely grounded in secular ethical perspectives, leaving insufficient exploration of how faith-based moral traditions may contribute to professional identity and moral agency. This paper examines gender interac

The phenomenon of euthanasia and/or assisted suicide (EAS) in unjust conditions has received much attention in the popular press, along with a handful of responses in academic venues. These responses typically defend the permissibility of EAS in unjust conditions by appeal to anti-discrimination or harm reduction arguments, which frame the issue in terms of exclusion from current EAS law. We first briefly examine these arguments within such a framing, and then argue that it fails to account for the deep moral concern elicited by EAS in unjust conditions. To address this moral concern more full

BACKGROUND/INTRODUCTION: The timeframe from delivery of critical care and the transition to comfort care for the dying child can occur over minutes, days, weeks, or months. During this transition from curative to comfort care, nurses can play an active role in the management of vulnerable dying children and their families.
OBJECTIVES/AIMS: The aims of this study were (1) to explore the actions, communication techniques, and interventions utilized by morally resilient registered nurses (RNs) and nurse practitioners (NPs) as they guide families through the transition from curative to comfort ca

BACKGROUND: Decision-making surrounding cardiopulmonary resuscitation (CPR) presents persistent ethical challenges in acute and critical care. Existing evidence points to ambiguous do-not-resuscitate pathways, moral distress, family-driven demands, and systemic pressures, yet these influences are often examined separately. This review synthesizes current evidence to clarify the ethical, relational, and organizational dynamics that influence CPR-related decisions.
METHODS: We conducted a structured narrative review of four databases (Web of Science, PubMed, Scopus, Embase) for studies publishe

BACKGROUND: Using extracorporeal membrane oxygenation (ECMO) in paediatric and neonatal intensive care units (PICU/NICU) creates ethical challenges and carries a high risk for moral distress, burn out and team conflicts.
AIM: The study aimed to gain a more comprehensive understanding of the underlying factors affecting moral distress when using ECMO for infants and children by examining the attitudes of ECMO nurses.
METHODS: Four focus groups discussions were conducted with 21 critical care nurses working in a Swiss University Children's Hospital. Purposive sampling was adopted to identify r

The preceding article in this issue of the Kennedy Institute of Ethics Journal presents the argument that "moral fundamentalism," the position that international bioethics rests on "basic" or "fundamental" moral prinicples that are universally accepted in all eras and cultures, collapses under a variety of multicultural and postmodern critiques. The present article looks to the contractarian tradition of Hobbes and Locke -- as reinterpreted by David Gauthier, Robert Nozick, and John Rawls -- for an alternative justification for international bioethics. Drawing on the central themes of this tra

BACKGROUND: Stigma remains a major barrier to mental health care in low- and middle-income countries (LMICs), yet little is known about how stigma operates for common mental illnesses (CMIs) and alcohol use disorder (AUD) in rural Uganda.
METHODS: We conducted 80 in-depth interviews (IDIs) with individuals and family members affected by CMIs (n=34) or AUD (n=46) in Buyende District, Uganda. Interviews were analyzed inductively using the framework method to identify cross-cutting themes.
RESULTS: Three interlocking themes emerged: Internal Dysphoria, Community Dynamics, and Family Burden. Int

BACKGROUND: Depression in elderly people may be contributed to by the multiple losses of aging. Exercise has the potential to positively impact many of these losses simultaneously. We tested the hypothesis that progressive resistance training (PRT) would reduce depression while improving physiologic capacity, quality of life, morale, function and self-efficacy without adverse events in an older, significantly depressed population.
METHODS: We conducted a 10-week randomized controlled trial of volunteers aged 60 and above with major or minor depression or dysthymia. Subjects were randomized fo…

This paper explores organ donation through the perspective of Reconstructionist Confucianism. I argue that for organ donation in China to be morally permissible, public policy must conform to the norms of Confucian benevolence. Reconstructionist Confucianism appreciates benevolence as an objectively important feature of morality deeply connected to moral rules governing propriety, integrity, righteousness, and human freedom. Here, benevolence involves sincere affection for another as an intrinsic good, rather than as a means to achieve other purposes. It requires developing self-restraint and

Four systems for analyzing thinking about 2 personal-life dilemmas, as discussed by 29 men and 35 women (ages 35-85), were compared. Kohlberg's (1976) moral judgment stages, Kegan's (1982) ego-development stages, Gilligan's (1982) moral orientation system, and Suedfeld and Tetlock's (1977) integrative complexity scoring were used. Subjects completed Kohlberg's (Colby & Kohlberg, 1987) standard moral judgment measure, a self-concept description, and several questionnaires. The Kohlberg, Kegan, and integrative complexity codings of the dilemmas were positively related to each other and to the st

OBJECTIVES: This study examined the effects of two service-learning experiences on the psychosocial and moral reasoning development of occupational therapy students. The assumption was that ethical reasoning ability can be facilitated through participation in value-laden experiences.
METHOD: Participants visited older adults in nursing homes (n = 19) or interacted with persons with disabilities in community settings (n = 33). All participants reflected on their experiences through weekly journals. Psychosocial and moral reasoning development were measured at the beginning and end of the exper

INTRODUCTION: Humble leadership is increasingly recognized as a catalyst for organizational adaptability. Although prior research has examined several cognitive, relational, and affective mechanisms underlying humble leadership, less is known about whether humble leadership evokes specific self-transcendent moral emotions, such as elevation, that motivate followers to engage in risky but prosocial voice behavior. Departing from traditional exchange-based leadership perspectives, this study draws on Affective Events Theory to conceptualize humble leadership as a "moral event" that elicits eleva
